Book excerpt: The "Veterans Choice Program" requires the VA to furnish hospital care and medical services through eligible non-VA health care providers to eligible veterans who either cannot be seen within the wait-time goals of the Veterans Health Administration or who qualify based on their place of residence or face an unusual or excessive burden in traveling to a VA medical facility as reported in chapters 1 and 2. Chapter 3 examines how officials at VHA facilities responded to adverse-action information received through National Practitioner Data Bank, how VHA facilities adhered to polices regarding providers with adverse actions, and steps VHA has recently taken to ensure that providers meet licensure requirements. Chapter 4 reviews the extent to which VA collects reliable information associated with employee misconduct and disciplinary actions, adheres to documentation-retention procedures when adjudicating cases of employee misconduct, ensures allegations of misconduct involving senior officials are reviewed according to VA investigative standards and these officials are held accountable, and has procedures to investigate whistle-blower allegations of misconduct; and the extent to which data and whistle-blower testimony indicate whether retaliation for disclosing misconduct occurs at VA.

Book excerpt: The purpose of this study is to collect data from employees working for the Ambulatory Care Center of the Department of Veterans Affairs regarding the Veterans Access, Choice, and Accountability Act of 2014. The research question is; how do the Department of Veterans Affairs employees feel regarding the implementation of the Choice Program. The method used to explore the research question is to survey a group of 42 staff working in primary care at the Department of Veterans Affairs with an emphasis on its importance to their role, it’s effectiveness on patient care, and their own knowledge of the Choice Program. The results of this study show a significant relationship between role and importance of the Choice program x2 (27,N=42) = 50.72,p=.004 as well as role and the view on effectiveness of the Choice Program x 2 (9,N=42) = 18.29,p=.03. The health care of millions of military veterans is directly affected by the Choice Program.

While the existing Choice Program will expire on or before August 7, 2017, VHA has submitted to Congress a plan for consolidating several programs that provide similar care through non-VA providers into a new, single VA community care program in fiscal year 2018.This statement provides GAO's preliminary observations about, among other things, (1) VA's process for scheduling Choice Program appointments for routine care, and (2) what is known about the timeliness of veterans' Choice Program appointments for routine care and urgent care. This statement is based on ongoing work for which GAO reviewed applicable laws and regulations, VA's TPA contracts, and relevant VHA policies and guidance. GAO also visited 6 of 168 VAMCs and reviewed a random, non-generalizable sample of 196 Choice Program authorizations. These authorizations were for veterans who were referred to the Choice Program by the 6 VAMCs between January 2016 and April 2016. GAO also interviewed VA, VHA, and TPA officials. Results from GAO's analysis cannot be generalized to all Choice Program authorizations or all VAMCs.

VHA is responsible for ensuring that its more than 400,000 health care providers and support staff are qualified, competent, and suitable to provide safe care. This report examines the extent to which (1) VHA responded to adverse information regarding employees’ criminal history or DEA registrations and (2) vulnerabilities exist in VHA’s processes for completing and documenting background investigations. GAO is making 14 recommendations to VA, including that VHA establish a timeline for finalizing and implementing a policy regarding DEA employment waivers. Such a policy should include guidance for determining whether employees have access to controlled substances. GAO also recommends that VHA establish control procedures to ensure background investigations are completed and documented.
