For immediate implant placement in the mandibular molar region, the quantity of the interradicular bone (IRB), limiting structures like the inferior alveolar nerve and dimensions of the socket affect the overall success rate.AimThe aim of this study was to retrospectively evaluate the alveolar dimensions of the mandibular first molar using cone beam computed tomography (CBCT) for prospective immediate implant placement.Materials and MethodThe material for this retrospective study comprised of CBCT images taken using the same scanning machine (Galileosu00ae, Sirona, Germany) with a standardized patient positioning and exposure factors (0.4 mm Voxel, 120 kVp, 3u20138 mA). The following measurements were done for the permanent mandibular first molar for prospective immediate implant placement u2013 1) The width of buccal (WB) and lingual (WL) bone 2mm apical from the crest 2) The distance from the inferior alveolar nerve to the furcation (IAN-F) in the mid slice 3) The distance from the inferior alveolar nerve to the apex of the mesial (IAN-M) and distal (IAN-D) root 4) The mesio-distal (MD) and bucco-lingual (BL) socket size at the crest level 5) The width of the IRB 2mm (IRB2) and 4mm (IRB4) apical to the furcation, and at the root apex (IRBA) between the mesial and distal root. Descriptive statistical analysis of all data was done at alpha 0.05 (95% confidence intervals).ResultsThe CBCT images of 200 permanent mandibular first molars from 126 patients (83 males, 43 females; mean age 44.2 years) were evaluated. The MD and BL socket size at the crest was 8.77mm and 7.76mm respectively. The WB and WL, that determines the bone fill in the socket, averaged at 0.91mm and 2.92mm respectively, showing a statistically significant difference (p = 0.003). Among the scans evaluated, 48% had WB values 0.5mm. The IRB of the mandibular first molar, which influences the primary stability of the immediate implant, had a mean dimension of 2.12mm, 2.63mm and 4.35mm at IRB2, IRB4 and IRBA respectively. Only 18% of the subjects had an average IRB value 4mm. The IAN-F value averaged at 13.82mm, while the IAN-M and IAN-D values were 4.31mm and 4.61mm respectively, showing no significant difference (p = 0.15). Several studies demonstrated that anorganic bovine bone is considered a predictable option for grafting the maxillary sinus and provide high implants survival rates. Modern surfaces chemically modified sandblasted and acid-etched with hydrophilic properties aims to decrease healing time and afford early loading of dental implants. However grafted sites may represent a challenge for early loading of implants compare to native bone.AIMThe aim of this study was to evaluate the implant stability measurements using resonance frequency analysis (RFA) of a new surface implants placed in native bone and augmented sites at baseline and after 8 weeks.MATERIAL AND METHODSSeventeen edentulous patients with deficient bone high in the posterior maxilla (5mm) were select according to tomographic analysis for bilateral sinus floor elevation. All procedures were performed with the lateral window approach using 100% anorganic bovine bone and collagen membrane over the window. After 8 months, a second tomographic exam was used for planning the placement of 6-8 dental implants with a sandblasting and acid etching surface maintained in an isotonic solution of 0.9% sodium chloride. The implants were maintained submerged in all patients. After 8 weeks, the re-opening surgeries were performed and provisional prosthetic rehabilitation was started. All implants were rehabilitated with splinted crowns. The insertion torque was measured at the day of the implants placement (baseline) while the resonance frequency analysis (RFA) was performed at baseline and after 60 days. The implants survival was evaluated after 6 months of provisional prosthetic rehabilitation.RESULTS A total of 113 implants were performed in augmented sites and native bone (89 and 24, respectively) to allow fixed prostheses oral rehabilitation. Four implants failed and a survival rate of 96,46% was achieved after 6 months of provisional prosthetic rehabilitation. It was showed that the implants placed in the native bone (~60Ncm) presented higher values of insertion torque than the implants placed in grafted areas (~45Ncm). Regarding the RFA analysis, implants placed in grafted areas presented greater stability in relation to the implants placed in native bone in the period of 60 days. In addition, the RFA of the implants placed in native bone reduced at the period of 60 days in relation to the baseline period. All the implants presented an adequate value for the induction of occlusal loading ( 60).CONCLUSION AND CLINICAL IMPLICATIONSWithin the limitations of the present study, RFA analysis demonstrated that the new implant surface presented adequate values for occlusal loading after 60 days of implant insertion in augmented sites. Considering the short term period evaluation (6 months) a good implant survival rate of 96,46% can be achieved for early loading in augmented sites.

In the posterior maxilla, the sparse bone density leads to low primary stability. Aim: This retrospective study aims to analyze the factors that influenced the primary stability of dental implant of posterior maxilla.Material and methods: Patients records of treatment performed between January 2014 to December 2018 were reviewed for primary implant stability. Patients who had Straumannu00ae implants at posterior maxilla and exhibited RFA both buccal and mesial area immediate implant placement were included. The following variables were determined; patient age, gender, implantation region, timing of implant placement after tooth extraction (immediate placement, u2264 4 months after extraction, > 4 months after extraction), implant geometry (diameter and length), and adjunctive procedures during implantation (guided bone regeneration and transcrestal osteotomy). The RFA value was used The Kolmogorov-Smirnov Test to determine normal distribution. The Chi-Square Test was applied to compare the relationship between qualitative variables and the Kruskalu2013Wallis test was used to compare the mean. The Pearson correlation coefficient was used to determine correlation.Results: Seventy implants in 53 patients (male 19, female 34) were found. The mean age of this sample size is 59.04uf0b11.41 years old. The mean value of RFA of the buccal area is 65.76uf0b11.44 and the mean of RFA of mesial area 66.59uf0b11.55. Book excerpt: This clinical investigation evaluated differences in peri-implant gingival height of contour, implant stability and mid-tomographic crestal bone (vertical and horizontal) over time as a function of two implant loading techniques, delayed or immediate, used with immediate implant placement following tooth extraction in the maxillary esthetic zone. When comparing the treatment groups, there were no statistically significant differences in buccal and palatal soft tissue of horizontal and vertical bone changes. Similarly, there was no significant difference in implant stability levels between the two groups. However, over time there was a statistically significant improvement in the stability level for both treatment groups. Within the limitations of this preliminary study, reverse platform implants and associated tapered abutments used with either delayed or immediate loading were successful over the 4 month observation period. The soft tissue seal and maintenance of gingival contour and shape may be attributed to preservation of adequate bone, irrespective of loading technique.

Intraoral radiographs were used to evaluate the peri-implant bone level changes. The results demonstrated that there were significant differences between implants restored by platform-switching vs. platform-matching on the mesial and distal at the prosthetic restoration and the 1-year visits, and between implants in patients with diabetes vs. non-diabetes on the distal at the 1-year visit. This study complemented the present scarce literature on this topic through the identification of patient-related and implant-related characteristics that may contribute to peri-implant bone loss.

Sinus lift procedures are required for sufficient bone volume in the maxillary ridge. Osseointegration of titanium implants in and their stability in grafted and nongrafted sites has been shown by means of resonance frequency analysis (RFA). Aim/Hypothesis. The aim of our study was to evaluate the implant stability quotient (ISQ) results of the resonance frequency (RF) in grafted and nongrafted sinus lift procedures on the first day of surgery and after 1, 4, 16 weeks of healing period. Materials and Methods. In this study were included 58 healthy patients, where sinus lift was obtained. In all, 137 SLA ITI Standard plus implants were placed. In the first group of 27 patients, 62 implants were placed following sinus lift and grafted with Bio Oss 0.25u20131 mm particle size (small particles), and in the second group of 31 patients 75 implants were placed without bone augmentation. Resonance frequency analysis (RFA) was measured at both groups to get implant stability quotient (ISQ) values. Student's t test was used to calculate P values for ISQ with Statistica 7. Results. On the first day and one week after surgery no statistical difference was observed in ISQ values of implants placed in grafted and nongrafted sinus lift procedures (p=0,651). After 4 and 16 weeks, grafted sites showed higher ISQ values than the control site. Conclusion. Sinus lift procedures on grafted sites offer better long term stability. RFA values during the healing period are predictable for the implant outcome during maxillary sinus lift procedures.
