Book excerpt: Excerpt from Practical Introduction to Medical Electricity This is a new rather than the second edition of an old one; I have found it necessary not only to add new materials, but to rewrite the greater part of what I allowed to remain. The list of instruments which in the first edition gave the impression of an illustrated catalogue, and occupied much space uselessly, has been considerably reduced so as to keep the book within reasonable limits. I still insist upon measurements of current strength as the essential condition of a rational application of electricity to medicine. It is gratifying to me that the proposition I first made in 1878 of adopting the milliweber, now called milliampere, as the electrotherapeutical unit, has received the sanction of the special committee of the International Congress of Electricians. Absolute galvanometers are becoming very generally used on the Continent, especially in France and Germany. Of equal importance with an accurate knowledge of the actual electrical quantities we are using, is a clear understanding of the distribution of those quantities among the tissues we wish to influence. This electrical distribution, or diffusion, depends largely upon the extent of the surface through which the current is introduced into the body; I therefore propose the adoption of Prof. Erb's set of standard electrodes so as to fix once for all the meaning of the terms "small," "large," etc., hitherto applied to them in a loose manner. After passing through eight editions, the old work of Beard and Rockwell was thoroughly revised and mostly rewritten by the present writer. The old stereotyped plates were destroyed and all illustrations newly drawn. Four years ago another edition was issued in which no less pains were taken to keep the book fully abreast the times, in the departments of X-ray Diagnosis and Therapeutics, Light Therapy, Vibratory Therapeutics and High Frequency Currents. In the present edition, the chapter on High Frequency Currents has been enlarged and mostly rewritten, together with various needed changes and additions in the departments of X-ray Diagnosis and Therapeutics. The chapter devoted to Photo-Therapy has been enlarged and much of it rewritten, while the subject of Radio-Activity is considered in an additional chapter. But while welcoming the new it has been the aim of the writer not to discard the old simply because it is old. To his mind a grave error has been committed by those who have either abandoned or never attempted the use of those indispensable manifestations of electric force - the direct and induced continuous currents (galvanic and faradic). Elaborate electric paraphernalia (static and high-frequency), adorn the consulting room of many a physician who is quite ignorant of the fundamental principles on which is based the use of electricity in medicine, and who would be altogether perplexed if asked to differentiate in the use of electric modalities. Valuable as these newer forms of electricity are, they constitute not the whole of Electro-Therapy, and he who fails to study the art of differentiation and confines his efforts to any special electric modalities in response to the popular trend and because of ease of administration, will oftentimes widely miss the mark. Since the introduction of Static Electricity, its nutritional influence has been made much of as if the idea was new rather than otherwise. My gratification is the greater in that the sale of this, and other, works on Medical Electricity, afford indubitable evidence of growth in the appreciation of electricity as a remedial agent, by the medical profession in general. That this force should be utilized in therapeutics, just as any drug is employed as a remedy, is a fact of the highest importance. Besides the addition to the resources of the practitioner thus obtained, it is the one mode by which electricity may be divorced permanently from charlatanry, which has profited by it commercially, whilst it has impaired professional confidence in its utility. If electricity has real value as a remedy for disease, it is clear no physician is justified on moral grounds in ignoring it, and on commercial grounds, to neglect the employment of so useful an agent, must be regarded as an act of exceeding unwisdom. To utilize electricity has become a necessity of the times; hence, a full understanding of its principles and appliances is imperative. The accumulation of clinical facts and experiences, and the improvements in apparatus, have so simplified and facilitated the practical adaptations of the science and art to the work of the physician, that the least qualified members of the profession may achieve a considerable measure of success in practical electro-therapeutics, although unacquainted with the niceties of electrical science. As stated in the preface to the first edition, one of my purposes in preparing the work was to afford some aid in generalizing medical electricity, and that I have been able to contribute in any degree to this object has quite resigned me to the labor of composition. By rubbing a glass stick, we have separated the positive and the negative electricity contained in the stick: the positive electricity remained in the glass bar; the negative passed through the rubbing cloth and the body of the rubbing person into the earth. In rubbing a metal stick we separate the two kinds of electricity in the same way; but we can prove this only by insulating one end of the metal stick, for instance, . By cementing it into a glass tube, and holding the glass tube in our hand while rubbing the metal. If we were to touch the non-insulated metal stick, the separated negative and positive electricity would get te united again immediately through the body of the rubbing person, and would leave no trace at all.
