The difficulty of completing our task amid the many preoccupations of these times has been very great, alike for the Editors, the Contributing Authors, and the Publishers, although we have never wavered in our determination to carry out our project. The work was originally contemplated because in the opinion of the Editors the time had arrived when it was necessary to put into concrete form the great changes through which Gynaecological Practice in this country has passed during the last ten years. The change may be briefly summarized by saying that Gynaecology has become definitely a special branch of surgery, in close touch with abdominal surgery generally, and the old view that the gynaecologist is a physician not a surgeon is no longer tenable. In consequence of this development the interests of the gynaecologist have necessarily broadened. Pelvic disease in women is frequently associated, in the relation of cause or of effect, with diseases of the gastro-intestinal and urinary canals; the gynaecologist has accordingly realized that he must be prepared to deal with whatever condition he may find on opening the abdomen for the relief of disease apparently of pelvic origin. And further, he must be familiar with the special methods which are employed in the investigation of such diseases as those of the kidney, the bladder, and the rectum. Diseases of the female breast fall so obviously into the province of the gynae oologist that no explanation of the inclusion of this subject is called for. The vermiform appendix possesses relationships of unique intimacy with the pelvic Organs, and there is no doubt that cases of appendicitis in women very frequently come under the care of the gynaecologist. During pelvic operations, injury to the intestines necessitating enterorrhaphy or enterectomy frequently occur, with which it is quite obvious that he must be prepared to deal. And further, during convalescence from abdominal operations, intestinal complications may arise which call for a sound knowledge of intestinal surgery. Book excerpt: Excerpt from The New System of Gynaecology, Vol. 2 of 3 Acute Vulvitis. - Cause. - Acute vulvitis is generally gonorrhoeal in origin, but it is occasionally due to injury caused by a douche, the temperature of which was too high or the chemical it contained too strong, or to the septic infection of a wound of the vulva caused during labour or otherwise. Symptoms. - The patient complains of a profuse discharge and of heat, swelling, and pain in the vulva. Pain and swelling in the groins and scalding on micturition may also be present. Signs. - The labia are swollen, tender, and red, and there is a profuse purulent discharge covering the affected parts. The orifices of the urethra and Bartholin's ducts may be inflamed and red. The Special characters of gonorrhoeal vulvitis are described in the Article on Gonorrhoea (vol. I. P. Diagnosis. - The nature of the infecting organisms should be ascertained by a bacteriological examination Of a specimen of the discharge. Apart from the detection of the gonococcus, gonorrhoea may be suspected if the vulvitis was sudden in its onset and the inflammation very acute, so that the pain and swelling are particularly marked. A very profuse and green discharge is highly suggestive of gonorrhoea, and infection of the inguinal glands, urethra, and Bartholin's ducts all point strongly to such an origin. The complications mentioned in Vol. I. P. 574 are also liable to follow an attack of gonorrhoeal vulvitis. The practitioner at the present day is not only keenly alive to the fact that immense advances have been made in medicine since his student-days, but is filled with the laudable ambition to keep abreast of them. To enable him to do this, information must be imparted to him in a clear, concise, and more or less dogmatic form. This is the object aimed at in the present volume, which represents the combined experience of a number of clinical teachers selected by the former editor as men who, while disposed to be conservative, were none the less progressive and free from hobbies. The success which has attended the publication of the volumes of the International Clinics led the publishers to believe that a work on gynaecology treated from a clinical stand-point would find a place in the voluminous literature of diseases of women. Owing to the untimely death of Dr. Keating, the completion of the work devolved upon the present editor, who has as far as possible carried out the plans and wishes of his lamented predecessor. While seeking to preserve that coherence which is desirable in a text-book, he has in the main allowed each contributor to consult his own judgment with regard to the arrangement and phraseology of his article, believing that in this way the individuality of each would be best preserved. In view of the needs of the student and busy practitioner, especial attention is given to diagnosis and treatment, the sections on pathology being less extended than in most text-books on gynaecology. Especial care has been taken in the preparation of the illustrations, a large proportion of which have been drawn or photographed for this work under the immediate supervision of the contributors. She stated that a tubal abscess had been treated at that time. Celiotomy was performed, from which the patient made a good recovery. She continued in comfortable health for several months, when she began to notice an enlargement of the abdo men, especially prominent in the left epigastrie region. As this increase in size continued she suffered from sharp pains in this region, for Which she sought relief at the various gynecological clinics of this city. She first returned to the operator who had originally treated her, and was informed by him that she must not allow a further operation, as it would certainly prove fatal.
