Book excerpt: We propose a method for using instrumental variables (IV) to draw inference about causal effects for individuals other than those affected by the instrument at hand. Policy relevance and external validity turns on the ability to do this reliably. Our method exploits the insight that both the IV estimand and many treatment parameters can be expressed as weighted averages of the same underlying marginal treatment effects. Since the weights are known or identified, knowledge of the IV estimand generally places some restrictions on the unknown marginal treatment effects, and hence on the values of the treatment parameters of interest. We show how to extract information about the average effect of interest from the IV estimand, and, more generally, from a class of IV-like estimands that includes the two stage least squares and ordinary least squares estimands, among many others. Our method has several applications. First, it can be used to construct nonparametric bounds on the average causal effect of a hypothetical policy change. Second, our method allows the researcher to flexibly incorporate shape restrictions and parametric assumptions, thereby enabling extrapolation of the average effects for compliers to the average effects for different or larger populations. Third, our method can be used to test model specification and hypotheses about behavior, such as no selection bias and/or no selection on gain. To accommodate these diverse applications, we devise a novel inference procedure that is designed to exploit the convexity of our setting. We develop uniformly valid tests that allow for an infinite number of IV--like estimands and a general convex parameter space. We apply our method to analyze the effects of price subsidies on the adoption and usage of an antimalarial bed net in Kenya.

However, they fundamentally rely on the well-known monotonicity (threshold-crossing) condition on treatment choice behavior. Recent research has shown that this condition cannot hold with multiple instruments unless treatment choice is effectively homogeneous. Based on these findings, we develop a new marginal treatment effect framework under a weaker, partial monotonicity condition. The partial monotonicity condition is implied by standard choice theory and allows for rich heterogeneity even in the presence of multiple instruments. The new framework can be viewed as having multiple different choice models for the same observed treatment variable, all of which must be consistent with the data and with each other. Using this framework, we develop a methodology for partial identification of clearly stated, policy-relevant target parameters while allowing for a wide variety of nonparametric shape restrictions and parametric functional form assumptions. We show how the methodology can be used to combine multiple instruments together to yield more informative empirical conclusions than one would obtain by using each instrument separately. The methodology provides a blueprint for extracting and aggregating information about treatment effects from multiple controlled or natural experiments while still allowing for rich heterogeneity in both treatment effects and choice behavior.

It formulates a more complete lexicon of evidence-based recommendations and support shared, ethical decision making by doctors with their patients. Diagnostic and therapeutic technologies continue to evolve rapidly, and both individual practitioners and clinical teams face increasingly complex ethical decisions. Unfortunately, the current state of medical knowledge does not provide the guidance to make the majority of clinical decisions on the basis of evidence. The present research infrastructure is inefficient and frequently produces unreliable results that cannot be replicated. Even randomized controlled trials (RCTs), the traditional gold standards of the research reliability hierarchy, are not without limitations. They can be costly, labor intensive, and slow, and can return results that are seldom generalizable to every patient population. Furthermore, many pertinent but unresolved clinical and medical systems issues do not seem to have attracted the interest of the research enterprise, which has come to focus instead on cellular and molecular investigations and single-agent (e.g., a drug or device) effects. For clinicians, the end result is a bit of a “data desert” when it comes to making decisions. The new research infrastructure proposed in this book will help the medical profession to make ethically sound and well informed decisions for their patients.

The first paper presents an approach to sensitivity analysis for the instrumental variable (IV) method, which examines the sensitivity of inferences to violations of IV validity. Our approach is based on extending the Anderson-Rubin test and is robust to weak IVs. The second paper presents a unified R software ivmodel for analyzing instrumental variables with one endogenous variable. The package implements a general class of estimators, k-class estimators, and two confidence intervals that are fully robust to weak instruments. The package also provides power formulas. The sensitivity analysis discussed in the first paper is also included in the package. The third paper uses Hidden Markov Model to estimate the dynamic effects of lottery-based incentives towards patient's healthy behavior every day. The data is collected from randomized clinical trials.

Book excerpt: Instrumental variables (IV) are widely used in economics to address selection on unobservables. Standard IV methods produce estimates of causal effects that are specific to individuals whose behavior can be manipulated by the instrument at hand. In many cases, these individuals are not the same as those who would be induced to treatment by an intervention or policy of interest to the researcher. The average causal effect for the two groups can differ significantly if the effect of the treatment varies systematically with unobserved factors that are correlated with treatment choice. We review the implications of this type of unobserved heterogeneity for the interpretation of standard IV methods and for their relevance to policy evaluation. We argue that making inferences about policy-relevant parameters typically requires extrapolating from the individuals affected by the instrument to the individuals who would be induced to treatment by the policy under consideration. We discuss a variety of alternatives to standard IV methods that can be used to rigorously perform this extrapolation. We show that many of these approaches can be nested as special cases of a general framework that embraces the possibility of partial identification.

When such a goal involves an active intervention, such as the assignment of a particular treatment variable to participants, the analysis of its effects requires the tools of causal inference. In such settings, the opportunity to use experimental or observational data to tease out policy parameters of interest requires a combination of statistical and causal assumptions. In reduced form work, where an explicit economic theory is not laid out to allow identification of policy parameters from data, the investigation of the causal assumptions becomes a critical exercise for the credibility of the results. Many robustness exercises evaluate the effect that relaxing and/or modifying assumptions produces on the results of the study. The scope of these exercises is very broad, reflecting the need to tailor specific robustness exercises to whichever assumptions are most likely to be violated in a given domain. This dissertation is a collection of three essays on robust causal inference that share a unifying theme: preserving the nonparametric nature of the robustness exercise. This aspect has both a theoretical and practical relevance. First, causal assumptions are usually nonparametric: robustness exercises that restrict to parametric cases might lead to misleading insights. Further, economics research has started to incorporate more flexible nonparametric and semi-parametric techniques which may call for robustness exercises that are readily applicable to these approaches. Because robustness exercises are context specific, each of these essays addresses a separate aspect of it. Chapter 1 investigates how changes in the distribution of covariates may invalidate given experimental results, with implications for evidence based policy-making. It proposes an explicit metric of robustness that measures the distance of the closest distribution of covariates for which experimental results are violated. Chapter 2 analyses the practice of robustness checks as a way to validate a researcher's identification strategy. It details out the limitations of these exercises in detecting failure of identification and proposes a non-parametric robustness test that bypasses functional form assumptions. Finally, Chapter 3 focuses on the robustness of Marginal Treatment Effect identification when the instrumental variables fail to incentivize treatment for a subset of the population. It provides two alternative identification results which can be relevant in practice.

Median regression, as introduced in the 18th century by Boscovich and Laplace, is a special case. In contrast to conventional mean regression that minimizes sums of squared residuals, median regression minimizes sums of absolute residuals; quantile regression simply replaces symmetric absolute loss by asymmetric linear loss. Since its introduction in the 1970's by Koenker and Bassett, quantile regression has been gradually extended to a wide variety of data analytic settings including time series, survival analysis, and longitudinal data. By focusing attention on local slices of the conditional distribution of response variables it is capable of providing a more complete, more nuanced view of heterogeneous covariate effects. Applications of quantile regression can now be found throughout the sciences, including astrophysics, chemistry, ecology, economics, finance, genomics, medicine, and meteorology. Software for quantile regression is now widely available in all the major statistical computing environments. The objective of this volume is to provide a comprehensive review of recent developments of quantile regression methodology illustrating its applicability in a wide range of scientific settings. The intended audience of the volume is researchers and graduate students across a diverse set of disciplines.

Book excerpt: There is a growing interest in estimating heterogeneous treatment effects in randomized and observational studies. However, most of the work relies on the assumption of ignorability, or no unmeasured confounding on the treatment effect. While instrumental variables (IV) are a popular technique to control for unmeasured confounding, there has been little research conducted to study heterogeneous treatment effects with the use of an IV. This dissertation introduces methods using an IV to discover novel subgroups, estimate their heterogeneous treatment effects, and identify individualized treatment rules (ITR) when ignorability is expected to be violated. In Chapter 2, we present a two-part algorithm to estimate heterogeneous treatment effects and detect novel subgroups using an IV with matching. The first part uses interpretable machine learning techniques, such as classification and regression trees, to discover potential effect modifiers. The second part uses closed testing to test for statistical significance of each effect modifier while strongly controlling the familywise error rate. We apply this method on the Oregon Health Insurance Experiment, estimating the effect of Medicaid on the number of days an individual's health does not impede their usual activities by using a randomized lottery as an instrument. In Chapter 3, we generalize methods to identify ITR using a binary IV to using multiple, discrete valued instruments, or equivalently, multilevel instruments. Several new problems arise when generalizing to multilevel instruments, requiring novel solutions. In particular, multilevel IV give rise to many latent subgroups that may experience heterogeneous treatment effects. Additionally, it may be unclear how to combine and compare the different levels of the IV to estimate treatment heterogeneity. We provide methods that use a prediction of the latent subgroup to identify optimal ITR, and methods to dynamically combine levels of the multilevel IV to estimate the heterogeneous treatment effects, effectively individualizing estimation of an ITR. Further, we provide and discuss necessary and sufficient conditions to identify an optimal ITR using a multilevel IV. We apply our methods to identify an ITR for two competing treatments, carotid endarterectomy and carotid artery stenting, on preventing stroke or death within 30 days of their index procedure.
