Book excerpt: The goal of the research project, "Time-Resolved and Spectroscopic Three-Dimensional Optical Breast Tomography," was to develop a safe and affordable breast cancer detection modality that uses noninvasive near-infrared (NIR) light for imaging and diagnosing tumors in human breast with high resolution and specificity. In order to realize that goal three different approaches were developed. These are Optical Tomography using Independent Component Analysis (OPTICA), Round Trip Matrix (RTM) approach, and Center of Intensity Time Gated Imaging (CITGI). OPTICA is the most developed of these approaches. It has been demonstrated that OPTICA can detect, obtain the threedimensional location information and cross-section images of small absorbing, scattering, and fluorescent targets in model media, and tumor(s) in realistic model breast with considerable accuracy. Further accomplishments include development of a forward model for determining the backscattered polarized light intensity distribution, and initial development of a technique to determine scattering coefficient, absorption coefficient, and anisotropy factor (g) of breast tissue specimens. Besides detection and localization of tumors in breast, these results may have potential applications in optical biopsy, and lumpectomy. The graduate student supported by the project received his Ph. D., adding another scientist to the ongoing fight against breast cancer.

Book excerpt: The research carried out during the current reporting period involved: (a) extending the cumulant solution of radiative transfer to planar geometries and comparing with Monte Carlo simulations; (b)developing and enhancing the 3D tomographic image reconstruction algorithm by using the cumulant transport model; (c) developing the criterion of the optimal regularization for image reconstruction dependent on the noise presented in measurements; and (d) deriving the nonlinear correction factor for optical imaging due to multiple passages of an absorption inhomogeneity by a photon and providing a measure of the efficacy of linear inversion schemes. The cumulant transport model was found to provide a more accurate model than the conventional diffusion model for image reconstruction in turbid media such as human breasts. The proper modeling of the noise and the appropriate regularization improves the quality of image reconstruction. The nonlinear effect of the multiple passages of an absorption site by a photon on optical imaging only becomes appreciable when the size of the inhomogeneity reaches about ten times of the transport mean free path or larger in human tissues. The theoretical formalism and computer algorithm for 3D tomographic image reconstruction shows with simulated data the potential to provide fast 3D images of the scattering and absorption objects at various depths in turbid media.

By exploiting the phase shift of X-rays rather than their attenuation, phase-contrast tomography has the potential to dramatically increase the visibility of small and low contrast features, thus leading to better diagnosis. This thesis presents research on the first synchrotron-based project developing a clinical phase-contrast breast computed tomography (CT) setup at Elettra, the Italian Syncrotron Radiation Facility. This book includes a comprehensive theoretical background on propagation-based phase-contrast imaging, exploring and extending the most recent image formation models. Along with theory, many practical implementation and optimization issues, ranging from detector-specific processing to setup geometry, are tackled on the basis of a large number of experimental evidences. Most of the modelling results and data analysis have general validity, being a valuable framework for optimization of phase-contrast setups. Results obtained at synchrotron are also compared with "real world" laboratory sources: both a first-of-its-kind comparison with one of the few hospital breast CT systems and a state-of-the-art implementation of monochromatic phase-contrast micro-tomography with a conventional rotating anode source are presented. On a more general level, this work sheds a light on the importance of synchrotron-based clinical programs, which are key to trigger the long-anticipated transition of phase-contrast imaging from synchrotrons to hospitals.
