Book excerpt: Hip replacement operations have become commonplace during the past ten years, but none the less it is well to remember the struggle to overcome the initial difficulties before the achievement of the successful results which are confidently expected today. Certainly, in the 1950s attempts to relieve pain and restore movement to an arthritic hip frequently failed. Such operations which were practised often involved a prolonged period of immobilisation which would now not be tolerated. For progress to be made orthopaedic surgeons had to rely on the loyalty and stoicism of their patients who allowed untried procedures to be carried out on themselves, often without appreciating the possible outcome. These surgical experiments resulted in progress being made, and the failures led to a greater understanding of the problems involved. The importance of collaboration with bio-engineers was also recognised, and the search began for suitable materials with which to make an artificial joint. Progress was made in many centres throughout the world, and surgeons in the United Kingdom were at the forefront in devising original techniques. Although others also produced new models, John Charnley was a pioneer and an innovator who influenced the development of hip relacement more than any other individual. In telling the story of his life this book is inevitably also the story of the operation, but it is not intended to be a scientific evaluation of his work.

The survival of a total hip replacement depends principally on the enduring integrity of the fixation interfaces and of the articular interface. The design of the stem and the material properties of cement largely determine the state of the component-cement inter face, while the bone-cement interface is significantly influenced by both mechanical and biological factors. The surface finish and shape of cementless implants are designed to preserve the integrity of biological fixation (osseo-integration) at the bone-component interface. Once again, both mechanical and biological factors have to be considered, while bioactive coatings accelerate bone ongrowth. Metal-on-polyethylene is the most widely used articular interface. However, it has been suggested that wear of polyethylene is one of the major factors contributing to failure of total hip replacements. The increasing prevalence of total hip replacement in younger patients has stimulated the investigation of alternative, more durable couples -including ceramic-polyethylene, ceramic-ceramic and metal-on-metal. Modularity provides greater intra-operative flexibility, but each new modular interface introduces new mechanisms of failure. These need to be anticipated and appropriate measures taken to avoid them. Hopefully this book will provide a better understanding of the factors that contribute to stable interfaces and long-term survival of total hip arthroplasty.

The Witness Seminar, chaired by Mr Alan Lettin, discussed the remarkable postwar collaboration of British surgeons, engineers and manufacturing firms in the development of efficient alloys, surgical procedures, instruments and the implementation of clean, bacteria-reduced air in enclosed operating theatres, as illustrated by successful prostheses and techniques developed in Norwich (Kenneth McKee), Wrightington (Sir John Charnley), Stanmore (John Scales), Redhill (Peter Ring), and Exeter (Robin Ling and Clive Lee). Early failures - such as loosening from infection, osteolysis, and wear debris - stimulated the search for improved materials and fixation methods, as well as the addition of antibiotics to bone cement to reduce infection.
