Book excerpt: The purpose of this study was to investigate the effects of a fluoride releasing orthodontic primer on demineralization adjacent to brackets. Twenty-two patients were recruited for this study. One premolar was randomly chosen as the experimental tooth, the contralateral tooth was the control. Teeth were visually analyzed for white spot lesions (WSLs). Knoop microhardness was used to determine hardness. Visual examination results showed no significant difference in the number of WSLs observed between Opal Seal and Transbond XT over the duration of this study. Solid conclusions could not be drawn from the results of microhardness testing. Therefore, it cannot be concluded that there is a difference in enamel hardness between Opal Seal and Transbond XT. However, prior to 90 days, teeth showed a significant difference in WSLs. Suggesting a protective effect of Opal Seal that diminished with time. Future studies are necessary to determine the clinical performance of this product.

This thesis attempted to evaluate the effects of the use of APCTM Flash-Free (FF) and conventional adhesives in bonding of orthodontic brackets on plaque formation and retention, WSL development, and bracket failure in-vivo. This was accomplished by collecting plaque samples from around orthodontic appliances and analyzing them using ATP-driven bioluminescence; scanning enamel surfaces using the Canary SystemTM to evaluate WSL development, and counting the number of spontaneous debonding of brackets of the 4 maxillary incisors in the first 6 months of treatment. It was found that there were no significant differences in plaque formation and retention between brackets bonded with FF adhesive and those bonded with conventional adhesives. Furthermore, there were no significant differences in WSL development between FF adhesives and conventional adhesives during the same study period; however, demineralization of enamel surfaces took place in our sample with mesiofacial and distofacial surfaces being most affected. Failure rates of 5.26% and 0% were observed for FF and conventional brackets during the first 6 months of orthodontic treatment, respectively. The present study revealed that the presence or absence of excessive adhesive (flash) around fixed orthodontic appliances may not play a significant role in plaque retention and white spot lesion development in patients undergoing orthodontic treatment during the first 6 months of orthodontic treatment with fixed appliances. Furthermore, FF brackets may fail more often than conventional brackets during the first 6 months of orthodontic treatment with fixed appliances.

Book excerpt: Introduction: The purpose of this study was to evaluate the effect of 1.23% acidulated phosphate fluoride (APF) gel on white spot lesion (WSL) formation utilizing a typical orthodontic treatment interval for the applications. Methods: In this double-blind, placebo controlled trial, 55 patients starting orthodontic treatment, either at Texas A&M University, Baylor College of Dentistry or at a private practice, were randomly divided into two groups (a treatment and placebo). Every eight weeks, the treatment group received an application of Nupro 1.23% APF gel (Dentsply®) for one minute, while the placebo group received a gel treatment for one minute that was exactly the same composition, but lacked fluoride. Patients were followed for 6 or 8 months depending on when their second set of brackets was placed. Pre and post treatment photographs and Fluorecam® readings were used to quantify the development of WSLs. Results: There was no statistically significant difference in the percentage of patients who developed WSLs, the number of new WSLs that developed on each tooth during treatment, or in the number of new WSLs developed per patient when using photographic analysis or Fluorecam® readings. Based on photographic assessments, 37.5% of patients in the fluoride group and 32.3% of patients in the placebo group developed new WSLs (p=0.685), while with Fluorecam assessments 37.5% of patients in the fluoride group and 58.1% of patients in the placebo group developed a new WSL (p=.130). Five patients had pre-existing WSLs (1 patient in the treatment group and 4 patients in the placebo group). Only patients in the placebo group demonstrated worsening of white spot lesions (p=.375). When using the Fluorecam® to quantify WSL development by measuring the size, intensity, and impact of each lesion, the only measurement which demonstrated a significant difference between the fluoride and placebo group was the intensity of the lower left first premolar (P

Due to poor oral hygiene during treatment, accumulation of plaque around orthodontic brackets causes bacterial fermentation reactions producing acids that dissolve the mineral enamel. Such dissolution results in white spot formation, an early sign of dental caries. In principal, to inhibit the appearance of these spots during treatment, fluoridated elastomeric o-rings might be employed. Once placed around brackets, these o-rings should release fluoride therapeutically for at least 30 days, after which the orthodontist may replace them. At the present time, such fluoride-releasing o-rings are not available. In this study, model materials for fluoride-releasing elastomeric o-rings were developed. These materials were fluoride-impregnated poly(ethylene-co-vinyl acetate) (PEVA) films. In vitro release studies of these films were conducted, assessing their capacity to deliver fluoride within therapeutic range (1.2 [less than or equal to] [micro]g F/ring/day [less than or equal to] 51.4) for 30-45 days. Optimal sample and experimental parameters, including (1) fluoride loading in the film (wt% F), (2) overcoat thickness, and (3) temperature that provided therapeutic release, were determined. At 37°C, favorable release was observed for the following: (1) films with overcoat thickness of 0.14 mm and fluoride loadings of 3.9 wt% and 5.7 wt% and (2) films with overcoat thickness of 0.27 mm and fluoride loading of 5.7 wt%. These results are important in providing fundamental guidance for the development of o-rings that combat white spot formation among orthodontic patients.
