Book excerpt: With childhood obesity on the rise, there is increasing concern about the prevention and treatment of obesity. Obesity occurs in children/adolescents and is associated with increased health risks and psychosocial problems. Standard treatment options for obesity can include dietary, physical activity, and behavioral therapy. In this project, an enhancement to the current standard treatments was conducted. Motivational Interviewing has been shown to aid in changing weight-related behaviors for adults. The present study evaluated the effects of Motivational Interviewing on children/adolescents preceding an obesity intervention. A multi-method survey approach was utilized. Results indicated that four out of the sixteen participants showed improvement in their readiness to change weight-related behavior. Furthermore, the majority of parents reported that their children showed increased awareness in weight related behaviors, improvement in mood, and positive lifestyle changes. Parental involvement appears to be one significant component in obesity treatments, such that parents supported and reinforced children's healthy weight-related behaviors. Future studies should include a larger pool of participants with a control group and a specific parental involvement component.

Motivational interviewing (MI) is an emerging intervention for adolescent health risk behavior that has promise. Despite its potential fit in primary health care contexts, primary care providers tend to express reticence in learning or adopting MI practices. This program evaluation project assessed the usefulness of Motivational Interviewing (MI) by primary care providers with adolescents in school based health centers that was implemented by a large randomized clinical trial (RCT) in southwest United States high schools. Methods: As part of the RCT, the primary care providers were required to do MI sessions with 66, 45, 25, and 91 youth participants respectively at their School-based site. It was anticipated that providers would be increasingly comfortable with MI between their first and final participant and that youth would be increasingly satisfied as provider comfort increased. To evaluate this, as part of the RCT, a set of primary care providers working with youth in school based health centers were trained through didactic sessions, homework, and role-play. School-based providers were provided an introductory training in MI, MI reading materials, a study manual, and were required to participate in bimonthly tele-coaching to ensure treatment integrity and fidelity. Primary care provider and adolescent participant survey data from session 1 of the RCT was analyzed to determine provider comfort with MI and adolescent satisfaction with MI. Summary of findings: Despite the standard training practices, providers showed a range of interest in comfort with the intervention. Effective strategies and interventions to help reduce overweight and obesity rates in children are needed more than ever. The following proposed study will examine the counseling technique called motivational interviewing, as an interventional method for increased weight loss. This research proposal will specifically ask if integrating motivational interviewing into a 6-week, family-based, weight management program located in Los Angeles serving the Latino families, is an effective, feasible, and cost-effective intervention that will result in increased weight loss for participating children.

Book excerpt: Adolescent obesity has increased exponentially over the past three decades in the United States. In response, behavioral interventions have been developed and implemented to address this epidemic; however, treatment adherence is often suboptimal. Motivational interviewing (MI) is a directive, person centered approach to reducing patient ambivalence about change, which has been shown to increase engagement in obesity interventions. The current study investigated the underlying process of MI by exploring two different, but related pathways that explain how change happens (e.g., the technical and relational hypotheses) in the context of a multidisciplinary obesity intervention with African American adolescent girls (N = 30). Results demonstrated that MI-consistent skills were associated with client "change talk" or language consistent with their desire, ability, reasons, need, taking steps or commitment to change. Specifically, clinician reflection of client change language and support of client autonomy were especially important for increasing change language. Furthermore, client language related to change was associated with three-month treatment adherence, and increased fruit and vegetable intake. However, the clinician's ability to embody the MI "spirit" was not related to either client language or three-month treatment adherence. Findings suggest that the technical aspect of MI helps explain this approach's effects on treatment adherence among African American girls with obesity.
