Book excerpt: As providers of postpartum care, nurses are positioned to educate and support mothers in promoting breastfeeding and kangaroo care and to influence broader public health policy about maternal-child health. Based on documented research and studies, kangaroo care (skin-to-skin) immediately postpartum positively affects the rate of exclusive breastfeeding rates. Benefits of breastfeeding for the infant include the promotion of a natural source of nutrients, less risk of SIDS, fewer respiratory and ear infections, enhances newborn brain development, and less risk of diabetes and obesity in the infant (The Joint Commission, 2013). The mother can also benefit from breastfeeding by decreasing the risk of postpartum depression, less risk of breast and ovarian cancer and a faster recovery time (The Joint Commission, 2013). Education during pregnancy regarding breastfeeding and kangaroo care is important for the promotion of these actions. Education material including pamphlets and questionnaires and counseling from the staff nurses to their patients are important factors in the promotion of kangaroo care and breastfeeding in under-educated mothers. Questions that will be asked at admission regarding feeding plans include 'Do you plan to breast feed after delivery?' and 'Do you want to initiate kangaroo care immediately after delivery?' After these questions are asked, education material and counseling will begin for the patients who state no to one or both of these questions.

As light is shed on the mothers’ experiences during the postpartum period, there is an increasing need to find and implement evidence-based interventions to promote their health and wellness. As a vulnerable population, postpartum women require appropriate treatments and interventions to avoid postpartum complications. Three key themes of postpartum health and well-being that directly and significantly impact mothers are postpartum depression, maternal and infant attachment, and exclusive breastfeeding. This integrative review addressed the correlation between each of these themes and how kangaroo care, upright skin-to-skin contact, impacts them. Inconsistent use of kangaroo care as an intervention for mothers postpartum was discovered in this review, as it is typically used for infants. Statistical evidence supporting the use of kangaroo care to improve overall maternal health was discussed in this integrative review. Book excerpt: According to the World Health Organization (WHO) (1998), allowing new mothers to be cared for in the same room as their infant 24 hours a day is the seventh step of the ten steps to successful breastfeeding. Rooming-in and couplet care has been proven to increase positive outcomes, in many ways, for both the mother and infant (Couplet care for all, 2012). The following will identify a problem on a mother-baby unit in a hospital in Winchester, Virginia with proposed solutions related to couplet care. One recent research study found that couplet care positively effects breast feeding due to the promotion, education and support of breastfeeding (Grzeczinski Zenkner, Fortes Brasil Miorim, Silveira Cardoso, Portella Ribeiro, Cezar-Vaz and Pereira Rocha, 2013). The results of a similar study, in which couplet care was implemented, found that exclusive breastfeeding increased by 38% in the facility in which the study was conducted (Maugans, Buchko and Gutshall, 2013). This project will attempt to answer the question in new mothers and infants being cared for on a post-partum inpatient unit, what is the impact of fully utilized couplet care versus modified or non-use of couplet care on the overall care of the patient's including breastfeeding, maternal-infant relationship, safety and satisfaction? The implementation will begin by educating potential patients and all staff effected by this change. Meetings will be held among staff and the implementation team in order to ensure all questions are answered and increase the chance for 100% compliance. The practice change, detailed below, will then take effect with constant monitoring and evaluation by the implementation team. Results will be evaluated after three months, final changes will be made as necessary and then steps to finalizing the change will ensue.

In order for a facility to reap the benefits of Kangaroo care, a policy must be proposed and implemented. Proper education and implementation must be done in order for success to happen. With the implementation of skin to skin contact within the immediate postpartum period following both a vaginal and cesarean section will promote temperature control, breastfeeding success, and glycemic control of the newborn.

Project Aim: Improve the percentage of mother/baby skin-to-skin (kangaroo holding) from a baseline of 55% to 65% by August 2017. Setting: Perinatal Units Participants: Newborns birthed via cesarean section Evidence: According to the World Health Organization (2003), hospitals should have a goal of >80% exclusive breastfeeding rates for all newborns during their acute hospital stay and it begins with skin-to-skin contact (SSC), newborn's bare chest placed on mom's bare chest immediately after birth which does not exclude the operating room. SSC after cesarean decreases breastfeeding initiation time, increase the rates of breastfeeding duration and exclusivity, enhance infant feeding responses (Conroy & Cottrell, 2015). An estimate that the United States would save $13 billion annually if 90% of infants were exclusively breast fed in the first six months of life (Hung & Berg, 2011, 319). Method: Two nurses were assigned to the operating room for delivery. CNL to review electronic medical records (EMR) to evaluate if skin-to-skin is completed in less than ten minutes and is appropriately documented. Results: After education to 86% of the labor and delivery staff nurses, a review of the EMRs for all cesarean section deliveries was completed and skin-to-skin occurred 70% of the time. This result exceeded the initial goal of 65%.

Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems. Preterm Birth assesses the problem with respect to both its causes and outcomes. This book addresses the need for research involving clinical, basic, behavioral, and social science disciplines. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work.The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.
