Building upon two prior studies from the Institute of Medicine (the 1991 Institute of Medicine's report Disability in America and the 1997 report Enabling America), The Future of Disability in America examines both progress and concerns about continuing barriers that limit the independence, productivity, and participation in community life of people with disabilities. This book offers a comprehensive look at a wide range of issues, including the prevalence of disability across the lifespan; disability trends the role of assistive technology; barriers posed by health care and other facilities with inaccessible buildings, equipment, and information formats; the needs of young people moving from pediatric to adult health care and of adults experiencing premature aging and secondary health problems; selected issues in health care financing (e.g., risk adjusting payments to health plans, coverage of assistive technology); and the organizing and financing of disability-related research. The Future of Disability in America is an assessment of both principles and scientific evidence for disability policies and services. This book's recommendations propose steps to eliminate barriers and strengthen the evidence base for future public and private actions to reduce the impact of disability on individuals, families, and society.

Book excerpt: This report looks at progress on improving accessibility since 2003 and ways of improving accessibility. Problems with transport provision and the location of services can reinforce social exclusion by preventing people from accessing key local services and undermines government policies to tackle worklessness, increase participation in education, reduce crime and narrow health inequalities. Insufficient progress has been made since the 2003 Social Exclusion Unit's Making the Connections report, many findings of which are relevant today. There is evidence that accessibility is worsening, driven by tight budgets in central and local government. Accessibility statistics show travel times to key services steadily increasing over time, particularly for access to hospitals. The Department for Transport needs to focus more closely on improving accessibility as well as on supporting the economy. Existing transport funding could be better coordinated and directed to 'accessibility'-focused initiatives, which will have a swifter impact on people's well-being than large infrastructure projects. The social value of transport and accessibility needs to be explicitly considered in policy-making and in the planning system and should no longer be seen as a second-order criterion.The Committee believes it will take time for any improvements to make a noticeable difference. Their recommendations focus on improving how government operates rather than funding. Central government cannot abdicate its role in coordinating action across departmental silos and helping local authorities and service providers to share best practice. Accessibility planning, introduced by Making the Connections, has had limited success and needs to be re-energised.

How can the economic benefits associated with accessibility changes be measured? What are the accessibility benefits of having a public transport service available as a transport option for unexpected future use? How can the land-use, transport and accessibility impacts of Dutch compact urbanisation policies implemented in the last decades be measured? The research presented in this thesis seeks to answer these and related questions. Results suggest that current practices in evaluating accessibility in the Netherlands can be improved using geographical accessibility measures within an integrated land-use/transport perspective. Another major outcome is formed by the possible significant benefits associated with option use of public transport services in addition to use and non-use benefits traditionally included in transport policy appraisal. Residents in the service area of regional railway links seemed to be willing to pay significant amounts for the continued availability of the railway links for unexpected future use. Dutch compact urbanisation policies were also shown to have contributed to the land-use and transport-related intentions of the Dutch national government. Without compact urban development urban sprawl would likely have been greater, resulting in greater car use and related environmental impacts, higher congestion and lower accessibility levels, along with stronger fragmentation of wildlife habitats.

Book excerpt: Despite the essential role transportation plays in most people's daily lives, the ways in which our interactions and experiences with transportation systems affect our well-being is often overlooked. Transportation is an issue more significant than the political battles over infrastructure and urban planning generally acknowledge. Existing research has shown that people's access to reliable, high-quality transportation options as well as the degree to which these options provide timely and convenient access to destinations of civic, social, educational, and recreational opportunity varies across race and income lines. As made clear through social science frameworks like social exclusion theory, these variations in accessibility can have significant consequences not only on individuals, but on entire communities. Despite the strong body of research that finds evidence of inequities in the degree to which people can use public transit services to access certain destinations, several questions have gone unanswered. Only a handful of analyses have sought to establish macro-level trends that tell us about overall social patterns regarding variations on the quality and utility of public transit service. Further, much of this work has failed to probe the sources of these variations or looked into the institutional drivers that might explain why some people have different experiences riding transit than others. This dissertation project is comprised of three empirical research articles that respond to these oversights by introducing a sociological lens to the study of public transportation services broadly, and destination accessibility research specifically. In the first paper, I generate and describe patterns of transit-based access to destinations of opportunity across twelve cities nationwide. In the second paper, I investigate the organizational elements native to transit agencies that have been shown to impact the effectiveness of public services and the degree to which users can reap their benefits. Finally, in the third paper, I evaluate the use and utility of traditional and alternative transportation planning paradigms for engendering robust accessibility outcomes. While the three analyses engage three unique research questions with their own theoretical foundation, hypotheses, and methodological technique, there is an overarching question that guides my analysis: how useful is public transportation service when it comes to actually meeting people's accessibility needs, and in what ways do public transit agencies themselves affect these accessibility outcomes? Results of this analysis demonstrates there are macro-level, observable differences in people's ability to use public transportation to access the places they need or want to go, and that particular elements of an agency's organizational structure do in fact impact the utility of transit to various destinations in ways that are both straightforward and complex across cities and between social groups. This work also demonstrates that although transportation-based planning initiatives are currently incorporated in transit agency planning standards and guidelines, the impact of this approach is limited. Collectively, results across the three studies provide solid evidence that the physical outcomes of transit systems are reflections of institutional conditions in transit agencies. In reinforcing the role and impact of public institutions for shaping social service delivery outcomes, this research is an important contribution to both urban sociology and urban transportation planning literatures.

Abstract: Taking public transport seems to be an easy task for many people, and because it is easy, many people have neglected the special travel needs of Persons with Disabilities (PWD). To create a "barrier-free" city, one should think from the perspectives of the PWD. This dissertation will illustrate different approaches to cater the travel needs of PWD and how the enhancement of public transport accessibility would contribute to the betterment of the society as a whole, and help us to achieve the goal of sustainable development. This is followed by the evaluation of the accessibility of public transport service provision in Hong Kong by fitting to the evaluation framework widely adopted in European Cities with advanced accessible public transport systems. The evaluation considers wide spectrum of aspects, ranging from the evaluation of government policies on provision of public transport services accessible by PWD to the evaluation over the sufficiency of travel information, stations or stops facilities as well as carriage facilities provided by franchised buses, the MTR and public light buses in Hong Kong. Franchised buses, the MTR and public light buses are the three major public transport modes in Hong Kong which has covered almost 90 percent of the total public transport trips in Hong Kong (TD, 2012). Among them, public light bus is the least accessible by PWD. In this study, modal choice preferences of PWD and factors that influence their preferences are also studied qualitatively. It is found that "accessibility" is the most important factor that influences the modal choice preferences of PWD. When PWD are asked to rank their modal choice preferences among the three public transport modes under studies, public light bus is the least popular option. Such finding has further supported the evaluation result that public light buses are the least accessible by PWD. Based on the evaluation and inputs from various stakeholders, recommendations are put forward in the last chapter. Among the recommended initiatives, the most important one is the establishment of a clear implementation framework over actions that should be done for enhancing the accessibility of public transport systems in Hong Kong through the formulation of an action plan. With the action plan, all the actions to enhance the accessibility of public transport services in Hong Kong can be carried out effectively, while actions can be done in a much comprehensive manner.
