Book excerpt: The sellar region and paranasal sinuses constitute the anatomical sections of the skull base in which pathological entities warrant interdisciplinary management. Processes originating in the paranasal sinuses can reach and involve the skull base in and around the sella, sometimes not respecting the natural dural boundary. On the other hand, lesions involving the sellar block, such as pituitary adenomas and meningiomas, can also extend downwards into the paranasal sinuses. The orbit and cavernous sinus may be subject to involvement and infiltration by both paranasal and sellar pathology. The advancement and new achievements of modern diagnostic procedures, such as high-resolution CT, three-dimensional reconstruc tion, MRI, and MRI angiography, as well as the detailed selective angiographic protocols and endovascular techniques, have increased the possibilities for surgical management of this type of pathology with extra- and intracranial involvement. Long-standing and intense inter disciplinary work has led to sophisticated operative approaches which for benign tumors allow total excision with preservation of structures and function, and for some malignant lesions permit an en bloc resec tion via a combined intracranial-extracranial approach. This volume reflects the work and scientific exchange which took place during the IV International Congress of the Skull Base Study Group, held in Hanover. Leading authorities in the basic sciences including anatomy joined with diagnosticians, clinicians, and surgeons from different fields to evaluate the state of the art of this topic in skull base surgery.

Each physician brings a specific expertise, but the goal should be that the end result of all this input is much more than the sum of the parts. Collaboration, insight and overall knowledge of all parties involved are essential to achieve an optimal patient outcome. Oral and Maxillofacial surgeons have traditionally focused on the bony parts of the orbit, but usually have limited knowledge of the intricacies of binocular single vision, and for the Ophthalmologist it may be the other way around. In the past, scientific articles were often written from a single point of view, resulting in tunnel vision for the physician. The multidisciplinary approach to the orbit leads to an improvement in treatment that should not be underestimated and from which the patient benefits immensely. But especially for the physician and the trainees, a multidisciplinary consultation is a real goldmine in which a lot can be gained. Not every medical problem needs to be tackled in a multidisciplinary framework, but problems around the eye socket often do. At Amphia Hospital in Breda, the Netherlands, more than fifteen years ago, a collaboration between Oral and Maxillofacial Surgeons, Endocrinologists, Radiologists and Ophthalmologists was initiated, which has proven to be very fruitful. The main focus was on Graves' Orbitopathy and orbital fractures, but problems associated with these conditions were also frequently encountered. For those who want to follow this example, or for those who are interested in both orbital surgery, anatomy or ophthalmological orbital issues, this book could prove to be complementary.

It is only a short time ago that the major concepts of the anatomy of the so-called cavernous sinus were described in detail. Surgical interventions in this region are very complex, they are time-consuming and require an extensive back ground of experience in surgery of the cranial base. Pioneer anatomical studies of the parasellar region done by Taptas, and the daring direct operative approach introduced by Parkinson pro moted the development of modern neuroradiological intervention proce dures, which were initiated by Serbinenko and further refined by Debrun, Vifiuela and others. The technique of the detachable balloon catheter stimulated surgeons to proceed with the direct operative approach to lesions of the parasellar region. Today, it is hard to imagine a successful man agement of vascular pathologies of this region without a complementary use of the two techniques.

We believe that this interesting yet complex region deserves special attention with a well-orchestrated multi-disciplinary effort. Traditionally, surgical treatments for diseases in this region involve different types of craniotomy and orbitotomy. In this book, it covers the advancement in imaging modalities, medical therapies, operative instruments, radiation therapy namely stereotactic radiosurgery or radiotherapy, management of diseases in orbital apex and periorbital skull base evolve and improve with time. Minimally invasive surgery in terms of mastering neuro-endoscopy contributes to the intervention advancement.
