Book excerpt: Excerpt from The Relation of Climate to the Treatment of Pulmonary Tuberculosis Tuberculous patients are sometimes puzzled by conflicting medical advice on the subject of climate in its relations to the treatment of their disease. It is not surprising that diverse opinions prevail. From very early times the cure of tuberculosis has been associated, more or less, with certain places, and, as Klebs says: At one time or another, almost any combination of known atmospheric conditions was considered to constitute a climate which had protective or cura tive qualities for consumptives. The reason for this is found, of course, in the fact that pulmonary tuberculosis is a chronic disease, depending for arrest upon a large number of factors, many of which are independent of atmospheric conditions and common to all climates. Another thing which has added to the confusion of ideas on the subject has been the search for immune zones, regions where tuber culosis is rare or absent, the inference being implied that such places offer climatic conditions useful in treating the disease. No region, however, has long continued free from tuberculosis after being reached by modern civilization. Remote districts became infected rapidly as soon as the disease was introduced. Thinly populated areas, what ever their geographical position, become tubercularized wherever cities are built and confining trades established. Latham states that tuberculosis is rife amongst the watchmakers of the high Alps. The death rate from tuberculosis among our native Indians is not appreciably less in those trib% which have always lived in the arid Southwest and other favorable climates. Neither the salts in sea air, the ozone of forests, nor the rarefied atmosphere of mountains are remedial in themselves; they simply typify an outdoor life. C., May 4, 1897 The purpose of this paper is not to build up new theories nor yet as an iconoclast to tear down where we are unable to replace, but to draw the attention of this Association and of those who may read our proceedings, to a few important facts which seem established by experience. In the title of this paper I have referred to the antiseptic treatment of pulmonary tuberculosis which, in the light of our present knowledge of the origin of the disease, seems to me to have a specific tendency to check its progress, though I would in no way mini mize the importance of tonic and supporting treat ment and of good hygienic surroundings. Since the Koch bacillus was recognized as the ulti mate cause, that under favorable conditions, produces tuberculosis, there have been unceasing experimenta tion and careful clinical observation to find some agent or agents that would destroy the microbes with out injury to the tissues upon which they rest or within which they may be imbedded. Contemplating this long line of experiments and scrutinizing the vast array of statistics, we are forced to admit that it has not been demonstrated that any remedy, or any class of remedies, has the power to destroy this bacil lus when ensconced in the human body, with any degree of certainty; yet when we carefully analyze the various methods of treatment that have been more or less successful in checking the progress of the dis ease, we nearly always find that they have been of a distinctly antiseptic character. Then for the second time came Hope; more solid Hope. Given a fairly early case, and three years, and recovery was in the offing. And so we went on cheerfully with Davos. But Davos was not for every one; nor was every case an early 'one. Then came the discovery that lower altitudes would do if certain conditions were obtained; and so arose the great sanatorium movement. But slowly we found that patients could not spend their lives in sanatoriums; and one day on making my way up to one of them in England, I met on the way patient after patient, slouching along, bored to death with themselves and with each other; and even worse in morale than in body. Better discipline and better notions of thera peutics mended some of that; still I could not forget those listless saunterers, and it became evident to some of us, however unwillingly, that Hope was drooping again. The sanatorium was doing a great educative work no doubt; but at the end of its four or six months - what then? To send the patient away with recommendations about light jobs, and a regime, was almost a mockery or quite. What about the wage, and the family to be supported? The next lesson was brought home to me by a visit with other commissioners to certain cities, concerning some such problems. Before me now I see a gaunt hollow-eyed man, coughing, and leaning against the wall as he tried to talk to us, saying that his mates when he came out of the sanatorium - good fellows as they were - had bought him a milk that he might creep round, and earn a bit. The brave wife, shawl on head and mill apron on, had just come from the factory, and apologised for the dirty house - as well she might. The poor thing was working all day at the factory to keep the wolf from the door. All being dragged down together into the pit! What is the value of a good house, or a clean house, if no wages! What is there for the children? And what is to stop the infection! Who then would have the imagination, the initiative, the business capacity, to lift this burden, like lifting a world? Book excerpt: This book tells the story of the thousands of “health seekers” who journeyed to New Mexico from 1880 to 1940 seeking a cure for tuberculosis (TB), the leading killer in the United States at the time. By 1920 such health seekers represented an estimated 10 percent of New Mexico’s population. The influx of “lungers” as they were called—many of whom remained in New Mexico—would play a critical role in New Mexico’s struggle for statehood and in its growth. Nearly sixty sanatoriums were established around the state, laying the groundwork for the state’s current health-care system. Among New Mexico’s prominent lungers were artists Will Shuster and Carlos Vierra, who “came to heal and stayed to paint.” Bronson Cutting, brought to Santa Fe on a stretcher in 1910, became the influential publisher of the Santa Fe New Mexican and a powerful U.S Senator. Others included William R. Lovelace and Edgar T. Lassetter, founders of the Lovelace Clinic, as well as Senator Clinton P. Anderson, poet Alice Corbin Henderson, architect John Gaw Meem, aviator Katherine Stinson, and Dorothy McKibben, gatekeeper for the Manhattan Project. New Mexico’s most infamous outlaw, Billy the Kid, first arrived in New Mexico when his mother, Catherine Antrim, sought treatment in Silver City.

The standard short-course 6-month regimen used since about 1980 has helped to save millions of lives, but co-infection with HIV has had a devastating effect on the epidemic, and multidrug-resistant TB is a growing problem, particularly in communities with a high incidence of HIV. Following the declaration by the WHO in the early 1990s that TB was a 'global health emergency', interest in TB research and the development of new drugs has increased significantly. This volume reviews anti-TB chemotherapy with the emphasis on the actions and pharmacology of existing drugs and the development and evaluation of new agents. A close look is taken at new research regarding our existing drugs by some of the best-known specialists in the field, and historical aspects of these agents are reviewed from a modern perspective. The prospects for the introduction of new drugs and different approaches of how to assess them in adults and in children are discussed in detail. Several papers address the problems associated with drug resistance, its spread and diagnosis.
