Book excerpt: Excerpt from The Johns Hopkins Hospital Bulletin, Vol. 31 We had of course regarded the whole alteration as due to loss of hydrochloric acid with the result that an excess of base had been left in the circulating fluids, so that the natural experiment would be the replacement of the chlorine ion by the introduction of hydrochloric acid. This was done several times but without much success (1620, 1622, 1709 and others), whereas the introduction Of sodium chloride has a life saving effect and the administration of sodium chloride continuously after the operation prevented the development of changes in the electrical excitability, disturbances of the alkali reserves or symptoms. The fate of the Na ion re quires explanation since it appears that so long as the 01 ion is supplied there is no heightened alkali reserve. The prompt effect of calcium salts in stopping parathyroid tetany led to their trial in some of these cases. The injection of calcium lactate, although it exerts a temporary effect in stopping the twitchings or modifying the convulsions, has no such controlling influence, as is seen in the tetany following parathyroidectomy after which an animal can be kept perfectly well for a long time by the proper administration of calcium. This knowledge has come mainly since the introduction of the stains, Sudan III and Scharlach R and is the result of work of Sata, Stangl, Fischer, Herxheimer, Hansemann, Cordes, Erdheim, and others. They have shown that fat is present in almost all of the glands of the body and occurs there under normal conditions in some way associated with cell metabolism. The almost constant presence of fat in the liver has been long recognized and its large amount distinguishes it from other organs, so that the liver is very generally regarded as a fat depot analogous to the adipose tissue. Perhaps here the fat may undergo some change preparatory to its utilization by the other cells and so stand in a different relation to liver cells. That fat is normally present in the epithelium of the salivary glands, pancreas, adrenal, testis, ovary, thyroid, thy mus, sweat and sebaceous glands, and breast is not so generally recognized. Of particular interest to the pathologist is the question of the occurrence of fat in the kidney and heart. In the kidney iof many of the lower animals fat occurs under normal conditions, particularly in the straight tubules where it has a basal position in the cell. In the human kidney it seems that small amounts of fat may be normally present and that Slight amounts of fat cannot without other evidence be considered as pathological. Of the cases showing tuberculous ure teritis, Case 19 healed at once before leaving the sanitarium, Cases 8 and 23 closed after two years and Case 24 is still open after 16 months.
