Book excerpt: Regulatory T cells (Tregs) are a vital component of the T cell immune system by their ability to control T cell responses that would lead to autoimmune disease. Tregs also protect damaged normal cells that are healing from T cells programmed to kill any abnormal cells in the body. Cancer (tumor) develops from normal cells and can express normal self-antigens. Tregs protect precancerous cells as if they were healing damaged cells and inhibit the anti-tumor T cell response by the use of advanced effector mechanisms, which stop the T cell immune system from effectively removing the tumor. The function of cells involved in this process is controlled by the cell membrane activation of intracellular translational pathways interacting with the nucleus that produces transcriptional proteins, which control cellular behavior such as secretion of lymphokines or cell proliferation. This book examines the function and related translational pathways of Tregs, anti-tumor T cells, and cancer cells. It relates that information to the treatment of cancer by examining human clinical trials of new immune cell-based treatments (immunotherapy). The book also proposes ways to improve those treatments by manipulating the translational pathways of immunotherapeutic cells. The hope is that these new treatment proposals stimulate positive thought about the future of cancer immunotherapy.

However, increasing evidence suggests that tumor-infiltrating immune cells may have a dual function: inhibiting or promoting tumor growth and progression. Although regulatory T (Treg) cells induce immune tolerance by suppressing host immune responses against self- or non self-antigens, thus playing critical roles in preventing autoimmune diseases, they might inhibit antitumor immunity and promote tumor growth. Recent studies demonstrate that elevated proportions of Treg cells are present in various types of cancers and suppress antitumor immunity. Furthermore, tumor-specific Treg cells can inhibit immune responses only when they are exposed to antigens presented by tumor cells. Therefore, Treg cells at tumor sites have detrimental effects on immunotherapy directed to cancer.

IDO may also participate in the functional tolerance of the immune system towards tumors. Foxp3+ Tregs are major contributors to tumor-induced immune suppression, and emerging evidence links the IDO pathway with Treg activation. IDO-expressing dendritic cells (DCs) can drive the differentiation of naive CD4+ T cells into Foxp3+ Tregs. IDO+ DCs can also directly activate mature, preformed Tregs to mediate enhanced suppression. In experimental models, IDO also stabilizes the suppressive Treg phenotype and prevents inflammation-induced reprogramming of Tregs into pro-inflammatory (T-helper-like) cells. IDO may thus represent an important regulatory checkpoint that enhances Treg activity in tumor-bearing hosts. Drugs that target the IDO pathway may assist in reducing Treg-mediated suppression during antitumor immunotherapy.

Recent developments in the field of immuno-oncology has led to unprecedented clinical benefits for a broad spectrum of solid tumors. However, immunotherapy (IT) approaches currently have several limitations including (i) low response rate; (ii) development of resistance and (iii) causing severe immune-related adverse effects (IrAEs), which underline the importance of adequate patient selection. Importantly, IT holds promising synergistic potential when combined with standard-of-care chemotherapy, radiotherapy (RT) and anti-angiogenic therapy (AAT) as part of multi-modal oncologic treatment regimes. Published data suggest that there are potential synergy between RT and AAT, which ultimately could help potentiate the response to IT. However, the complex interactions between RT and IT and/or AAT remain poorly understood. Many research questions including optimal timing, scheduling and dosing, as well as patient selection and side effects of combined therapy approaches, remain to be addressed. This Research Topic aims to give a comprehensive overview of the current field with particular emphasis on the future outlook of RT and AAT as complementary approaches to improve IT in solid tumors.
