The older adult has become vulnerable to the ever present breakdowns in the healthcare system creating serious gaps in services. Society and social services have not maintained the continual rapid steady growth of services to the older adult to match the increased life span of Americans. As Rennke (2013) reports, "Patients are vulnerable to a wide range of adverse events after discharge, with more than 20% of medical patients sustaining a preventable adverse event within three weeks of discharge" (p.433). Ineffective care transitions have many contributing factors including lack of crucial communication between health care providers, unclear medication changes at time of health status change, patient perspective on medical diagnosis, lack of adequate follow-up needs including physician visits, and incomplete or unfinished diagnostic work-ups.

Book excerpt: Evidence-based guidelines for discharge instructions, when successfully administered, reduce readmission rates in patients, leading to improved quality of life and economic savings. Unfortunately, effective delivery is complex and time consuming, placing a high demand on already overworked bedside nurses. Failure to provide complete discharge instructions can result in non-adherence to patient treatment regimens and lack of essential follow-up, the most commonly identified reasons for readmissions. To improve quality of care, hospitals need to adopt a new model that incorporates delivery of intensive, one on one education to patients during the hospital stay with continuing support, guidance, and education throughout the transition from hospital to home. The purpose of this study was to determine the relationship between the education provided by a staff registered nurse and a clinical patient educator, a registered nurse who specializes in specific disease processes. An exploratory descriptive study design was used to examine and describe the perception of registered nurses regarding discharge education provided by a clinical patient educator who specializes in disease processes versus that provided by a staff registered nurse. An Independent samples t test was conducted comparing the mean of each survey question for those working in acute care to those not working in acute care. Only two survey questions' mean answers were statistically significantly different between the two groups. Those nurses not working in acute care felt more strongly that they spent, on average, more than 30 minutes with discharge teaching than those working in acute care. Those nurses working in acute care felt more strongly that they spent less than 30 minutes with discharge teaching than those nurses not working in acute care.

With one exception, the remaining articles describe individual EBP projects from five different health care facilities that used the Iowa Model to guide their work. Each article includes an evidence summary, a description of implementation strategies, an evaluation of the innovation, and lessons learned. These completed projects were initiated between 2009 and 2012, address a variety of topical nursing issues, and, for the most part, focus on preventing complications (ie, blood sugar elevations, increased lengths of stay, extubation failures, noise-related injury, pain, surgical site infections, pneumonia, restraint use, delirium, and fever). An additional article describes the use of evidence to inform simulation-based learning, a possible strategy for ensuring competencies in and compliance with EBP interventions.
