Book excerpt: Introduction : Le triage des patients traumatisés sévères, élément essentiel de la qualité des soins, peut s’appuyer sur des scores de gravité préhospitaliers. Au sein du CHU de Rouen, il n'existe pas d'algorithmes robustes pour diriger un patient traumatisé vers la Salle d’Accueil de Réanimation (SAR) ou le Service d’Accueil des Urgences Vitales (SAUV). Cette étude vise à évaluer l'utilisation des scores de gravité préhospitaliers comme aide au triage intra-hospitalier entre SAR et SAUV. Matériel et méthodes : Nous avons conduit une étude monocentrique rétrospective entre 2021 et 2022, incluant les patients traumatisés sévères admis à la SAR et à la SAUV après transfert médicalisé. Les scores de gravités et la classification TRENAU (Trauma system du Réseau Nord Alpin Urgences) étaient ensuite comparés selon le service d’hospitalisation (réanimation ou service de chirurgie conventionnel) Résultats : Nous avons inclus 345 patients, répartis entre la SAUV (116 patients) et la SAR (229 patients). Deux cent trente-deux patients ont été hospitalisés en Réanimation. Le taux de triage approprié était de 85,5%. Le taux de sur-triage était de 6,96%, et le taux de sous-triage était de 7,54%. Parmi les scores évalués, le score MGAP, le score NTS, et le Shock-Index ont montré des différences statistiquement significatives entre les services, sans différence clinique. Concernant la classification TRENAU, des différences significatives ont été observées pour les catégories A et C. Aucune différence n'a été trouvée pour la catégorie B, sauf pour le score NTS. Conclusion : Ce travail nous permet d’évaluer les traumatismes sévères admis à la SAR et à la SAUV. Les taux de sous-triage dépassent les recommandations. La mise en place d’une aide pour le triage des patients semble nécessaire. L’usage des scores de gravité ne semble pas être adapté pour l’ensemble de ces patients.
