Book excerpt: Excerpt from The Ophthalmic Record, Vol. 8: October, 1899 I have the pleasure of bringing before the society this year an inter esting case of cyst of the vitreous. It is, as I take it, a simple cyst as we understand that term in surgery. A cyst may be defined as a cavity bounded by a distinct envelope composed of fibrous tissue lined with endothelium and called a cyst wall; or it may be covered by epithelium and certain secreting structures: a cyst may result from the increase of the secretion of an already formed space or cavity by extravasation into it, or the cavity may be a new formation. The patient, L B age 17, is the son of a practicing physician of New York City, and he came to me for' advice January 4, 1897, saying that his sight in the right eye had been defective for about a year; there was no pain or discomfort of any kind, but he thought he could not see as well as he had previously; there had been no change for the past year, and it had become no worse than when he first discovered that it was affected. Book excerpt: Excerpt from The Ophthalmic Record, Vol. 8: September, 1899 The ocular condition may go long undetected, for subjective symp toms may, and generally do, remain absent until the case is very far advanced indeed. So long as the macula escapes and the disordered areas are individually of small extent the eye may be yet very seriously diseased before the vision is notably affected. The state of the eye is not infrequently quite unexpectedly discovered, which is often the patient's as well as the physician's first indication of any serious trouble. When vision does become affected it often rapidly goes down hill, though it seldom entirely disappears. The affection is generally binocular, though a number of monocular cases have been reported. I have myself seen only one such. This is mainly a disease'of late middle life, but cases are found in youth, even in childhood. These latter seem rather to indicate a dependence upon congenital syphilis. It is not really known in what proportion of cases of granular kidney albuminuria develops, though various statements have been'made. The ophthalmosc0pic' picture of typical retinitis albuminurica ought to be of interest to all_practitioners of medicine, because it reveals to the eye in the living body pathological changes which may be more marked in the retina than in other tissues, its vascular supply being terminal, without anastomoses, but is illustrative of lesions present in other and more vital organs, such as brain and kidney. These visible ocular changes have been best described by Mar cus Gunn, but I shall place them under five heads. One or other of them usually predominates, though they may all be present at the same time. They are: (i) vascular changes, (2) hemorrhages, (3) white spots, (4) exudations, (5) neuritis. It is equally certain that in these exceptional ones the results of surgical treatment will be more favorable. In the first place, the globe of the eye by reason of the tensile strength, inelasticity and resiliency of the outer covering, sclera and cornea, tends to preserve its proper curvature and shape unless divided to more than half its circumference. One can easily demonstrate the principle with a hollow rubber ball, by making a clean incision in it with a sharp knife. If the ball is not subjected to pressure the line-of the incision is closed so effectually the place can hardly be found; but the slightest pressure on any portion of the ball will cause the incision to gape. So it is with the eye. Every one who has made many corneal sections has certainly been impressed with this fact. A nice clean section in the cornea, if no pressure on the globe by speculum or other instrument is permitted, will coapt so nicely that the place can scarcely be discovered; but the slightest touch on the globe will cause the wound to open. The ideal condition for primary union in such a wound is to leave it in just the condition we see it when quiescent. How is this best accomplished? We know that the lids are perfectly adjusted to the front surface of the eyes, and are intended for their natural protectors. There was no photophobia or excessive lacrymation. The eye-ball was free from hyperemia. The patient was rather anaemic and was losing weight, about two pounds a week. There was no enlargement of lymphatic glands in other parts of the body, and no history or other symptoms pointing to syphilis. The family history was good, except that the mother suffered from scrofulous glands in childhood, and presented extensive scars there from upon her neck.
