Annually approximately 30,000 Americans are diagnosed with the disease and most will die from it within five years. P- creatic ductal adenocarcinoma is unique because of its late onset in age, high mortality, small tumor samples infiltrated with normal cells, and a lack of both early detection and effective therapies. Some of these characteristics have made studying this disease a challenge. Pancreatic cancer develops as a result of the accumulation of genetic alterations in cancer-causing genes, such as the oncogenes and the tumor-s- pressor genes. In the last decade, major progress has been made in identifying important oncogenes and tumor-suppressor genes for the disease. Pancreatic cancer (PC) is the third leading cause of cancer-related deaths in the United States. Greater than 90% PCs are Pancreatic ductal adenocarcinomas (PDAC) which have a dismal estimated 5-year overall survival of only 11%. PDAC is predicted to become the second leading cause of mortality in the United States by 2030. In the clinic, the PDAC patients often present with incurable disease at the time of first diagnosis. This cancer is particularly lethal because of its propensity to spread quickly to distant organs including the lymphatic system. PDAC metastasis occurs shortly after tumor initiation and is caused by a complex interaction between autonomous tumor cells and cellular elements of the tumor microenvironment. Although PDAC remains a challenging disease to treat and a leading cause of cancer-related deaths, progress is being made in disease etiology and pathogenesis, and intensive research is focused on developing optimal treatment approaches for patients. Considering the continuous progress being made in understanding PDAC pathogenesis and designing new therapies, it was considered timely to develop an up-to-date thematic issue of Advances in Cancer Research (ACR) focused on PDAC. In this thematic issue, we have included ten review articles by experts that cover cancer disparities, PDAC pathobiology, the roles of the tumor microenvironment, tumor heterogeneity, PC stem cells, genetic and signaling events critical for PDAC pathogenesis, Immunotherapy and chemoresistance in PDAC, and different PDAC mouse models. Book excerpt: Carcinoma of the pancreas has markedly increased in incidence over the past several decades, and now ranks as the fourth leading cause of cancer death. Cancer of the pancreas is abnormal cell growth in the tissue of the pancreas which is a small organ (about six inches long) located next to the small intestine, behind the stomach. The pancreas has two main jobs in the body: to produce juices that help digest (break down) food and to produce hormones, such as insulin and glucagon, that help control blood sugar levels. Both of these hormones help the body use and store the energy it gets from food. The majority of pancreatic cancers are adenocarcinomas. Adenocarcinomas are tumours that arise from the exocrine portion of the pancreas. The exocrine portion of the pancrease produces digestive fluids. A small number of pancreatic cancers begin in the endocrine pancreas. The endocrine portion produces hormones (such as insulin). Despite the high mortality rate associated with pancreatic cancer, its aetiology is poorly understood, it is rarely curable and has an overall survival rate of less than 4%. Research on changing the survival rates of this stubborn disease is highlighted in this new book.

In 2000, the National Cancer Institute (NCI) convened the Pancreatic Cancer Progress Review Group (PRG) to review the pancreatic cancer research field and make recommendations concerning the most urgent needs and promising directions for future NCI investment. The PRG's report, Pancreatic Cancer: An Agenda for Action provided priority recommendations in six major areas: Health of the Field and Overarching Issues; Tumor Biology; Risk, Prevention, Screening, and Diagnosis; Therapy; Health Services Research: Scientific Toolkit. In 2007, a Clinical Trials Planning Meeting on Pancreas Cancer Treatment was convened to discuss the integration of basic and clinical knowledge in design of clinical trials in pancreatic adenocarcinoma. A Consensus Report of the meeting published in November 2009 provided recommendations in the following emphasis areas: New Targets for Drug Development; Utility of Preclinical Models; Future Clinical Trials; Establishment of Biorepositories; Development of Biomarkers. NCI's efforts in pancreatic cancer research have increased our understanding of the biological basis for pancreatic cancer, developed potential means to reduce morbidity and mortality associated with the disease, and expanded pancreatic cancer research capacity. The following or some research highlights in the five key priority areas identified by the PRG: Tumor Biology – Research on the tumor biology of pancreatic cancer has increased understanding of factors critical to key steps in the progression of pancreatic cancers; Risk, Prevention, Screening, and Diagnosis – Results of NCI-sponsored research related to the risk, prevention, screening, and diagnosis of pancreatic cancer have identified potential markers associated with pancreatic cancer risk as well as potential diagnostic markers; Therapy – NCI-sponsored research on therapy for pancreatic cancer has investigated optimal timing, delivery, and drug combinations that may improve the treatment of pancreatic cancer.~
