Book excerpt: L'implantologie occupe une place de choix dans l'approche thérapeutique de l'édentement, pouvant résoudre les différents problèmes d'une prothèse amovible. Mais, ces thérapeutiques implantaires ne sont pas toujours acceptées par les patients et ce particulièrement chez les sujets âgés. Même si les raisons financières occupent une place non négligeable, les raisons de ce refus peuvent être très variées. L'objectif de cette thèse sera d'aborder les différentes causes évoquées par les patients et orientant leur choix vers des traitements de prothèses adjointes conventionnelles plutôt que vers une prothèse implanto-portée ou une prothèse à complément de rétention implantaire. Une étude sera conduite auprès des patients de l'hôpital Rothschild et de cabinets privés, patients venant consulter pour une réhabilitation prothétique pour laquelle une solution implantaire aurait pu être envisagée, solution refusée par les patients. Quatre questionnaires seront réalisés : - Questionnaires GOHAI : évaluation de la qualité de vie en relation avec l'état de santé bucco-dentaire ; - Questionnaire portant sur le niveau de connaissance sur les implants ; - Questionnaire portant sur les différentes raisons quant au refus du traitement implantaire ; - Questionnaire permettant d'effectuer une analyse de la prothèse de façon reproductible. Les réponses nous permettront de mettre en évidence les différents motifs d'objection. Ainsi si le coût reste une raison essentielle de la réalisation d'un traitement de l'édentement sans apport de l'implantologie ; la peur de l'intervention, la durée du traitement, l'âge sont aussi largement impliqués dans ce refus.

Book excerpt: Depuis l’essor de la prothèse fixée (PF) et de l’implantologie, la prothèse amovible partielle métallique (PAPM) est de moins en moins privilégiée, tant par les patients que par les praticiens. Cependant, le traitement par PAPM demeure la meilleure solution dès lors que la mise en place d’implants multiples s’avère impossible, et à condition d’en respecter les règles fondamentales de conception et de réalisation. En effet, il est nécessaire, d’une part, de se conformer aux principes de biomécaniques indispensables à l’équilibre de la PAPM. D’autre part, toutes les étapes cliniques de réalisation de la PAPM doivent être parfaitement réalisées. Celles-ci varieront selon la classe et l’étendue de l’édentement. Cependant, le traitement par PAPM peut être limité dans certains cas, et il existe toujours l’inconvénient de l’amovibilité. C’est donc dans le but de limiter voir d’éliminer certains effets iatrogènes ou contre-indications de la PAPM que la mise en place d’implant(s) va être utilisée. Pour cela, le diagnostic pré-implantaire et l’analyse clinique devront être rigoureux pour permettre de positionner au mieux le ou les implants, et de choisir le système de jonction implant-PAPM le plus adapté. Cette association a cependant fait l’objet de peu de recherches et la littérature sur le sujet demeure limitée. Dans l’objectif de mieux aborder cette problématique et de contribuer à élaborer un guide d’utilisation de la prothèse stabilisée sur implant, nous avons choisi de traiter ce sujet par une double approche, théorique et pratique.

La question qui en découle est alors la suivante : quelle solution thérapeutique peut-on apporter à ces patients ? L'utilisation d'implants pour stabiliser ces prothèses amovibles partielles est alors détaillée afin de répondre aux différentes interrogations posées. La position des implants, leur nombre, les indications de cette thérapie, ses avantages et ses inconvénients sont approfondis. Trois cas cliniques sont détaillés pour mieux illustrer les parties théoriques précédentes.

It is different from other histories of American literature in that the language of the writers and their readers was not English. There have been studies of American authors who have used languages such as French, German, Spanish, or Swedish, but this is the first comprehensive history of any literature written and read in the United States in another language than English. Indeed, most histories of American literature are based on the theory that English is the only American literary language. Such a theory, however, dismisses the fact that English has in periods been a minority language in many areas. In this book American literature is the literature of people who are American by choice or by birth regardless of the language they may have used.
