These facts may seem frightening, but they need not be. The good news is that you have a lot of power to protect and improve your heart health. A damaged heart can damage your life by interfering with enjoyable activities, preventing you from holding a job, and even keeping you from doing simple things, such as taking a walk or climbing steps. What can you do to reduce your personal risk of heart disease? First, you can learn about your own risk factors. Second, you can begin to make healthful changes in your diet, physical activity, and other daily habits. Whatever your age or current state of health, it's never too late to take steps to protect your heart. It's also never too early. The sooner you act, the better. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. The book details how VHP establishes a framework or pathway model for each step of the patient experience, intentionally guiding, improving and eliminating risk when possible.
