Book excerpt: Changes in the allocation of healthcare resources have raised issues related to the efficacy and outcomes of medical therapy and how such factors may be measured. The questions associated with the quality of life and functional capability of patients with chronic health conditions have been of special interest. Endocrine disorders have the potential for disrupting the general health and well-being of affected individuals and their families; thus they warrant serious attention. This symposium was convened in November 1997 at Palm Beach Gardens, Florida, to bring together medical, behavioral, and social scientists. The meeting fostered the presentation and discussion of the most current clinical research on the effects of various therapies on a wide range of endocrine disorders from diabetes to adrenal insufficiency and growth hormone deficiency. The participants, all noted national and international experts in their fields, focused their attention on both the biomedical value and effectiveness of treatment, as well as on the impact such treatments have on psychological states such as mood and cognition. Many presentations specifically emphasized the quality of life (QOL) indicators that now regu larly appear in many research protocols and reports. The pioneering work of two clinical researchers was a major highlight of the meeting and an Award of Recognition was presented to Robert Blizzard, M. D. , and John Money, Ph. D. , for their innovative and insightful work in pediatric endocrinology and psychosexual development.

This meet ing was organized as a Satellite Symposium of the 27th International Congress of Physiological Sciences, Paris, 18-23 July 1977. This volume is a record of the com munications presented at the symposium. The objectives of the program were to examine the role of the endocrine system in a wide spectrum of adjustments and adaptations to changes in environmental conditions by various spe cies of animals, including man, and to promote an ex change of ideas among investigators who have approached these functions from diverse aspects. The diversity of the information and ideas communicated is great. Of necessity, they represent only an extremely modest se lection of the many facets of endocrine function in the interaction of animals with their environments. Be yond the usefulness of the communications individually, we hope that they collectively demonstrate the substan tial heuristic value of the concept of environmental endocrinology as it was perceived by the participants. A final chapter includes extensive additional resources for both transgender patients and providers. Thus, an endocrinologist providing care to a transgender person will be able to use this single resource to address most of the patient’s needs. Even though late effects observed in cancer survivors can affect any organ or system, endocrine and metabolic dysfunctions are the most frequently reported. Endocrine complications rarely influence life expectancy of cancer survivors, but they can significantly impact morbidity and quality of life. Among endocrine adverse effects, severe hypothalamic damage may be considered the most harmful in survivors, leading to morbid obesity, propensity to metabolic syndrome and cardiovascular disease. This book aims to disseminate the knowledge about endocrine and metabolic adverse effects of cancer therapies and about survivorship care. Since the number of cancer survivors is steadily growing in the general population, this publication is intended not only for endocrinologists but also for oncologists, onco-hematologists, internists, pediatric specialists in those areas and general practitioners, with the aim to better counsel and monitor cancer survivors.

Other drugs used for nonendocrine ther apies can likewise affect the endocrine system. Endocrine pharmacology emerged in the early 1900s with the use of crude pituitary extracts. By the mid-1900s several investigators had isolated and begun to synthesize hormones or hormonelike substances. Recognizing the limited supply of hormones that could be obtained both from animal sources and human autopsy material, the search for so called hormone substitutes also began early in the 1900s. Recently, re combinant DNA technologies have been used to provide alternative therapeutic sources of human insulin and human growth hormone. Aside from insulin, perhaps no other use of hormonally-active sub stance is better exemplified by those drugs which affect fertility. The synthesis of an orally-effective steroid represented one of the first major breakthroughs in the chemical suppression of ovulation. Since the orally active 19-norsteroids were introduced in the 1950s, several oral contra ceptive steroid preparations have been marketed. Indeed, the advent of oral contraceptives for birth control has led to a renewed interest in endocrine pharmacology.
