It is based on the Emergency Update 2008 of Guidelines for Programmatic Management of Drug-resistant Tuberculosis, and may be considered a companion document to these guidelines. It also draws on the experience of the international health NGO Partners In Health (PIH) in many countries. This module should be introduced to health workers in the context of a training course with a strong emphasis on TB-HIV co-management.

World-wide, the burden of these infectious diseases are huge, especially in the 22 countries with a high TB burden, where more than 80% of TB world-wide occurs and in sub-Saharan Africa, where 67% of the world's HIV/AIDS cases occurred. Despite different ways of transmission and the fact that several separate national programs often exist, tuberculosis and HIV/AIDS are linked. They are often affecting similar population groups (e.g. vulnerable and other minorities groups) and HIV is the most potent risk factors in increasing the risk of progression from latent TB infection (LTBI) to disease among the two billions individuals estimated with LTBI world-wide, while the relative risk of HIV is over 20. Therefore, it is crucial to combine and co-ordinate the efforts when addressing TB and HIV issues. Diverse responses have been developed, both at international and country levels for tackling the different health and social components of these diseases.

Bovine Tuberculosis (BTB), caused by Mycobacterium Bovis, is a well-known zoonotic and economically important disease which represents a serious health threat for livestock, wildlife and humans. BTB aggravates the 'triple trouble' of HIV/AIDS and TB infection and malnutrition. More specifically, in Ethiopia, even if the extent of its transmission to human beings has been given less attention and not yet been well researched, this scientific work has plausibly found out that the significant arise in the incidence of extra-pulmonary TB, the high prevalence of TB in cattle, isolation of M. tuberculosis complex from animal tissue and milk, the lower awareness of cattle owners about the disease, close contact of cattle and human, the prevailing habit of consumption of raw milk and meat of the community, and increasing HIV epidemics suggests that the impact of M. bovis on human TB could be significant.

The constant knowledge generation that expands the frontiers of understanding is a key factor for finding solutions and successful activities for public health.

Book excerpt: Tuberculosis (TB) remains a major global public health threat and host genetic factors are known to modulate the outcome of exposure to M. tuberculosis. We studied the genes NRAMP1, which has been associated with TB susceptibility in several ethnic groups, and IL12RB1, that plays a major role in Mendelian Susceptibility to Mycobacterial Disease (MSMD) and found no evidence for association with pulmonary TB in an adult Moroccan population. Thus, complex susceptibility to TB disease in this population seems to be governed by other mechanisms than Mendelian susceptibility in children, which might explain the different clinical presentations and physiopathological pathways in these different age groups. We then analysed familial correlations of the immune response to infection studying the tuberculin skin test (TST) size and IFNy-production of PBMC's after stimulation with mycobacterial antigens in a population from a low to moderate incidence region near Paris (Val de Marne). We observed strong familial correlations for the TST, and after restriction of the study population to non-infected individuals, correlations for the ESAT6 trait were borderline significant (p=0.047). These studies have both scientific implications, as they will enhance understanding of immune mechanisms controlling infection, and medical implications, as they will open the way for new preventive and therapeutic strategies.
