Cet ouvrage comporte 15 chapitres concis. Le premier décrit les principes généraux de l'évaluation du malade et de la prise en charge. Il donne une liste des questions à poser systématiquement pour évaluer la nature et la gravité d'un symptôme puis il explique les principes du traitement à la fois par des méthodes non médicamenteuses et des thérapies médicamenteuses. Les autres chapitres au coeur de l'ouvrage traitent de 14 symptômes courants : l'anorexie l'anxiété l'asthénie la constipation les nausées les vomissements les problèmes cutanés les symptômes urinaires et d'autres. Même des maux comme la toux ou le hoquet sont abordés car ils peuvent donner lieu à une gêne considérable chez le patient en phase terminale. Chaque symptôme est couvert de la même façon en décrivant les causes possibles les étapes à suivre pour l'évaluation puis en expliquant comment retenir et mettre en oeuvre la meilleure option thérapeutique. Les détails vont d'un plan de traitement de la constipation simple en cinq étapes aux conseils portant sur les aliments particulièrement susceptibles de déclencher des nausées en passant par des avertissements à propos des traitements qui peuvent se révéler nocifs ou dangereux. En ce qui concerne les thérapies médicamenteuses on trouvera des informations sur les médicaments recommandés les posologies les voies d'administration ainsi que des conseils sur les effets secondaires spécifiques et d'autres problèmes pouvant survenir chez le patient en phase terminale. Cet ouvrage vient compléter les informations apportées par la publication de référence de l'OMS Traitement de la douleur cancéreuse deuxième édition complétée par une analyse des problèmes liés à la mise à disposition des opioïdes. Il n'aborde donc pas la prise en charge de la douleur.

Book excerpt: The subject of the apallic syndrome is one which has long been familiar to me, although I have not personally studied it as deeply as I would have wished. I became acquainted with this syndrome long before the last war, when my neurosurgical colleague Hugh Cairns (1952), made his pioneer contribution under the term "akinetic mutism" . This was an ar resting title, but it was one which did not altogether satisfy some of his colleagues, includ ing myself. We found it difficult to suggest an alternative. That is one reason why I wel come the expression "apallic syndrome" . Forensic practice has forced me from time to time to consider rather more deeply this distressing syndrome, and to try and marshal my ideas in a form which would satisfy my colleagues in the legal profession. More than once I have been instructed to make a medico legal assessment of these unfortunate patients. The points which have concerned my lawyer friends have not been matters of diagnosis, or of morbid anatomy, or of etiology. The fac tual problem which has been put before me was to make some approximate assessment as to the expectation of life. Vague guess-work is unacceptable in such circumstances. What the lawyers require is a precise and dogmatic answer.

This was a unique conference in that it comprehensively addressed various disorders that affect the organ situated at the "center of the male", the penis. As an important beginning, the sociocultural aspects of the erect phallus were presented by G. Wagner from Copen hagen. The anatomy of the penis and the physiological con ditions of erection were then discussed by K. -P. J Unemann from Mannheim, Germany. Previous conferences on the penis had concentrated only on specific areas of disease such as impotence. However, it became readily apparent that at this conference something new for almost every as pect of disease would be discussed, including congenital disorders such as hypospadias and epispadias, sexually transmitted and noninfectious dermatological diseases, and congenital and acquired penile curvatures and penile fractures. An excellent presentation of managing penile cancer by stage related therapeutic decision was presented by S. C. MUller from Bonn, Germany. There is no better per son to present a discussion of Peyronie' s disease in 1996 from a historical and management perspective than J. Pryor from London, UK. This same degree of expertise was also demonstrated by I. Saenz de Tejada from Madrid, Spain, re garding priapism.
