Treatment by competent men without much, if any, result. L. Eye normal; slightly myopic. R. Eye moderate diffuse ciliary injection, and irritability. On temporal side a small elevated vascular nodule in the sclera, running into the cornea. In the substance of the cornea four slightly yellowish gray discrete nodules, of about pin-head size, supplied with blood vessels.' Entire cornea hazy, but surface normal. Pupil fairly atropinized. Book excerpt: Excerpt from The Ophthalmic Record, Vol. 8: September, 1899 The ocular condition may go long undetected, for subjective symp toms may, and generally do, remain absent until the case is very far advanced indeed. So long as the macula escapes and the disordered areas are individually of small extent the eye may be yet very seriously diseased before the vision is notably affected. The state of the eye is not infrequently quite unexpectedly discovered, which is often the patient's as well as the physician's first indication of any serious trouble. When vision does become affected it often rapidly goes down hill, though it seldom entirely disappears. The affection is generally binocular, though a number of monocular cases have been reported. I have myself seen only one such. This is mainly a disease'of late middle life, but cases are found in youth, even in childhood. These latter seem rather to indicate a dependence upon congenital syphilis. It is not really known in what proportion of cases of granular kidney albuminuria develops, though various statements have been'made. The ophthalmosc0pic' picture of typical retinitis albuminurica ought to be of interest to all_practitioners of medicine, because it reveals to the eye in the living body pathological changes which may be more marked in the retina than in other tissues, its vascular supply being terminal, without anastomoses, but is illustrative of lesions present in other and more vital organs, such as brain and kidney. These visible ocular changes have been best described by Mar cus Gunn, but I shall place them under five heads. One or other of them usually predominates, though they may all be present at the same time. They are: (i) vascular changes, (2) hemorrhages, (3) white spots, (4) exudations, (5) neuritis. Swanzy credited him with 10 per cent. Of prolapses, but Only fourteen cases, or per cent. Of all, had required Operative treatment. The oth ers recovered spontaneously, some with persistence of a small un irritative protrusion, not seriously interfering with sight, and in the others the prolapse disappeared. The eye, after a combined extraction, was, by the frequent incarceration Of invisible iris and capsule in the scar, less resistant to Obnoxious influences than when the iris was preserved. He practiced secondary dis cission in about two thirds Of his cases, and few patients Oh jected to it. If successful, it left an ideal eye. The discission can be done at any time, without notable reaction, if the pupil is free from inflammatory products. In skiascopy there are movements of light and shadow in the pupil that can be seen at one-half metre, which are not to be seen at one metre or any greater distance and in many eyes it is these movements, and these alone, that truly indicate the refraction of the eye.
