Book excerpt: 21st Century System for Evaluating Veterans' Disability Benefits recommends improvements in the medical evaluation and rating of veterans for the benefits provided by the Department of Veterans Affairs (VA) to compensate for illnesses or injuries incurred in or aggravated by military service. Compensation is a monthly cash benefit based on a rating schedule that determines the degree of disability on a scale of 0 to 100. Although a disability rating may also entitle a veteran to ancillary services, such as vocational rehabilitation and employment services, the rating schedule is out of date medically and contains ambiguous criteria and obsolete conditions and language. The current rating schedule emphasizes impairment and limitations or loss of specific body structures and functions which may not predict disability well. 21st Century System for Evaluating Veterans' Disability Benefits recommends that this schedule could be revised to include modern concepts of disability including work disability, nonwork disability, and quality of life. In addition to the need for an updated rating schedule, this book highlights the need for the Department of Veterans' Affairs to devote additional resources to systematic analysis of how well it is providing services or how much the lives of veterans are being improved, as well as the need for a program of research oriented toward understanding and improving the effectiveness of its benefits programs.

Because the population of veterans who receive these supplemental benefits has been growing, GAO was asked to review VA's management of these benefits. This report (1) examines age-related trends in the population of Individual Unemployability beneficiaries and benefit payments; (2) assesses the procedures used for benefit decision-making; and (3) describes suggested options for revising the benefit. GAO analyzed fiscal year 2009 through 2013 data provided by VA, the most recent years available; reviewed applicable federal laws, regulations, and program policies; visited six regional offices selected for their differing accuracy rates, workload, and geography; reviewed a non-generalizable sample of claims; and spoke with rating specialists, experts, and VSO representatives. GAO recommends that VA issue updated guidance to determine eligibility; identify a comprehensive quality assurance approach to assess benefit decisions; verify veterans' self-reported income; and move forward on studies suggested by its advisory committee.

Book excerpt: As part of its Disability Compensation program, the Department of Veterans Affairs (VA) provides Individual Unemployability (IU) benefits to veterans of any age who are unemployable because of service-connected disabilities. Over the last decade, the number of IU beneficiaries and benefit costs have more than tripled. In 2005, about 220,000 veterans received an estimated $3.1 billion in IU benefits. In response to a congressional request, GAO assessed VA's management of IU benefits. This report (1) examines the added value of IU benefits for veterans of selected ages and disability ratings, (2) assesses the criteria, guidance, and procedures used for initial decision making, (3) assesses VA's ongoing eligibility enforcement procedures, and (4) compares VA's decision-making and enforcement procedures with those used by other disability programs.

Months after the accident, outfitted with sleek new prosthetic legs, he jogged alongside President Bush for a photo op at the White House. The photograph served many functions, one of them being to revive faith in an American martial ideal—that war could be fought without permanent casualties, and that innovative technology could easily repair war’s damage. When Bagge was awarded his Purple Heart, however, military officials asked him to wear pants to the ceremony, saying that photos of the event should be “soft on the eyes.” Defiant, Bagge wore shorts. America has grappled with the questions posed by injured veterans since its founding, and with particular force since the early twentieth century: What are the nation’s obligations to those who fight in its name? And when does war’s legacy of disability outweigh the nation’s interests at home and abroad? In Paying with Their Bodies, John M. Kinder traces the complicated, intertwined histories of war and disability in modern America. Focusing in particular on the decades surrounding World War I, he argues that disabled veterans have long been at the center of two competing visions of American war: one that highlights the relative safety of US military intervention overseas; the other indelibly associating American war with injury, mutilation, and suffering. Kinder brings disabled veterans to the center of the American war story and shows that when we do so, the history of American war over the last century begins to look very different. War can no longer be seen as a discrete experience, easily left behind; rather, its human legacies are felt for decades. The first book to examine the history of American warfare through the lens of its troubled legacy of injury and disability, Paying with Their Bodies will force us to think anew about war and its painful costs.
