The number of people requiring long term care in Malaysia is projected up to the year 2040 using the multiple state method from Rickayzen & Walsh (2002). The method has been applied in developed nations (Leung 2004, Hariyanto et al. 2013) and a developing country (Bueno 2013). Rickayzen & Walsh's 2002 methods are extended in this paper to allow the estimation of mortality improvement based on Malaysian historical time-series data. The future long term care costs are also estimated, providing valuable information to policy makers if the current policy continues unchanged. In addition, the cost effectiveness of current programs is estimated and compared with an Asian developed nation, Japan. We make use of WHS (2002) survey data to estimate the health utility index for elderly living in public institutional care.

Increased life expectancy and lower fertility rates are changing the population structure worldwide in a major way: the proportion of older persons is rapidly increasing, a process known as population ageing. The process is inevitable and is already advanced in developed countries and progressing quite rapidly in developing ones. The 2007 Survey analyses the implications of population ageing for social and economic development around the world, while recognising that it offers both challenges and opportunities. Among the most pressing issues is that arising from the prospect of a smaller labour force having to support an increasingly larger older population. Paralleling increased longevity are the changes in intergenerational relationships that may affect the provision of care and income security for older persons, particularly in developing countries where family transfers play a major role. At the same time, it is also necessary for societies to fully recognise and better harness the productive and social contributions that older persons can make but are in many instances prevented from making. The Survey argues that the challenges are not insurmountable, but that societies everywhere need to put in place the policies required to confront those challenges effectively and to ensure an adequate standard of living for each of their members, while respecting and promoting the contribution and participation of all.

Book excerpt: We review standard economic growth models concentrating on the relationship between population ageing and economic growth and develop a growth model with endogenous human capital and endogenous fertility. With this model we analyse the effects of education policy and family policy on economic growth. The main result for economic policy is that education policy boosts human capital and thus economic growth only in the short-term (i.e. during the transition phase towards the steady state). To boost economic growth in the long-term (i.e. when the economy has reached its steady state), it is necessary to employ family policy as this increases both the fertility rate and the time individuals spend accumulating human capital. The main result for economic growth theory is that steady state economic growth is not possible when population is ageing. Steady state growth is only possible if the age structure of the population is constant.
