Book excerpt: Depression is a widespread condition affecting approximately 7.5 million parents in the U.S. each year and may be putting at least 15 million children at risk for adverse health outcomes. Based on evidentiary studies, major depression in either parent can interfere with parenting quality and increase the risk of children developing mental, behavioral and social problems. Depression in Parents, Parenting, and Children highlights disparities in the prevalence, identification, treatment, and prevention of parental depression among different sociodemographic populations. It also outlines strategies for effective intervention and identifies the need for a more interdisciplinary approach that takes biological, psychological, behavioral, interpersonal, and social contexts into consideration. A major challenge to the effective management of parental depression is developing a treatment and prevention strategy that can be introduced within a two-generation framework, conducive for parents and their children. Thus far, both the federal and state response to the problem has been fragmented, poorly funded, and lacking proper oversight. This study examines options for widespread implementation of best practices as well as strategies that can be effective in diverse service settings for diverse populations of children and their families. The delivery of adequate screening and successful detection and treatment of a depressive illness and prevention of its effects on parenting and the health of children is a formidable challenge to modern health care systems. This study offers seven solid recommendations designed to increase awareness about and remove barriers to care for both the depressed adult and prevention of effects in the child. The report will be of particular interest to federal health officers, mental and behavioral health providers in diverse parts of health care delivery systems, health policy staff, state legislators, and the general public.

Book excerpt: Human capital development, including both cognitive and non-cognitive skills, at early childhood can have large long-term impacts not only on individual labor market outcomes and socio-economic success but also on economic growth. Studies suggest that parents, especially mothers in single-parent and low-income families, play an important role in developing the skills of their children at different stages of childhood. However, children from disadvantaged environments face many risk factors, such as poverty, parental health problems, and limited parental education. In particular, maternal depression is an "adverse early environment" for child development and is negatively associated with the quality of parenting practices and mother-child relationship. In turn, the lower quality of parenting practices and mother-child relationship might harm children's development. This thesis examines the effect of maternal depression on pre-school children's development in terms of their cognitive abilities and behavioral problems by using longitudinal data from the Fragile Family and Child Well-being Study (FFCWS). By using a family (child-mother) fixed effects (FE) model, we provide new evidence to the literature that maternal depression imposes a big risk for child development in fragile families, which are mainly those unwed parents and their non-marital childbearing. Specifically, maternal depression tends to reduce standardized Peabody Picture Vocabulary Test (PPVT) score by about 16.4 percent of a standard deviation. We also find that maternal depression has a similar adverse effect within non-marital families, but the effect gets more adverse (about 18.5 percent of a standard deviation) within non-marital families with higher poverty level. In addition, we find that maternal depression has a much larger adverse effect on girls (about 23.2 percent of a standard deviation), as well as children whose mothers have an education level of high school or above (about 24.4 percent of a standard deviation). Regarding child's behaviors, maternal depression tends to increase of the child's Anxiety/Depression problems from ages three to five. Specifically, maternal depression tends to increase the Anxiety/Depression Index, on average, by about 20.9 percent of a standard deviation. We also find that maternal depression has a much larger adverse effect (about 31.3% of a standard deviation) on children from households living below the Local Poverty Line (LPL). In addition, we find that maternal depression has a moderately large effect on Black children (about 36.4 percent of a standard deviation), as well as girls (about 30.7 percent of a standard deviation). We find no evidence that maternal depression affects contemporaneous child's Withdrawal behavior, but find weak evidence that maternal depression affects child's Aggression behavior as a whole. The findings in our study have important implications regarding public policies for dealing with the problem of maternal depression and child development within fragile families.

Many of these women are mothers whose depression negatively affects their parenting quality and the wellbeing of their children. Using Ordinary Least Squares regression and the Early Head Start Research and Evaluation (EHSRE) Study, this paper estimates the effect of maternal depression on parenting quality and child outcomes and examines the ability of Early Head Start participation to reduce the negative impact of maternal depression on parenting and child outcomes. The findings support the hypotheses that maternal depression negatively impacts parenting quality and child outcomes and that participation in Early Head Start compensates for part of the negative effect of maternal depression on children. Furthermore, the findings show that maternal educational attainment has a bigger impact than Early Head Start in increasing parenting quality and child wellbeing, as measured by child success on cognitive, social, and emotional assessments. These findings support policy interventions aimed at providing various types of mental health services to low-income parents, increasing participation in Early Head Start, and promoting maternal educational attainment.

Book excerpt: Context: Cognitive development is an important building block of health and wellbeing because it equips individuals with the necessary skills to increase control over and improve their health as they age. To ensure healthy cognitive development, we need to better understand how the early childhood environment influences development. Maternal depression in early childhood is a prevalent (11-25%) public health problem and a robust risk factor for poor cognitive outcomes in the child. However, there remain important gaps in our understanding of the longitudinal mechanisms through which it influences children's cognitive outcomes. Understanding why and for whom this association exists can inform the development of interventions to promote healthy child development. Objectives: The overall aim of this dissertation is to examine mediating and moderating factors of the association between maternal depression and children's cognitive development using the Developmental Model for Understanding Mechanisms of Transmission as a framework. Specific objectives - each corresponding to a research paper forming the body of this dissertation - are to: (1) summarize existing evidence on mediators of the association between maternal depression and children's cognitive development; (2) examine the mediating role of children's school experiences in the association between maternal depression and academic performance, including the moderating role of child's sex in these associations; (3) examine the role of gene-environment mechanisms in the association between maternal depression and children's cognitive school readiness; and (4) perform a meta-analysis of the moderating role of child's sex in the association between maternal depression and cognitive outcomes across childhood and adolescence. Methods: Systematic review and meta-analytic methods were used to address objectives 1 and 4. For objective 2, data from the Québec Longitudinal Study of Child Development were used. Maternal depression was self-reported (between child's ages 5 months and 5 years); peer victimization and school engagement were parent, teacher, and child-reported (6-10 years); and academic performance was assessed when children were 12 years. Data from the Québec Newborn Twin Study were used for objective 3: self-reported maternal depression (6 and 18 months) and assessment of children's cognitive school readiness (5 years). Structural equation models, adjusted for confounders where necessary, were used to estimate associations in both cohorts. Results: The studies in this dissertation show that (1) only a handful of studies have examined mediators of the association between maternal depression and cognitive development, with most identifying maternal cognitions, behaviours, and affect as a mediator; (2) school engagement mediates the association between maternal depression and academic performance in girls only; (3) genetic contributions to children's cognitive school readiness decrease - while environmental contributions increase - as the level of maternal depression increases; and (4) sex moderates the association between maternal depression and cognitive development, with boys at higher risk of poor cognitive outcomes. Conclusion: These findings suggest that addressing maternal cognitions, behaviours, and affect can help mitigate the negative effect of maternal depression on children's cognitive outcomes at the population level. They also suggest that boys are more negatively affected by maternal depression, and that for girls, addressing the impact of maternal depression on their school engagement may lead to improved cognitive outcomes. Further research on additional mediators and moderators can help strengthen our understanding of the association between maternal depression and children's cognitive development.
