Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. It accompanies two other Observatory publications Organization and financing of public health services in Europe and The role of public health organizations in addressing public health problems in Europe: the case of obesity alcohol and antimicrobial resistance (both forthcoming).

In recent years, however, economic incentives have become more and more important in health care, even in welfare states such as Sweden. In a decade and a half, Sweden has experimented with different health care modalities. As far as we know, relatively little research has been done to assess how these financial incentives, over time, have affected Sweden and the views of physicians. This work analyzes the implications of these reforms on the future structure of the well-envied health care of the Swedes.

By the 1970s it was considered a model of the successful welfare state, providing a broader and more elaborate system of social programs and security to more people than any other country, the centerpiece of which was its health care system. As Twaddle explains, however, by 1990 there was a significant shift in Sweden's health policy debates. Instead of speaking about the medical care system in terms of effectiveness, solidarity, and public planning, the discussions grew focused on competition, markets, and privatization, taking on more of the characteristics of the U.S. system. Twaddle explores the nature of the proposed changes in medical care, the context in which those changes were being proposed, and the steps that were taken to implement change. He concludes that the problem of market- oriented reforms in health care seems to be almost universal.

Book excerpt: During the 1980's, the relative scarcity of resources gradually increased. In view of difficult economic prerequisites as well as criticism regarding the productivity development of the health-care sector and the individual's limited freedom of choice, many county councils discussed through changes in the way of controlling the activity at the end of the decade. The solutions discussed were models where the responsibility for financing and purchasing health care would be separated from the responsibility for providing health care. Furthermore, the health-care providers would be reimbursed prospectively per case and act in some form of competition. A corresponding process was to confirm the individual's freedom of choice. During 1992, in all nine (of 26) county councils had some kind of purchaser organization which reimbursed at least part of the health care prospectively per case. In 1993, another two county councils came in addition. The solutions tried differ between the country councils regarding the purchasers' organization and the way of reimbursing the health-care providers, among other things. Starting from the reforms which has been initiated in several county councils, at least three conceivable scenarios of the future development can be identified. The first two scenarios imply a pure separation of the responsibility for purchasing and providing health care. The third scenario implies that the integrated responsibility for financing and providing care in principal remains.
