Book excerpt: This book examines the factors influencing women's choices of obstetrical care in a Bariba community in the People's Republic of Benin, West Africa. When selecting a research topic, I decided to investigate health care among the Bariba for several reasons. First, I had served as a Peace Corps Volunteer in northern Benin (then Dahomey) and had established a network of contacts in the region. In addition, I had worked for a year as assistant manager of a pharmacy in a northern town and had become interested in the pattern of utilization of health care services by urban residents. This three-year residence proved an invaluable asset in preparing and conducting research in the northern region. In particular, I was able to establish relationships with several indigenous midwives whose families I already knew both from prior research experience and mutual friend ships. These relationships enabled me to obtain detailed information regarding obstetrical practice and thus form the foundation of this book. The fieldwork upon which the book is directly based was conducted between June 1976 and December 1977 and sponsored by the F ord-Rockefeller Popula tion Policy Program, the Social Science Research Council, the National Science Foundation, and the FUlbright-Hays Doctoral Dissertation Research Program. The Ford-Rockefeller Population Policy Program funded the project as a collab oration between myself and Professor Eusebe Alihonou, Professor Agrege (Gynecologie-Obstetrique) at the National University of Benin.

The country's history is also, however, brimming with social movements that have condemned such restrictions as violations of fundamental American liberties. This fierce conflict is one of the defining features of the social history of medicine in the United States. In Choose Your Medicine, Lewis A. Grossman presents a compelling look at how persistent but evolving notions of a right to therapeutic choice have affected American health policy, law, and regulation from the Revolution through the Trump Era. Grossman grounds his analysis in historical examples ranging from unschooled supporters of botanical medicine in the early nineteenth century to sophisticated cancer patient advocacy groups in the twenty-first. He vividly describes how activists and lawyers have resisted a wide variety of legal constraints on therapeutic choice, including medical licensing statutes, FDA limitations on unapproved drugs and alternative remedies, abortion restrictions, and prohibitions against medical marijuana and physician-assisted suicide. Grossman also considers the relationship between these campaigns for desired treatments and widespread opposition to state-compelled health measures such as vaccines and face masks. From the streets of San Francisco to the US Supreme Court, Choose Your Medicine examines an underexplored theme of American history, politics, and law that is more relevant today than ever.

Book excerpt: This book presents an in-depth qualitative study carried out with inpatients under treatment for substance use disorders (SUDs) in seven therapeutic communities (TCs) located in three countries: Peru, Nicaragua and Czech Republic. By comparing the experiences in these different cultural contexts, the book presents a grounded theory of SUDs treatment in TCs from a cross-cultural perspective, identifying factors that influence the efficacy of SUDs treatment in TCs based on interviews carried out with inpatients. Based on rigorous qualitative research methods, this book presents not only a comparative analysis of TCs located in different cultural contexts, but also analyzes the cross-cultural nature of the therapeutic programs adopted in these communities, such as the combination of traditional Amazonian medicine based on the therapeutic use of ayahuasca with conventional psychotherapy and occupational therapy, among other approaches. Departing from the interviews carried out with inpatients, the authors present a comparative analysis of how the different TCs address important issues related to SUDs treatment, and complement this analysis with machine-generated summaries of relevant scientific papers. These summaries contain results of similar research projects conducted in other cultural contexts. Substance Use Disorders Treatment in Therapeutic Communities: A Cross-Cultural Approach presents the results of a unique comparative study with great translational potential which will be of interest to both researchers and practitioners working in TCs. This unique comparative study identifies factors affecting the efficacy of therapeutic programs and proposes a grounded theory which aims to serve as an important source of information for therapists and other professionals working with SUDs treatment and for the replication of applied therapeutic methods in other TCs.

Major changes have taken place in the last 15 years in the way decision-makers think about the subject and the manner in which programmes deliver comprehensive sexual and reproductive health services. The turning point was the International Conference on Population and Development (ICPD) held in Cairo, Egypt, in 1994. ICPD was a watershed for several reasons. First, more than in any of the preceding United Nations population conferences, the issue of population was clearly placed as being central to sustainable development. Second, the narrow focus on population growth ("the population bomb") which had been a neo-Malthusian concern and preoccupation ever since the Club of Rome published its 1972 report Limits to Growth, was replaced by the comprehensive concept of (sexual and) reproductive health. Third, and linked to the definition and introduction of the reproductive health concept, was the strong call for a paradigm shift away from a policy environment driven by demographic considerations (sometimes to the point of using coercion in family planning services in order to reach demographic targets) to an environment that recognized the right of individuals to make their own choices. But in rural Gambia and other parts of sub-Saharan Africa, many women use contraceptives for the opposite reason—to have as many children as possible. Using ethnographic and demographic data from a three-year study in rural Gambia, Contingent Lives explains this seemingly counterintuitive fact by juxtaposing two very different understandings of the life course: one is a linear, Western model that equates aging and the ability to reproduce with the passage of time, the other a Gambian model that views aging as contingent on the cumulative physical, social, and spiritual hardships of personal history, especially obstetric trauma. Viewing each of these two models from the perspective of the other, Caroline Bledsoe produces fresh understandings of the classical anthropological subjects of reproduction, time, and aging as culturally shaped within women's conjugal lives. Her insights will be welcomed by scholars of anthropology and demography as well as by those working in public health, development studies, gerontology, and the history of medicine.

Book excerpt: Central to this book are Gbigbil women's experiences with different "reproductive interruptions": miscarriages, stillbirths, child deaths, induced abortions, and infertility. Rather than consider these events as inherently dissimilar as women do in Western countries, the Gbigbil women of eastern Cameroon see them all as instances of "wasted wombs" that leave their reproductive trajectories hanging in the balance. The women must navigate this uncertainty while negotiating their social positions, aspirations for the future, and the current workings of their bodies. Providing an intimate look into these processes, Wasted Wombs shows how Gbigbil women constantly shift their interpretations of when a pregnancy starts, what it contains, and what is lost in case of a reproductive interruption, in contrast to Western conceptions of fertility and loss. Depending on the context and on their life aspirations--be it marriage and motherhood, or an educational trajectory and employment, or profitable sexual affairs with so-called "big fish"--women negotiate and manipulate the meanings and effects of reproductive interruptions. Paradoxically, they often do so while portraying themselves as powerless. Wasted Wombs carefully analyzes such tactics in relation to the various social predicaments that emerge around reproductive interruptions, as well as the capricious workings of women's physical bodies.

Ritchey demonstrates that women in premodern Europe were both deeply engaged with and highly knowledgeable about health, the body, and therapeutic practices, but their critical role in medieval healthcare has been obscured because scholars have erroneously regarded the evidence of their activities as religious rather than medical. The sources for identifying the scope of medieval women's health knowledge and healthcare practice, Ritchey argues, are not found in academic medical treatises.
