Book excerpt: Ageing population poses a set of complex policy and dilemmas for social security systems, intensifying the concerns about rising expenditures in health care and long-term care for elderly. In this context, ageing societies has many valuable lessons to learn by studying Japan's experience dealing with its hyper-aged society and particularly from its strategies to ensure the financial sustainability of the Long-Term Care Insurance (LTCI) system. Based on an exhaustive literature review, and the results from six original researches on long-term care expenditures in Japan (LTCE) conducted during a doctoral program, the book provides a comprehensive view in analyzing trends and factors associated with increasing expenditures in the Long-Term Care Insurance system in Japan. The book address relevant topics such as; the main socio-demographic changes experienced by the Japanese society during the last three decades, predictors of the LTCE, measuring efficiency in nursing homes, the impact of the LTCI 2005-reform to contain expenditures, cost-effectiveness of the in-home and community based services and institutional LTCE in the last year of life. The book end with a discussion on futures challenges and strategies oriented to contribute with the sustainability of LTCI system in Japan.

Book excerpt: In Latin America and the Caribbean, more than 8 million older people are unable to independently complete at least one basic activity of daily living, such as eating, bathing or showering, or getting in and out of bed. This situation, called care dependence, affects 12% of people over age 60 and nearly 27% of people over age 80 in the region. The demand for long-term care services will more than triple in the next 30 years, due primarily to population aging and the epidemiological transition. The region will also see a drop in the traditional supply of long-term care services, which are based mainly on the unpaid work of women in families. Given this outlook, societies and governments must ready themselves to support care-dependent people and their caregivers, while at the same time advancing gender equity. Countries need to design long-term care systems, and they need to do so now. This publication addresses five key questions for countries in the region that wish to start this process: 1) Why is long-term care an issue that cannot wait?; 2) How do countries design a long-term care system?; 3) What are the countries in the region doing?; 4) How much would a long-term care system cost?; 5) Where should we start? This is the first IDB publication that is interactive and accessible to as many people as possible.

Book excerpt: During the last 25 years, life expectancy at age 50 in the United States has been rising, but at a slower pace than in many other high-income countries, such as Japan and Australia. This difference is particularly notable given that the United States spends more on health care than any other nation. Concerned about this divergence, the National Institute on Aging asked the National Research Council to examine evidence on its possible causes. According to Explaining Divergent Levels of Longevity in High-Income Countries, the nation's history of heavy smoking is a major reason why lifespans in the United States fall short of those in many other high-income nations. Evidence suggests that current obesity levels play a substantial part as well. The book reports that lack of universal access to health care in the U.S. also has increased mortality and reduced life expectancy, though this is a less significant factor for those over age 65 because of Medicare access. For the main causes of death at older ages -- cancer and cardiovascular disease -- available indicators do not suggest that the U.S. health care system is failing to prevent deaths that would be averted elsewhere. In fact, cancer detection and survival appear to be better in the U.S. than in most other high-income nations, and survival rates following a heart attack also are favorable. Explaining Divergent Levels of Longevity in High-Income Countries identifies many gaps in research. For instance, while lung cancer deaths are a reliable marker of the damage from smoking, no clear-cut marker exists for obesity, physical inactivity, social integration, or other risks considered in this book. Moreover, evaluation of these risk factors is based on observational studies, which -- unlike randomized controlled trials -- are subject to many biases.

In 2020 28% of the Japanese population will be over 65 years. In Germany it is prognosticated to be around 21% (Tab.1). Moreover the Japanese Health Care System is known as own of the cheapest of the industrialized countries. This leads to the question of adoption of some parts or ideas from the Japanese system to the German one. In my elaboration I want to describe a case of a family in Japan regarding the family and work situation, the Health Care System in general and additionally the system of taking care of the elderly and the children. My example family has got following parts: The mother is 42 years old, she works as a nurse in an outpatient department of a local government. In addition she has to take care of her parents almost every day. The father is 45 years old, works as an engineer and likes his hobby, which is driving motorbikes. The daughter of them is 7 and their son is 13, both go to school. The grandmother has got dementia, she is 76 years old and lives together with her husband, who is 76 and has got diabetes. They live in the neighborhood of their children and grandchildren. Case 1: Values, culture, roles of the family members. Case 2: Health Care System of Japan in comparison to the German one. What happens after a traffic accident of the father? Case 3: Elderly and Child Care in general and in the case of the accident and depression of the mother.
