Book excerpt: Pulmonary embolism is a serious, potentially life-threatening cardiopulmonary disease that occurs due to partial or total obstruction of the pulmonary arterial bed. Recently, new improvement occurred in the diagnosis and treatment of the disease. The aim of this disease is to re-review pulmonary embolism in the light of new developments. In this book, in addition to risk factors causing pulmonary embolus, a guide for systematic approaches to lead the risk stratification for decision making is also presented. In order to provide a maximum length of active life and continuation of functional abilities as the aim of new interventional gerontology, the risk factors causing pulmonary embolus in elderly individuals are evaluated, and the approach to prevention and treatment are defined. The risk of the development of deep vein thrombosis and pulmonary embolism, combined with obesity due to immobility, the disease of this era, irregular and excessive eating, and treatment management are highlighted. Non-thrombotic pulmonary emboli are also covered and an attempt is made to constitute an awareness of this picture that can change the treatment and prognosis of the disease to a considerable extent. In addition to the pathophysiological definition of pulmonary embolus, the priority goal of quick and definitive diagnosis is emphasized, and diagnostic strategies are discussed in the book. A numerical analysis of the vena cava filters, which is a current approach to prevent pulmonary emboli recurrences, is presented in the last chapter.

The only distinction between the two diseases is where the clot finally stops. If in the brain, it results in a stroke; if in the heart muscle, a heart attack. Thus the first portion of the book (Chapters 1-4) documents how the mechanism of clot formation was studied through the eyes of a lifelong research scientist who was not a stroke expert, but a stroke survivor. Described in those chapters are entirely new ideas about how and why clots form, and how to prevent them. While my four years of stroke investigation (so far) was unfolding, I simultaneously carefully recorded my daily observations as I experienced an embolic shower-type of stroke. My goal was to assemble a guide for all stroke victims alerting them what to expect, and also my evaluation of a number of treatment modalities, nutrients/supplements etc., designed to save money and frustration to newcomers to the stroke world as well as the initiated. The big difference between this and other accounts of embolic stroke is that in this case it is framed in the context of an aging body. If you do not already know, this distinction is important since there is a fine line between the effects of aging and the effects of stroke. The upshot of this book is to offer my evidence to the reader that stroke and heart attack are largely avoidable diseases. My thought is, if the reader thinks what I say makes sense, they can take the recommended action starting today to minimize their risk. And it is so incredibly simple and virtually cost-free.

In the past twenty years, medical education in China has initiated a series of reforms. The current reforms have mainly been led by the Ministry of Education and Ministry of Health. These initial actions have evidenced both positive and negative attitudes and reactions. Is there a need to make further reforms and changes? If so, in what aspects? This book documents a national investigation of attitudes from teaching staff on the reforms and changes. Nearly 1800 teaching staff from 23 medical universities participated in this investigation. The results suggest that sustainable educational change demands not only supports from policy-makers and leaderships, but also active participation from teaching staff. In order for the implementation of reforms and changes to be successful, two factors are essential from the teaching staff’s perspective. First, it is important for teaching staff to gain a deep understanding of educational reform and change, and second, they should develop appropriate skills to be able to conduct the reforms through their teaching practice. To provide these two factors, institutional facilitation is necessary and crucial.
