May it Please Your Honour: In presenting its ninth annual report the Provincial Board of Health is gratified to be able to record the fact, that during the past year there has not been a single outbreak of smallpox. The Province has not been equally fortunate in escaping the visitation of other zymotic diseases. Thus in Toronto, with an estimated population of 178,000, typhoid fever caused 117 deaths, diphtheria 71, measles 18, and scarlet fever 14. The total deaths for the year were 2,919, the ratio per 1,000 of population 16.39, and the zymotic death-rate 3.43. Now the zymotic death-rate, or in other words, the mortality from preventable diseases is a measure whereby one may test the purity of a municipality's water supply, its freedom from nuisances, and the fidelity with which notification, isolation and disinfection are attended to in resisting the attacks of contagious disease within its borders. The postulate of hygiene is: given pure air, particularly in inhabited places, purity of water supply, prompt action in giving notice of the existence of zymotic disease, immediate isolation of patients and exposed persons, with the necessary disinfection of persons and things, and zymotic disease will soon cease to appear in any locality. The nearer a municipality approaches to the conditions of the postulate, the freer will it be from diseases of this class. Measured by this standard, Toronto with its zymotic mortality of 3.43 maybe considered one of the healthiest cities in the Dominion, the zymotic death-rate of Quebec for 1890 being 9.88 per 1,000, Montreal 7.08, Halifax 5.82, Ottawa 5.36, Winnipeg 4.89, St, John, N.B., 3.49, Hamilton 3.49, and London 2.09. And yet if we compare even the best of these, that of London, Ontario, with 1.5 the zymotic death rate of Kensington, a London parish having a population of 178,000, we gain some idea of the extent of our own shortcomings, and also of the influence which hygiene can and does exert even in densely populated cities, in preventing the spread of contagious diseases, and consequently preserving the lives of the people. It is evident, therefore, that if we would attain to the highest degree of sanitary well-being, the people of Toronto as well as other Ontario municipalities must strengthen the hands of their Local Boards of Health, and see to the appointment of competent medical health officers whose duty it will be to look narrowly into existing defects, and by advising their removal, accomplish much actual good, while at the same time raising the standard of hygienic endeavor among the people. In this Province, as in other countries, about 85 percent, of all deaths from diphtheria are of children under ten years of age. Among grown people the mortality is much less. This disease, though not one which ordinarily appears in epidemic form, nevertheless, though endemically present in most populous districts, has appeared in localized outbreaks, and, owing to its great fatality and painful symptoms which it exhibits, is one to which the attention of bacteriologists was early directed. So early as 1884, Carle and Cattani discovered virus in tetanus that could be inoculated from men to animals but it was not until 1889 that the. Tetanus bacillus was obtained in pure cultures by Kitasato, and als by Tizzoni and Cattani. As an anaerobe, which was spore-bearing. These spores are exceedingly resistant even to moist heat; but, fortunately, the germs do not develop in free air. Its presence, however, in the earth of gar dens and streets in various localities, made it' very desirable that investiga tions be carried on to discover if possible its antitoxin. Behring, Kitasato, Ehrlich and Tizzoni labored at this for some years, and by 1890 the fact of an immunity being established against the disease through the injection of a toxin of graduated strength, was confirmed. Its utility has hitherto been limited by the fact that the quantity of antitoxic serum required for a suc cessful result is at least two thousand times as great after the symptoms of tetanus have appeared, as compared with that required when an injection has been made antecedent to inoculation with the disease. This fact, asso ciated with the further fact that the tetanus germ goes on developing in an ordinary punctured wound without any sign of its presence till the onset of the spasms caused by it, makes any chance of treating the disease suc cessfully by the antitoxin relatively slight. The very complete knowledge which we now possess of the cause of tetanus, and the. Conditions under which it develops have, however. Enabled the surgeon when called upon to treat punctured wounds, to take such steps at the time the wound occurs as will preclude in large measure any danger of its development. I am sure that you all are equally glad with myself, of the large amount of good practical work which we have been able to accomplish during the year 1904, and that you will be equally pleased with the very satisfactory condition of the health of this fair Province during that period. For twenty-two long years we have been striving to make our land one of the healthiest places on this earth, and I am sure that the following report of the year just closed will bear me out in saying, that in no year have we had better results than in the last. The health of the people has been fair, and I now will place before you, the statistics, of a few of the contagious show what has been done with them. Scarlet fever, had its greatest mortality during the months of January, February, and December, being 24, 13, and 15 respectively While the least was in the months of May, June and September, being 7, 5, and 7. The mortality for the year reached 129, while the preceding year it was four times greater, reaching 529.
