Health Care Transition: Building a Program for Adolescents and Young Adults with Chronic Illness and Disability is a uniquely inclusive resource, incorporating youth/young adult, caregiver, and pediatric and adult provider voices and perspectives. Part I of the book opens by defining Health Care Transition, describing the urgent need for comprehensive transition planning, barriers to HCT and then offering a framework for developing and evaluating health care transition programs. Part II focuses on the anatomic and neuro-chemical changes that occur in the brain during adolescence and young adulthood, and how they affect function and behavior. Part III covers the perspectives of important participants in the HCT transition process – youth and young adults, caregivers, and both pediatric and adult providers. Each chapter in Part IV addresses a unique aspect of developing HCT programs. Part V explores various examples of successful transition from the perspective of five key participants in the transition process - patients, caregivers, pediatric providers, adult providers and third party payers. Related financial matters are covered in part VI, while Part VII explores special issues such as HCT and the medical home, international perspectives, and potential legal issues. Models of HCT programs are presented in Part VIII, utilizing an example case study. Representing perspectives from over 75 authors and more than 100 medical centers in North America and Europe, Health Care Transition: Building a Program for Adolescents and Young Adults with Chronic Illness and Disability is an ideal resource for any clinician, policy maker, caregiver, or hospitalist working with youth in transition.

Available in PDF, EPUB and Kindle. Book excerpt: Through a series of conversations with prominent theorists and leading political activists in different parts of the world, this book explores the causes of the setbacks of both the reformist and revolutionary strategies of the socialist Left. Addressing the larger questions around the agenda(s) of socialism in the era of globalization, the interviewees believe that capitalism is not sustainable in the long run, due largely to its inherent contradictions, and that a post-capitalist social world order is inevitable and attainable. Despite notable differences, what emerges from this collection of voices and perspectives is a unified call to mobilize and organize for radical change through engagement with the various social groups that could play instrumental roles in the transition beyond a capitalist social formation.

Book excerpt: Pastoral-to-program size change is frequently described as the most challenging of growth transitions for congregations. Now Alban senior consultant Alice Mann, author of The In-Between Church: Navigating Size Transitions in Congregations, addresses the difficulties of that transition in this resource designed specifically for a congregational learning team. Conceptualized and developed by Mann for an Alban on-line seminar program test event conducted with 12 congregations in transition, her newest book features a five-step process enabling the learning team to engage a wider circle of congregational leaders and members in study, discernment, and planning. Never-before published resources include discussion of a major new concept—passive barriers to growth—plus Mann’s "System Change Index" tool to help congregations measure their progress from pastoral-size to program-size ways of operating. From preparing the congregation’s board and members, selecting the person to guide the learning process, and recruiting the learning team, to creating and celebrating a plan for congregational learning and action, Mann provides all the resources a congregation needs to address this significant size transition period.

This book was released on 2005 with total page 118 pages. Available in PDF, EPUB and Kindle. Book excerpt: Organizations today are awash in change. Managing change requires leaders to focus simultaneously on managing the business and providing effective leadership to the people. More often than not, it is the focus on the people side that loses out. This book offers a framework for understanding the issues and competencies that contribute to effective leadership during times of change. Its purpose is to help leaders determine how to choose and move among a variety of managerial approaches--to help them see what's working, what's not working, and what's missing. In this way, leaders can more clearly assess their impact and learn how to meet the demands of both managing the business and leading the people.

Download or read book Bridges to Independence written by National Research Council and published by National Academies Press. This book was released on 2005-08-26 with total page 173 pages. Available in PDF, EPUB and Kindle. Book excerpt: A rising median age at which PhD's receive their first research grant from the National Institutes of Health (NIH) is among the factors forcing academic biomedical researchers to spend longer periods of time before they can set their own research directions and establish there independence. The fear that promising prospective scientists will choose other career paths has raised concerns about the future of biomedical research in the United States. At the request of NIH, the National Academies conducted a study on ways to address these issues. The report recommends that NIH make fostering independence of biomedical researchers an agencywide goal, and that it take steps to provide postdocs and early-career investigators with more financial support for their own research, improve postdoc mentoring and establish programs for new investigators and staff scientists among other mechanisms.
