Book excerpt: Research on HIV treatment has indicated there are numerous factors that contribute to medication nonadherence, and therefore, risk of transmission. Mental health issues, substance abuse, and co-occurring physical health issues have shown to have negative impacts on HIV medication adherence. Additionally, living in a rural area compared to an urban region negatively impacts adherence. African American males fare the worst in HIV treatment, having the least engagement with HIV care resulting in late stage HIV upon diagnosis, and are within a group that are at the highest risk of infection. Gender role socialization theory and research on male help-seeking behavior guides the current study that aims to investigate predictive factors and the intersection of these factors on HIV medication adherence. This theory states that males are less likely to seek medical and mental health care, or underreport symptoms. Participants were 133 Ryan White Part B clients who have received support and treatment for HIV for at least two years. A retrospective longitudinal design examined how intersecting factors predicted viral variability and CD4 counts in HIV-positive (HIV+) individuals. It was hypothesized that mental health issues, substance abuse, physical health comorbidities, region (e.g., South versus North), and race (e.g., African American/Black versus White counterparts) would impact viral variability and CD4 counts. Results showed that substance abuse, changes in urban/rural residence, and physical health comorbidities impacted viral variability or CD4 counts, and ultimately, substance abuse had the greatest negative impact. Implications of these findings can guide future research and clinical interventions for co-treating substance abuse and HIV.

Book excerpt: The present study investigated whether cognitive function was associated with adherence to highly active antiretroviral therapy (HAART) in 32 asymptomatic and symptomatic HIV-infected men and women. Participants (53% African American) were recruited from Louis Stokes Cleveland Veterans' Affairs Medical Center (LSCDVAMC) and were interviewed and tested on site. Cognitive domains assessed were attention, executive function and information processing speed. Other variables included adherence, substance use, depression and medical variables. A multiple hierarchical regression analysis revealed a significant model fit for the association between cognitive variables and adherence at one month. High levels of previously undetected cognitive impairment were found. High levels of adherence were also reported. People with HIV/AIDS may benefit from monitoring of adherence at each visit and utilization of appropriate interventions as needed. Further, this population may benefit from regular screening for cognitive impairment and referral for complete neuropsychological evaluation as appropriate. Further studies need to investigate the relationship between cognitive function and medication adherence in people living with HIV.

Book excerpt: People living with HIV (PLWH) display disproportionally high levels of psychopathology (i.e., posttraumatic stress disorder symptoms [PTSS] and depressive symptoms) that may interfere with optimal medication adherence and health-related quality of life (HR-QOL). The adverse consequences of non-adherence and diminished HR-QOL, including reduced length of survival, highlight the need for intervention in PLWH. Though recent research suggests that the removal of psychological barriers (i.e., PTSS and depressive symptoms) may result in improvements in adherence and HR-QOL, this has never been directly tested in PLWH. We previously demonstrated the success of prolonged exposure (PE) therapy at improving PTSS and depressive symptoms in PLWH. The proposed study extends these findings by determining whether PE also impacted self-reported and electronically monitored medication adherence, and self-reported adherence self-efficacy, physical health symptoms, and HR-QOL in 65 PLWH. Participants were randomly assigned to either the PE (n = 40) or weekly monitoring/wait-list control group (n = 25), and completed assessments at baseline, post-treatment, and 3-months post-treatment. Following the 3-month assessment, control group participants were offered the intervention, and all PE recipients completed the 6-month assessment. Multilevel growth curve models were conducted on the completer and intent-to-treat samples using the HLM software, and exploratory mediation analyses were conducted in SPSS. Compared to the controls, PE recipients did not experience direct benefits in self-reported or electronically monitored medication adherence or HR-QOL throughout 3-months post-intervention; however, physical health symptoms improved for participants in both groups, and PE recipients reported increased adherence self-efficacy throughout 3 months. Mediation analyses further demonstrated that the removal of psychological barriers led to better short-term adherence and health.

Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.
