Book excerpt: All 11 Member States of the WHO South-East Asia Region have sustained country-wide access to DOTS. Each year more than 2 million TB cases are registered for treatment and the treatment success rate among new smear-positive pulmonary TB cases has remained above 85% since 2005 and was 89% in the 2011 cohort. the TB mortality rate has decreased by more than 40% since 1990 and the South-East Asia Region is on track to achieve the global target of a 50% reduction by 2015. the decline in prevalence is observed in all Member States and in some it is over 50% since 1990. Approximately 39% of the estimated global number of cases occurs in the Region (based on current estimates) as well as almost 30% of the multidrug resistant TB burden. the collaboration between TB and HIV control programmes is improving. Many of the constraints to effective implementation of TB control services in Member States relate to the need to strengthen and better finance national health systems in general many of which are already overstretched in terms of both infrastructure and staffing. to enable universal access and continue scaling-up of critical interventions there is an urgent need to sustain current financial commitments and to advocate for additional financial resources.

Book excerpt: This report updates information regarding the extent of tuberculosis and the progress made by Directly Observed Treatment Short-course (DOTS) programmes within the WHO Western Pacific region. This is based on case notification and treatment data from national TB programmes of 33 countries and areas (which accounts for a population of 1.702 billion) for 2001. Findings include that 811,263 cases were notified for all types of TB, with 379,060 new smear-positive pulmonary cases; 20 per cent of notified cases were in the 25-34 year age group; and the seven countries with the highest regional TB rates were Cambodia, China, Laos, Mongolia, Papua New Guinea, the Philippines and Vietnam.

Book excerpt: The status of the tuberculosis (TB) epidemic and progress in control of the disease has been assessed annually since 1997. This report presents an overview of progress in reducing the burden of TB in the Western Pacific Region using data on disease burden and case notification in 2008 and treatment outcomes for patients registered in 2007. The data were collected from 36 countries and areas in the Region using the WHO standard form for reporting surveillance data. The report contains data trends on estimated TB prevalence and mortality, TB notifications by age group and sex, TB/HIV co-infection and other indicators of progress, including treatment outcomes and laboratory capacity and quality. This report presents data on multidrug resistant-TB from various drug resistance surveys from the countries and areas in the Region. The specific country profiles of countries with a high burden of TB provide country-specific information on progress of TB control. TB data and trends in the Pacific island countries and areas are also included in the report. A directory of partners in the countries with a high burden of TB in the Region is appended.

Module 2: screening – systematic screening for tuberculosis disease is an updated and consolidated summary of WHO recommendations on systematic screening for tuberculosis (TB) disease, containing 17 recommendations for populations in which TB screening should be conducted and tools to be used for TB screening. TB screening is strongly recommendations for household and close contacts of individuals with TB, people living with HIV, miners exposed to silica dust, and prisoners. In addition, screening is conditionally recommended for people with risk factors for TB attending health care, and for communities with risk factors for TB and limited access to care (e.g. homeless, urban poor, refugees, migrants). General population screening is recommended in high-burden settings (0.5% prevalence or higher). Symptoms, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests for TB are recommended as screening tools for all adults eligible for screening. Computer-aided detection programmes are recommended as alternatives to human interpretation of CXR in settings where trained personnel are scarce. For people living with HIV, C-reactive protein is also a good screening tool. This guideline document is accompanied by an operational handbook, the WHO operational handbook on tuberculosis. Module 2: screening – systematic screening for tuberculosis disease, that presents principles of screening, steps in planning and implementing a screening programme, and algorithm options for screening different populations.

Book excerpt: Between 2011 and 2019, WHO has developed and issued evidence-based policy recommendations on the treatment and care of patients with DR-TB. These policy recommendations have been presented in several WHO documents and their associated annexes, including the WHO Consolidated Guidelines on Drug Resistant Tuberculosis Treatment, issued by WHO in March 2019. The policy recommendations in each of these guidelines have been developed by WHO-convened Guideline Development Groups, using the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach to summarize the evidence, and formulate policy recommendations and accompanying remarks. The present WHO Consolidated Guidelines on Tuberculosis, Module 4: Treatment - Drug-Resistant Tuberculosis Treatment includes a comprehensive set of WHO recommendations for the treatment and care of DR-TB. The document includes two new recommendations, one on the composition of shorter regimens and one on the use of the BPaL regimen (i.e. bedaquiline, pretomanid and linezolid). In addition, the consolidated guidelines include existing recommendations on treatment regimens for isoniazid-resistant TB and MDR/RR-TB, including longer regimens, culture monitoring of patients on treatment, the timing of antiretroviral therapy (ART) in MDR/RR-TB patients infected with the human immunodeficiency virus (HIV), the use of surgery for patients receiving MDR-TB treatment, and optimal models of patient support and care. The guidelines are to be used primarily in national TB programmes, or their equivalents in Ministries of Health, and for other policy-makers and technical organizations working on TB and infectious diseases in public and private sectors and in the community.
