Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: The clinical question that guided this quantitative pre/post knowledge, attitude, and practices (KAP) survey practice improvement project was: Do nurse manager's knowledge, attitude, and practices related to facilitating evidence based practice (EBP) change following an educational intervention on EBP? Spanning the gap of translating evidence into practice is an imperative need in providing quality patient care and requires nurse managers to be involved stakeholders and change agents. The areas examined were the knowledge, attitude, and reported practices within a KAP survey specifically designed for this project, which was reviewed by three contents experts and received a 0.90 Cronbach's alpha coefficient rating, indicated statistically significant increases following the educational intervention. A total of 8 participants completed all of the required components within an online format and were included in the final data analysis. The participants were predominately female with more than 10 years of experience as a nurse manager, over forty-five years of age and possessed a Master of Nursing Science degree. The demographics of the sampled population, is a solid representation of nurse managers within the healthcare industry. Taking into consideration the low sample size the indication of the effect of an educational intervention suggests clinical significance. The participants displayed an increase in their knowledge, attitudes, and practices toward facilitating EBP by exposure to a comprehensive educational intervention that was conveniently available, as well as providing clear and concise data for further professional exploration.

Book excerpt: Despite evidence that organizational outcomes are negatively affected when professionals have limited decision making and their practice is constrained, little is known about the relationship of decentralized decision making (DDM) structures to the professional practice environment (PPE). In this study, the relationship of DDM to clinical nurses' perceived aspects of a PPE was examined. DDM consisted of the hierarchy of authority for work decisions and participation in strategic and unit group decisions. The PPE aspects of interest to administrators in complex organizations who are charged with organizational effectiveness include: control over practice, professional development, innovation activity, and peer relationships. Using a correlational design, data were collected from a random sample of 320 clinical nurses employed in a university teaching hospital. Instruments used included the Index of Centralization and the Professional Practice Environment Scale. Multivariate procedures were used to test hypotheses and to construct models. Authority for work decisions was positively related to the four aspects of PPE. Participation in decision making was positively related to all aspects except control over practice. Several individual and situational characteristics were found to moderate the relationship between DDM and PPE. Work pressure was statistically controlled. Model building showed that unit decision making was the most important variable in the relationships with professional development and innovativeness. Results contribute to organizational and nursing administration theory and are a useful guide for administrators to enhance PPE as a way to help manage constant change.
