Book excerpt: The recent interest in biomedical ethics has resulted in the publication of a great many textbooks in the field. As good as many of these texts are, their attempts to encompass the ethical issues in all areas of health care have left them wanting in comprehensive treatments of specific areas that are of immediate concern to clinicians, and over-comprehensive in areas that are peripheral. While the numerous anthologies of articles have the merit of not presenting students with a single biased approach, they usually have the disadvantage of pre senting articles that are narrowly focused criticisms of other narrowly focused articles. On the other hand, texts by single authors tend to be overly theoretical in their approach. The philosopher teaching ethics in a medical school or in a hospital set ting must tread a difficult intellectual path. There are no "desert island" issues in clinical ethics, and few of the actual cases can be simply stripped down to clear con flicts between two philosophical theories. The horns of vii viii Preface the dilemmas that he encounters are more likely to re semble a stag's horns than a bull's. A philosopher work ing in these settings must quickly change his accus tomed approach to philosophical issues if he is to be effective. Very often he will be presented with an issue that he would prefer to mull over for a year or two, but which will require some sort of immediate direction or resolution because action must be taken.

This novel approach evolved from applying the principles of therapeutic communities to a group relations form of experiential conference. The group relation tradition, however, does not focus particularly on mental health organisations and tends to focus on senior management issues of leadership and authority. In contrast, the learning from action workshops are tailored to the care workers engaged in the direct work, in particular for those working with clients and patients with significant problems with verbal and symbolic communication. The workshops also include an element of research into the unconscious messaging systems employed in making relations, which contribute to therapeutic and other mental health care services. There are also chapters on a related form of workshop - the living and learning experience - which was established primarily for learning about therapeutic communities, which bring further insight to working practices. Those patients receiving intravenous feeding, cytotoxic drugs and new insulin therapy, are fully trained by the health-care professionals (doctors, pharmacists and nurses) to encourage self-reliance and understanding which are vital to success. There are hazards in treating these patients in the home environment, e.g. microbiological risks which are currently being evaluated, failure of equipment, and social problems as patients adjust to a new life in the community. Neverthless, the balance of benefits favour restoration of the patient to a self-reliant positive life at home. The delivery of this important new dimension in therapy was described at a symposium held in the University of York by innovators in the field. Discussion enabled the core of knowledge and experience developed by multi-disciplinary teams to be shared. The problems of supplying parenteral nutrition and medication for the compromised patient in the home and the family situation were aired and solutions presented. The special care for hospital out-patients receiving cytotoxic drug therapy and safe procedures for staff handling these agents was presented and together with responses to questions indicated the direction for UK hospital pharmacists to follow. More information is needed and it is hoped that this account of these proceedings will stimulate interest in these aspects of health care.

The present volume is composed of revised papers on the theme of "Responsibility in Health Care" presented at the Eleventh Trans Disciplinary Symposium on Philosophy and Medicine, which was held in Springfield, illinois on March 16-18, 1981. The collective focus of these essays is the clinical practice of medicine and the themes and issues related to questions of responsibility in that setting. Responsibility has three related dimensions which make it a suitable theme for an inquiry into clinical medicine: (a) an external dimension in legal and political analysis in which the State imposes penalties on individuals and groups and in which officials and governments are held accountable for policies; (b) an internal dimension in moral and ethical analysis in which individuals take into account the consequences of their actions and the criteria which bear upon their choices; and (c) a comprehensive dimension in social and cultural analysis in which values are ordered in the structure of a civilization ([8], p. 5). The title "Responsibility in Health Care" thus signifies a broad inquiry not only into the ethics of individual character and actions, but the moral foundations of the cultural, legal, political, and social context of health care generally.
