Book excerpt: This book examines the present and future role of community health workers (CHW) as part of ongoing efforts to develop sustainable health systems and improve public health. Noting that many programs based on the use of community health workers have fallen short of initial expectations, the book draws on numerous evaluations conducted over the past three decades to identify problems, explore their roots, and propose concrete solutions. Information ranges from advice on the composition and functions of village health committees through a discussion of the effect of cost recovery programs on community participation, to the simple reminder that programs that were never correctly implemented should not be dismissed as ineffective. Throughout lessons learned from past experiences are discussed in terms of their relevance to market-oriented reforms that are currently changing the health sector. The analysis is presented according to three main groups of factors known to have an impact on the performance of CHW programs. Attitudes are considered in the first chapter, which aims to define the actions health authorities can take to improve the attitudes of health workers and communities towards each other. Proposals include changes in selection criteria, curricula, teaching methods, on-the-job training, and supervision. Ways to increase community participation are also discussed. Chapter two focuses on the management and structure of district health systems as an area where organizational reform could have a significant impact on the success of CHW programs. Three components of the district health system are given particular attention: health service structure, health management structure, and health development structure. Practical advice is given for improving the functions of village health committees securing community participation and involving traditional healers. The final chapter, on resources, considers the important issues of remuneration, planning and budgeting for programs, and mechanisms for securing financing from governments, communities, and the private sector. Experiences from Thailand and Jamaica are used to explore methods for assessing program costs. The book concludes with a call for continuing international commitment to the CHW concept and a discussion of the main challenges likely to confront programs in the future. .

Book excerpt: In a country as diverse as South Africa, sickness and health often mean different things to different people so much so that the different health definitions and health belief models in the country seem to have a profound influence on the health-seeking behaviour of the people who are part of our vibrant, multicultural society. This book is concerned with the integration of indigenous health knowledge (IHK) into the current Western--orientated Primary Health Care (PHC) model. The first section of the book highlights the challenges facing the training of health professionals using a curriculum that is not drawing its knowledge base from the indigenous context and the people of that context. Such professionals will later recognise that they are walking without limbs in matters pertaining to health. The area that was chosen for conducting the research was KwaBomvana in Xhora (Elliotdale), Eastern Cape province, South Africa. The people who reside there are called AmaBomvana. The area where the Bomvana peoples reside is served by Madwaleni Hospital and eight surrounding clinics. Qualitative ethnographic, feminist methods of data collection supported the research done for Section 1 of the book. Section 2 comprises the translation and implementation of PhD study outcomes and had contributions from various researchers. In the critical research findings of the PhD study, older Xhosa women identify the inclusion of social determinants of health as vital to the health problems they managed within their homes. For them, each disease is linked to a social determinant of health, and the management of health problems includes the management of social determinants of health. For them, it is about the health of the home and not just about the management of disease. They believe that healthy homes make healthy villages, and that the prevention of the development of disease is related to the strengthening of the home. Health and illness should be seen within both physical and spiritual contexts; without health, there can be no progress in the home. When defining health, the older Xhosa women add three critical components to the WHO health definition, namely, food security, healthy children and families, and peace and security in their villages. Prof. Mji further proposes that these three elements should be included in the next revision of the WHO health definition because they are not only important for the Bomvana people where the research was conducted, but also for the rest of humanity. In light of the promise of National Health Insurance and the revitalisation of PHC, this book proposes that these two major national health policies should take cognisance of the IHK utilised by the older Xhosa women. In addtion to what this research implies, these policies should also take note of all IHK from the indigenous peoples of South Africa, Africa and the rest of the world, and that there should be a clear plan as to how the knowledge can be supported within a health care systems approach.

The conjuncture between deep sociopolitical crises, witchcraft, the ravaging HIV/AIDS pandemic and the government’s initial reluctance to adopt antiretroviral therapy turned away desperate HIV/AIDS patients to traditional healers. Drawing on historical sources, policy documents and ethnographic interviews, Pemunta and Tabenyang convincingly demonstrate that despite biomedical hegemony, patients and members of their therapy-seeking group often shuttle between modern and traditional medicine, thereby making both systems of healthcare complementary rather than alternatives. They draw the attention of policy-makers to the need to be aware of ‘‘subaltern health narratives’’ in designing health policy.

Despite the growing interest in traditional medicine as an integral part of health care delivery, the bulk of it still remain unregulated. This observation may account for the contempt and distrust existing between the traditional healers and their orthodox counterparts, with each group claiming supremacy and relevance over the other. In Nigeria modern medicine continues to remain costly, heavily bureaucratised and elitist-oriented with large parts of the populace continuing to visit traditional clinics and healing homes in order to find succour and solace in the hands of the uncurbed and poorly regulated traditional healers. It is the view of the author of this book that some form of regulation between the two systems is necessary, to begin the debate the following questions are addressed here: What constitutes traditional or modern medicine? What are the criticisms against them and how are they refuted? How do you identify the beneficial, neutral, harmless and harmful aspects of the practice of indigenous medicine? What aspects of these, should or should not be integrated? What are the modifications the orthodox practitioner has to make? What are the problems and prospects of integration?
