Book excerpt: The generalizability of the potential superiority of aerobic interval training (AIT) over moderate intensity continuous exercise (MICE) in improving aerobic exercise capacity (VO2peak) remains elusive in women with coronary artery disease (CAD), as they have been underrepresented in the majority of studies to date. The overarching aim of this doctoral thesis was to optimize exercise training modalities for outpatient, cardiac rehabilitation (CR) programs to maximize the potential gains in quality of life, and survival. The central hypothesis was that a six-month CR program of AIT would significantly improve VO2peak more than six-months of usual care CR involving MICE, in postmenopausal women with CAD. The first of three studies sought to retrospectively determine the effectiveness of progressive AIT in the form of walk/jog intervals in individuals with CAD. Treatment effect analysis revealed a 3.84 mL·kg -1·min -1 superior improvement in VO2peak in response to AIT compared to usual care MICE (p.

Book excerpt: Background: Cardiac rehabilitation demands the most effective and time efficient training methods. Recent studies involving patients with heart disease have found greater improvements in aerobic capacity following high intensity interval training (HIIT) compared to the standard cardiac rehabilitation (SCR) model of continuous moderate intensity exercise. However, these studies have not examined whether interval training is practical for use within the current class based cardiac rehabilitation model. The aim is to determine the practicality and effectiveness of HIIT to improve aerobic capacity compared to SCR using an iso-chronous (40 minute) exercise model. Methods: Coronary artery disease patients were recruited from Henry Ford Hospital's Cardiac Rehabilitation program and randomized to treadmill exercise using HIIT (4 sets of 4 minutes at 80-90% HRR, 3 minutes at 60-70% HRR) or SCR (30 minutes at 60-80% HRR) 3 times per week for 10 weeks. Visual guides were provided to assist patients in following the prescribed exercise model. Recordable heart rate watches were worn during all training sessions. Aerobic capacity was measured at baseline and after 30 training sessions. Results: Eight patients (5m, 3 f) completed the follow-up testing (age 56 yrs, pVO2=24.8 mL*kg−1*min−1, p=.012), but not in the SCR group(+1.8(1.6) 8 mL*kg−1, p=.105) (between group difference=p.047). Conclusion: Heart disease patients who performed HIIT experienced a greater improvement in aerobic capacity than SCR. Further, HIIT is practical for use in cardiac rehabilitation and can be independently performed with the help of a visual guide.

Book excerpt: Although cardiac rehabilitation (CR) is part of the current multidisciplinary approach to the management of cardiovascular disease (CVD), uptake and adherence is low. Reasons are multifactorial and include, time, accessibility, transport issues and motivation. Advances in technology have the potential to enable the delivery of home-based CR (HBCR). The purpose of this PhD research program was to evaluate the efficacy of a technology enabled HBCR program (PATHway) compared to usual care (UC) Study 1 Heart rate (HR) monitoring using wrist-worn watches allows patients to monitor and adjust their exercise intensity to meet their rehabilitation goals. This study assessed the accuracy of commercially available wristwatch HR monitors. There was a significant correlation between wrist-worn monitors and the criterion measure (3-lead Holter monitor) at both rest and during exercise with appropriate limits of agreement. Study 2 Exercise training is one of the core elements of CR. This study assessed exercise session duration, physiological and perceptual responses of participants (n=53) using PATHway, a 6 month, home-based technology enabled CR program. Participants used the PATHway system for an average of 36 min per session at an intensity corresponding to 67% HRR. The average RPE per sessions was 5.2 on the 0-10 Borg scale. Study 3 Study 3 compared anthropometric measures, cardiorespiratory fitness, strength (sit-to-stand, isokinetic and isometric strength and hand grip), vascular structure (cIMT) and function (FMD) and blood biomarkers between CVD patients (n=120) randomized to PATHway and UC. There was no change in cardiorespiratory fitness, strength, vascular structure or function in either PATHway or UC at 6-months.
