Book excerpt: The aging of the baby-boomer generation has resulted in a rise of the population of women in the United States between the ages of 45 and 54 years. The hallmark health issue of midlife women is menopause and the menopause transition (MT). Symptoms associated with the female aging process have been shown to significantly affect health-related-quality-of-life (HRQoL). An increase in complex decision making factors and lack of updated knowledge in menopause treatment, have resulted in deferment by providers to treat menopausal symptoms. Studies have concluded inadequate updated of midlife women's care is a detriment to women, leads to fragmentation of health care delivery and potentially reduces HRQoL. Health-related-quality-of-life has been recognized by the National Institutes of Health and the Centers for Disease Control as a significant indicator of population health and wellbeing. An organizational change to address the gap of women's care during the menopause and MT, was the development of a consultative Midlife Women's Clinic. The theory of Transistions, and the Steven's Ace Star Model of Knowledge Transformation, provided the theory and evidence-based practice model for the development of the project. The objective of the project was to evaluate the perceived change of HRQoL of women attending a Midlife Women's Clinic. The goal of the project was to improve HRQoL of women in midlife by providing an opportunity for education, decision making and treatment of menopause symptoms. The perceived change of HRQoL was evaluated by the self-administered Menopause Rating Scale (MRS), a menopause specific HRQoL tool. Evaluation was accomplished by comparing the completed MRS questionnaire from the initial and follow up clinic visits. The results of the project demonstrate a statistically significant improvement in the perceived HRQoL severity scores were compiled and interpreted from the Evaluation Form of the MRS. Recent literature identifies a gap of care for women in midlife related to treatment and knowldedge of menopause and the MT that can adversely affect HRQoL. The implementation of a Midlife Women's Clinic appears to improve the perceived HRQoL menopause symptoms of women who were in attendance. Recommendations for future study would include; educational opportunities for student healthcare providers to learn updated menopause treatments by participation in this specialty clinic, development of community based seminars to disseminate knowledge about the menopause transition, and further study on the long term effect of the treatments offered on the impact of HRQoL.

Book excerpt: Purpose of this study was to assess the amount of knowledge women have about menopause, its symptoms, and treatment options with an emphasis on using exercise as management tool. Fifty-two women from 40-65 years old volunteered to take the survey, in which most questions were taken from the NAMS-Gallup Survey of 1994. Two sites were utilized for data collection: the Capital Region site (29 participants) and the Central NY site (23 participants). Results indicated that menopausal women have sought out information about menopause by discussions with a doctor in order to help manage their symptoms. Topics discussed most often with a doctor included osteoporosis, calcium supplements, exercise, healthy eating, weight, and emotional and mental health. Only a little more than half of the participants had discussed exercise with a doctor, expressing a greater need for improved communication on that topic. Women were generally satisfied at some level with the information they have received and believe that symptoms can be dealt with by using natural (non-HRT) methods though most women have not tried those methods. From analysis of this active cohort (>82% being active 3 or more days per week) it was found that physical activity was not significantly correlated to lesser severity ratings of the top three most experienced symptoms. While not statistically significant in this study, other research has shown the importance of exercise to help aid women through this process and women in this cohort have adopted physical activity regimens to help cope with menopause. Sources, satisfaction of information, treatment options, physical activity associated with severity of symptoms, and interest of learning more were primary hypotheses of this study and while most were supported, physical activity and severity of symptoms need further evaluation. Exercise still remains a highly suggested treatment option to help women deal with menopause.

The intent of the project was to improve PCP's knowledge of menopausal health, and to create their comfort level when discussing menopausal healthcare with clients. The workshop Menopausal Women's Healthcare Needs for the Primary Care Provider (Doerr-Kashani, 2014) was created in response to an identified gap in knowledge among PCPs nationally on menopause-related health issues. The workshop was designed as a resource to support PCPs to competently address women's menopause-related health issues, and enable them to provide comprehensive, evidence-based care within the existing clinic setting. The curriculum of this workshop included: a) PowerPoint presentation, b) printed current, evidence-based practice guidelines for menopausal-related conditions, and c) a CD with further evidentiary information. The companion Toolkit (Doerr-Kashani, 2014) provided templates for patient education designed to inform the patient, identify patient's questions to focus the visit, facilitate patient-provider communication and joint treatment decision-making. Two pilot workshops were delivered at Lifelong Brookside Clinic as a test of change. Outcome measures were pre and post-tests that assessed knowledge of menopause health care and comfort level discussing menopause. Change was measured by comparing pre and post tests for each attendee and revealed both improved PCP knowledge and comfort level. The Clinic Director subsequently decided to adopt the Toolkit for use at the clinic Key words: menopause healthcare needs, primary care provider education and menopause, patient engagement in healthcare, patient education and menopause.

Book excerpt: In this dissertation, I look to contemporary menopause management in the Southwest, United States, as an ideal case study of the 'real world' negotiation of a widespread contemporary conundrum--characterized by discourses of risk, proliferation of information and choice, chronic doubt, and personal responsibility for decision-making. While there have been previous studies of menopause in the US, this circumstantial ethnography seeks to understand contemporary menopause management in an era characterized by a massive shift in the biomedical risk discourses about menopause, the explosion of therapeutic choice in a burgeoning pluralistic health care environment, and the broad expansion of women's identities, body projects, and life priorities over the last several decades. I report on fourteen months of ethnographic fieldwork conducted in 2007 and 2008 with menopausal women and health care providers in the southwestern US. Research components included ethnographic interviews (N=60) and focus groups (6 groups with 27 participants) with midlife women, interviews with health care providers across a range of therapeutic modalities (N=20), and observation of emerging discourses of menopause in science, media and marketing. This dissertation illustrates that contemporary menopause management is a recursive process characterized by the ongoing re-evaluation of the impact menopause is having on one's life--in context. Participants described the unfolding of the lived-experience of menopause over time--even years beyond the end of menstruation. Risk discourses are not embodied en masse but reflect the concerns most salient in women's lives. While women access various expert and lay resources, they favor personal experts--sources deemed professionally sound and personally relevant--and their own embodied knowledge. For their part, health care providers described themselves as "normalizing" menopause and practicing patient-centered care aimed at empowering women to make their own decisions about how to manage menopause. Lacking an ideal choice, women make provisional treatment decisions that resonate with their current menopausal subjectivity. Despite abundant options, menopause management is increasingly stratified, with some able to access more information resources and afford more extensive decision-support. Among women with severe symptoms, bioidentical hormone therapy--productively positioned between biomedicine and complementary/alternative medicine--has emerged as a popular harm reduction strategy.
