Book excerpt: "Objective: Attention-Deficit/Hyperactivity Disorder (ADHD) is currently the most commonly diagnosed psychiatric disorder among school aged children. Various studies have found an association between ADHD and adversive childhood experiences. However, there are many conflicting hypotheses about the nature and direction of this relationship. The first aim of the study was to establish whether there is a relationship between adversive childhood experiences and the symptoms, and diagnoses, of ADHD, Conduct Disorder (CD), Oppositional Defiant Disorder (ODD), and Post Traumatic Stress Disorder (PTSD). Secondly, the study aimed to determine whether there is a group of children/adolescents with symptoms of ADHD that constitute a 'complicated ADHD' group. The study aimed to investigate whether this 'complicated ADHD' group had higher rates of adversive childhood experiences, higher rates of diagnostic co-morbidity, and more serious externalizing symptoms. Methodology: The clinical records of 217 consecutively admitted clients of a New Zealand Child and Adolescent Mental Health Service (CAMHS) were read in their entirety. Information regarding diagnoses, symptomology, and adversive childhood experiences was recorded and analysed. Latent class analysis was used to group children and adolescents into homogenous groups based on their profiles of ADHD, ODD and CD symptoms. Results: The study found significant associations between adversive childhood experiences and the symptoms of ADHD, PTSD, and other disruptive behavioural disorders. Latent class analysis revealed a group of children and adolescents who had more serious externalizing symptoms (a high number of ADHD, ODD and CD symptoms) and a high rate of diagnostic co-morbidity. This group had a higher average rate of adversive childhood events, physical abuse, and exposure to domestic violence, compared to the other children and adolescents with symptoms of ADHD. Conclusions: The findings from this study confirm other findings that early adversive childhood experiences place children and adolescents at significant risk of developing ongoing issues with inattentiveness, hyperactivity and impulsivity. Furthermore, the study provides tentative evidence that early adversive experiences may create a more complicated profile of ADHD symptoms and perhaps impact the way that ADHD symptoms manifest".

Book excerpt: In 1981, Leonard Pearlin and his colleagues published an article that would ra- cally shift the sociological study of mental health from an emphasis on psychiatric disorder to a focus on social structure and its consequences for stress and psyc- logical distress. Pearlin et al. (1981) proposed a deceptively simple conceptual model that has now influenced sociological inquiry for almost three decades. With his characteristic penchant for reconsidering and elaborating his own ideas, Pearlin has revisited the stress process model periodically over the years (Pearlin 1989, 1999; Pearlin et al. 2005; Pearlin and Skaff 1996). One of the consequences of this continued theoretical elaboration of the stress process has been the development of a sociological model of stress that embraces the complexity of social life. Another consequence is that the stress process has continued to stimulate a host of empirical investigations in the sociology of mental health. Indeed, it is no exaggeration to suggest that the stress process paradigm has been primarily responsible for the growth and sustenance of sociological research on stress and mental health. Pearlin et al. (1981) described the core elements of the stress process in a brief paragraph: The process of social stress can be seen as combining three major conceptual domains: the sources of stress, the mediators of stress, and the manifestations of stress. Each of these extended domains subsumes a variety of subparts that have been intensively studied in recent years.

Book excerpt: Attention-deficit/hyperactivity disorder (ADHD) is a neurodevelopmental disorder with onset usually occurring in childhood and adolescence. During these developmental stages, social interactions and external environments can have significant impact on children’s growth and development. Few studies previously used a theoretical model to examine the effects of social and external environments on ADHD. The goal of this dissertation is to apply an ecological perspective to examine the roles of school and community in the development of childhood ADHD through school engagement and physical activity. Three studies were implemented to carry out this goal. Study 1 examined the associations of neighborhood, school, and family characteristics with school engagement. Study 2 assessed the associations between school engagement and ADHD. Finally, study 3 examined the relationship between neighborhood factors and physical activity in children with and without ADHD. We used data on 65,680 children aged 6 to 17 years from the 2011-2012 National Survey of Children’s Health (NSCH). Results indicated that pediatric ADHD diagnosis and symptoms can be influenced by family, school, and community characteristics. Findings from studies 1 and 2 demonstrated that school engagement, family factors (socioeconomic status (SES) and adverse childhood experiences (ACE)), and community characteristics (safety and amenities) are associated with ADHD diagnosis either directly or indirectly. Meanwhile, study 3 results showed that environmental characteristics (safety, social support, and access to physical activity resources) are related to physical activity only in children without ADHD. Further research is needed to understand the levels and factors associated with physical activity in children with ADHD. Overall, our results suggested that school and community factors should be examined as potential protective pathways against ADHD in children.

This book argues for the integration of the three perspectives on ADHD.
