Book excerpt: In the United States (U.S.), Type 2 Diabetes Mellitus (T2DM) has reached epidemic portions with a prevalence of approximately 29.1 million people and is the seventh-leading cause of death (Centers for Disease Control and Prevention [CDC], 2014; Healthy People 2020, n.d.). The U.S. spends an estimated $245 billion in direct and indirect medical costs. The indirect costs include disability and lost productivity (CDC, 2014). Previous studies focusing on diabetes education acknowledge the significance of self-management activities in managing T2DM. Extant studies examining self-management and treatment outcomes for persons with T2DM have paid limited attention to the relationship between patients' disease perceptions, diabetes knowledge, self-efficacy, self-management practices, and glycemic control. The purpose of this investigation was to identify factors and outcomes associated with healthy diabetes self-management practices among a cohort of individuals with T2DM residing in southern California. Nola Pender's Health Promotion Model (Pender et al., 2015) guided the study. Specific aims include: 1) Provide conceptual clarity for the phenomenon of perception through a concept analysis using Walker and Avant (2011) methodology, 2) Identify factors and outcomes associated with diabetes self-management through a systematic review of the literature, and 3) Examine relationships among perceived self-efficacy, specific patient characteristics, diabetic knowledge, self-management practices, and HbA1C values among a sample of individuals with T2DM living in southern California. The research aims provided the structure to generate new scientific data and evidence towards the improvement of self-management practices and those characteristics leading to improved HbA1C outcomes.

Book excerpt: Type 2 diabetes mellitus represents a daunting self-management challenge due to its complicated daily treatment regimen which includes adhering to diet, exercise, and medication recommendations. Little research has evaluated perceived barriers to diabetes self-care in a group that is at particular risk for diabetic complications: U.S. Latinos. This study had two aims. The first was to provide evidence that experimental and validated self-report instruments selected for this study functioned in a comparable manner in both Spanish and English in a sample of low income, predominantly Spanish-speaking diabetic patients (n = 77). The second aim was to examine hypothesized relationships of selected predictors to two criterion variables measuring health status (i.e., glycosylated hemoglobin A1c and Diabetes Impact) using a method described by Baron & Kenny (1986). Predictors included age, education, and income as covariates, and perceived environmental barriers to self-care, self-efficacy for treatment adherence, fatalistic beliefs (external health locus of control), and Latino cultural diabetes beliefs. Potential moderator and mediator variables of the relationship between perceived barriers to self care and criterion variables included self-efficacy for treatment adherence, fatalistic beliefs and Latino cultural diabetes beliefs. Scales used in this study had coefficient alpha values ranging from .82-.96 in Spanish and .74-.97 in English and also demonstrated strong factor structure integrity. Available in PDF, EPUB and Kindle. Book excerpt: The associations among perceived control, coping, and health in 69 adults with type 2 diabetes were examined. The goal of the current study was to determine whether a mediational model, based on Lazarus and Folkman's transactional model of stress and coping, or a mode rational model, based on Terry & Conway's goodness-of-fit hypothesis, was more appropriate to describe the relationships among perceived control, coping, and health. Regression analyses revealed that (a) emotion-focused coping mediated the relationships between perceived control, (b) perceived control also acted through an indirect effect on emotion-focused coping and health behavior to impact self-rated physical health, and (c) perceived control moderated the relationship between emotion-focused coping and psychological distress. These results provide partial support for both the transactional model and the goodness-of-fit hypothesis. Results are discussed in terms of the transactional model, goodness-of-fit hypothesis, optimal coping, and the importance of perceived control in psychological adjustments. Limitations and future research directions are discussed. Especially the potential role of patient education and its impact on perceptions of control are considered.

Book excerpt: The purpose of this pilot study was to examine exercise adherence in persons with Type 2 diabetes and how the stages of change in exercise are related to metabolic control. A sample of 28 persons with Type 2 diabetes in ages ranging from 53 to 77 (16 males and 12 females), was studied using the Transtheoretical Theory, of Change. Surveys on the Stages of Change, Processes of Change and Self-Efficacy, developed by Marcus, Rossi, Selby, Niaura, & Abrams (1992) were mailed to participants and followed-up with a telephone Stanford 7-day activity, recall. Subjects were assigned a stage of exercise adoption (Precomtemplation, Contemplation, Preparation, Action, and Maintenance) based on answers to questions in the mailed survey. Metabolic control was related to stage of exercise adoption. Reported physical activity, ' also were related to stages of change in exercise. The telephone survey was used as an internal validation of the mailed self-report. Processes of change in exercise and self-efficacy also were explored in relation to stages of change. Distribution among the stages was unusual. Half of the participants fell into the highest stage of exercise adoption (maintenance). Marcus, Rossi, et al. (1992) only found 22% of their sample to be in this stage. Also two of the stages (precontemplation and action) only had one individual and therefore the stages were collapsed into three stages combining precontemplation with contemplation and action with maintenance for many statistical calculations. In addition, for some calculations, the group was divided into exercisers (Stages 1,2, & 3) and non-exercisers (Stages 4 & 5), The most frequent type of exercise reported was walking. Thirty-five percent chose walking alone and an additional 60% who chose walking or treadmill as part of their fitness program.

Book excerpt: There is increasing evidence to suggest that self-management education programs can complement medical care for patients with chronic conditions, particularly diabetes. Research indicates that self-efficacy is important to improve outcomes and perceived health status for patients with chronic disease including diabetes. This nine article review of research literature was designed to identify self-management programs that positively impacted the health outcomes and perceived health status of those individuals with diabetes in both rural and urban communities. The review included quantitative studies done between 2008 and 2015 in the United States. Findings support the value of disease self-management programs for long-term support of diabetic patients to improve health outcomes and perceived health status of their chronic disease. To facilitate continuous reinforcement to achieve optimal health and perceived health status of these patients, specially trained nurses are needed within the community health setting.
