Book excerpt: While the systemic health consequences of silicone gel-filled breast implants have been explored in several large studies, little attention has been placed on factors associated with implant failure (defined as silicone gel on the outside of the implant and commonly referred to as implant rupture). This study explores woman-specific and implant-specific characteristics potentially associated with silicone breast implant failure. The study population consists of 441 women (807 implants) who received MRI examination to diagnose the integrity of their implants (i.e., failed versus intact) between 1992 and 1995 at the UCSD MRI Institute and who responded to a mailed survey in 1996 (79% response rate). Information included the woman's demographic characteristics and personal factors. Medical records provided information about the patient's implants and her implant surgery. Multivariate analysis using the Generalized Estimating Equation (GEE) approach was conducted in order to control for confounding and simultaneously take into account the correlation between factors from two implants within the same woman. Thirty percent of the implants had failed. Meme ME polyurethane-coated implants were identified as having a much greater risk of failure than other implants (86% compared to 27%), and failure occurred in this sub-group at a much younger implant age. Among non-Meme ME implants, increasing age and having had a previous implant were strong predictors of implant failure. Implants placed in the sub-muscular position were at higher risk of failure compared to sub-glandular implants, and smaller implants were more likely to fail than larger ones. Wearing a bra during exercise (always or most of the time) appeared to increase the risk of failure if the implants were sub-glandular or hard. Hardness was a risk factor for implant failure, but only among women who wore a bra during exercise. The risk of failure for a "typical" patient with a 10-year old implant was estimated to be 17%. The results of this study offer guidance to women with breast implants and their health care providers who are questioning their risk of silicone breast implant failure
