Book excerpt: The United States has entered a new security environment, causing its military to conduct operations in remote areas of the world with limited resources to support its troops. The Fleet Hospital Program is responsible for providing health service support ashore, mainly medical and surgical capabilities, to Navy and Marine Corps personnel during intense combat operations. The requirement for forces to be lighter, more maneuverable, and more readily deployed and employed in the 2001 "Quadrennial Defense Review" has created the need for change in this program. This study answers the following question: Should the Navy Fleet Hospital Program transform from a platform-based hospital to a modular, capabilities-based hospital? The study utilizes five future mission scenarios for deployable medical systems to evaluate the applicability of a capabilities-based fleet hospital to perform those missions. The study leads to the conclusion that, to remain a relevant deployable medical asset, the current fleet hospitals require reconfiguration to capabilities-based hospitals to meet the operational needs of the warfighter. The current fleet hospitals are simply too large and cumbersome to provide a flexible, modular, and maneuverable medical facility necessary to operate in today's security environment.

Available in PDF, EPUB and Kindle. Book excerpt: The global security environment has changed significantly since the original concept of fleet hospitals was developed. This thesis examines the process used to plan and budget for fleet hospitals, and examines the events that shaped the configuration and billet structure of fleet hospitals. Evidence for this thesis was taken from reports from DOD, GAO and Congress, congressional testimony, studies conducted by the Center for Naval Analyses, journal articles, after action reports and pertinent DOD directives and manuals. Additional data were obtained through interviews with key officials involved in resourcing and managing the Fleet Hospital Program and training personnel assigned to augment fleet hospital platforms. The thesis concludes that planning and budgeting for fleet hospitals is dependent on the structure of the THCSRR model. The establishment of Single Sourcing Hospitals to deploy as fleet hospital units is intended to enhance fleet hospital operational performance by capitalizing on working relations developed delivering the peace time benefit. As the Cold War ended and more accurate methods for estimating casualty rates emerged, the requirement for fleet hospitals decreased from 17 to 10. Shifts in Navy and Marine Corps doctrine to lighter, faster and more flexible maneuvers have led to the development of NEMSS.

Book excerpt: Deployable medical systems are standardized modular field hospitals that can be pre-positioned in the event of a contingency, national emergency, or war operations. The Navy maintains 10 pre-positioned, 500-bed fleet hospitals in its deployable medical systems inventory. Eight of the hospitals are pre-positioned throughout the world. The fleet hospitals are containerized and equipped with biomedical devices such as anesthesia apparatus, monitor-recorder electrocardiographs, and visual ultrasonic apparatus. Each fleet hospital requires about 450 containers for storage. The annual budget for maintaining 10 Navy fleet hospitals is about $23 million. The 10 fleet hospitals contain a reported $236 million of material. The Naval Fleet Hospital Program Office was a detachment of the Naval Supply Systems Command until October 1, 1999. On October 1, 1999, the office was transferred to the Navy Bureau of Medicine and Surgery.
