Matériel et méthode : tous les enfants de 6 mois à 3 ans accueillis dans les crèches municipales de la ville de Nantes entre septembre 2016 et mai 2018 ont bénéficié d'un examen orthoptique comprenant une étude des reflets cornéens, de la motilité oculaire, de la réaction à l'occlusion, un test à l'écran alterné, la recherche d'un nystagmus, d'une anomalie anatomique, d'une anisocorie et une mesure de la réfraction non cycloplégiée par auto-réfractomètre portatif (Plusoptix). Résultats : les principaux facteurs de risque d'amblyopie retrouvés sont les anomalies réfractives (60 à 80% des cas) et notamment l'astigmatisme. À Nantes, il existe un dépistage préverbal de l'amblyopie réalisé dans les crèches municipales de la ville depuis 2012. Notre étude a pour objectif l'évaluation rétrospective de 2 années non consécutives de ce dépistage. Nos résultats montrent qu'une combinaison de tests est nécessaire pour ne pas méconnaître des situations pouvant entraîner une amblyopie, mais la multiplication des tests augmente le nombre d'enfants dépistés positifs. Le dépistage réalisé a permis de diagnostiquer sur deux années 117 troubles visuels chez l'enfant de moins de 3 ans.

Book excerpt: Objectif : comparer la valeur prédictive positive (VPP) du dépistage collectif des troubles visuels et des troubles auditifs en fonction de l'âge chez les enfants scolarisés entre trois et cinq ans. Méthodes : les enfants, inclus dans les écoles maternelles, étaient nés en 2011 et avaient eu un dépistage pathologique, visuel avec le test Cadet-images ou auditif avec l'audiométrie à balayage de fréquences, par le service de PMI de la Vendée. Les résultats de référence, obtenus à partir des consultations ophtalmologiques ou oto-rhino-laryngologiques, représentaient 75,5 % des dépistages. Deux groupes étaient formés en fonction de l'âge au moment du dépistage : le groupe A (enfants de trois à quatre ans) et le groupe B (enfants de quatre à cinq ans). Résultats : pour le dépistage des troubles visuels, la VPP du groupe B (77,6 %) était supérieure à la VPP du groupe A (65,6 %). Pour le dépistage des troubles auditifs, la VPP du groupe A était (58 ,7 %) supérieure à la VPP du groupe B (45,7 %). Conclusion : les enfants du groupe A étaient moins coopérants lors du dépistage collectif des troubles visuels et auditifs et ils présentaient davantage de pathologies ORL bénignes, le plus souvent spontanément régressives, que les enfants du groupe B. Dans une logique de dépistage efficace, précoce et rentable, un bilan de santé chez les enfants de trois ans et huit mois à cinq ans et demi, scolarisés en moyenne section de maternelle, aurait un intérêt individuel et économique tout en incluant un maximum d'enfants à une période clé de leur développement.

Le dépistage des troubles visuels chez les enfants de la naissance à 6 ans fait partie des objectifs de prévention attribués aux médecins généralistes, pédiatres de ville, professionnels de santé de PMI et d'établissement scolaire. Les anomalies du développement visuel concernent 10 à 15 % de la population des moins de 6 ans et conduisent en l'absence de prise en charge à une amblyopie dans 30 % des cas avec des conséquences sur la qualité de vie à l'âge adulte et un impact économique défavorable pour la société en raison du risque accru de malvoyance. Cette étude s'est intéressée aux habitudes des médecins généralistes et internes en médecine générale d'Ile-de-France, en matière de dépistage des troubles visuels des enfants de 0 à 6 ans. Matériel et méthodes. Le questionnaire a été distribué sur 2 groupes privés de médecins sur un réseau social, après plusieurs relances, 92 réponses ont été récoltées de décembre 2021 à avril 2022. 8 réponses ne correspondaient pas au critère d'inclusion d'exercice en Ile-de-France. Résultats. Dans cette étude, 90,1 % des participants sont des femmes, 81,3 % exercent en milieu urbain et la moyenne d'âge est de 31 ans. 47,8 % des personnes interrogées déclarent ne pas faire de dépistage des troubles visuels chez l'enfant. Les principales raisons évoquées sont l'absence de matériel de dépistage dans 80,9 % des cas, l'absence de formation spécifique dans 80,9 % des cas, le manque de temps dans 36,2 % des cas, le manque d'organisation adéquate dans 21,3 % des cas. Conclusion. Le manque de formation et le manque de rémunération spécifique semblent être deux facteurs décisifs pour la mise en place d'un dépistage à large échelle. Devant le désintéressement des pouvoirs publics, il semble important d'améliorer les pratiques de dépistage des médecins généralistes ; une formation dédiée pourrait être instaurée dans les facultés lors de l'internat idéalement sous la forme d'ateliers pratiques, et/ou lors des stages de médecine chez le praticien, ces derniers semblant être les moments les plus adaptés à cette formation. Une autre piste peu envisagée consisterait à transférer des compétences à des professionnels de santé spécifiquement formés, ce qui libérerait dans le même temps les médecins généralistes d'une charge, à une époque où elles se font de plus en plus nombreuses.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
