Dans ce contexte, il est essentiel de développer des chaînes de traitement et d'analyse de ces informations afin d'aider le diagnostic médical.En échocardiographie, le traitement et l'analyse quantitatifs décuplent leur potentiel quand on utilise un système d'acquisition 3D. Dans cet objectif, nous avons développé une nouvelle chaîne de traitement qui contribue à améliorer le diagnostic médical à partir de la reconstruction 3D d'images ultrasons. Cette chaîne est divisée en plusieurs modules : acquisition, filtrage, segmentation et reconstruction 3D, modélisation, visualisation, et extraction de paramètres cliniques. Dans chacun des modules on a réalisé plusieurs contributions. La plus importante consiste en un modèle de fusion entre une méthode de segmentation locale évoluée (front de propagation 3D) et un modèle global de superquadriques afin d'extraire de manière précise les parois du ventricule gauche (VG) dans le cas de perte de régions. Cette segmentation permet en outre la visualisation 3D temporelle et le calcul de paramètres cliniques importants.En conclusion, la visualisation 3D du cœur en mouvement, le comportement du modèle virtuel et les valeurs des paramètres cliniques sont des informations importantes pour le cardiologue. Nul doute qu'elles contribueront à diagnostiquer avec une plus grande précision les maladies cardiovasculaires.

Book excerpt: Nous décrivons dans cette thèse le développement d'un système échographique permettant l'acquisition et le traitement de données échocardiographiques 3D+T. L'objectif ultime est de concevoir un système permettant la détection automatique du ventricule afin de pouvoir visualiser en 3D les déformations de l'endocarde du ventricule. Dans la première partie, nous décrivons le système échographique réalisé. Il est basé sur l'utilisation d'un transducteur 2D classique qui tourne autour de son axe pendant l'acquisition des données. Le transducteur est contrôlé par l'électrocardiogramme du patient. Des séquences d'images sont acquises pour chaque angulation du transducteur pendant la durée de la contraction du ventricule. Comme la durée des cycles cardiaques varie à cause des irrégularités de rythme cardiaque, nous avons extrait la période de la contraction (la systole) en utilisant l'électrocardiogrammme du patient. Nous avons alors créé une base de données qui est constituée de toutes les images nécessaires pour une reconstruction de type 3D+t. La construction de la base de données est complétement automatisée. La deuxième partie de la thèse est consacrée au traitement des images acquises afin de détecter automatiquement les contours internes du ventricule. Nous avons automatisé le processus de détection et de suivit des contours du ventricule gauche en nous basant sur un contour initial détouré par le cardiologue dans chacune des séquences. La méthode de suivit est constituée de deux étapes : dans la première étape, la position du contour est prédite en utilisant une mesure de similarité entre les régions dans des images consécutives ; dans la duxième étape, le contour est affiné en utilisant un modèle de contour actif qui évolue grâce à la minimisation d'une fonctionnelle d'énergie. La fonctionnelle est minimisée en utilisant un algorithme de recuit simulé. Nous présentons des résultats et nous discutons les avantages et les inconvénients de cette approche. Ces résultats sont utilisés pour la reconstruction et l'animation des surfaces en 3D. Ils permettent de plus de calculer des paramètres importants pour le médecin dont le volume du ventricule.

Book excerpt: Many noteworthy advances in our knowledge of the pericardium, its functions and diseases and their relation to heart failure have been made since the first edition of this book appeared in 1981; and no other book that covers in detail the physiology and pathophysiology has since been published. The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.
