Book excerpt: For adult subjects with temporomandibular disorders (TMD) of myofascial or joint origin, including arthritic changes, a self-care protocol with postural correction and active and passive jaw exercises decreases subjects' pain at rest and with movement as well as increases jaw range of motion greater than no treatment, self-care protocol alone or surgery. Self-care protocol consists mainly of the application of heat and/or ice, relaxation techniques, finding the resting jaw position, eating a soft diet, and massage to painful muscles. The majority of articles matched our clinical question with the largest variance being what type of TMD was allowed in each study. Each type of TMD was covered in at least one article. Unfortunately, half the articles were under a 5/10 on the PEDro scale ranking of research methodology and therefore of low quality making us unsure how applicable the outcomes are to the general population. Also, because there are few MCIDs for outcome measures used in the studies, it is not known how much of the statistical significance is of clinical importance.

Book excerpt: Temporomandibular disorders (TMDs), are a set of more than 30 health disorders associated with both the temporomandibular joints and the muscles and tissues of the jaw. TMDs have a range of causes and often co-occur with a number of overlapping medical conditions, including headaches, fibromyalgia, back pain and irritable bowel syndrome. TMDs can be transient or long-lasting and may be associated with problems that range from an occasional click of the jaw to severe chronic pain involving the entire orofacial region. Everyday activities, including eating and talking, are often difficult for people with TMDs, and many of them suffer with severe chronic pain due to this condition. Common social activities that most people take for granted, such as smiling, laughing, and kissing, can become unbearable. This dysfunction and pain, and its associated suffering, take a terrible toll on affected individuals, their families, and their friends. Individuals with TMDs often feel stigmatized and invalidated in their experiences by their family, friends, and, often, the health care community. Misjudgments and a failure to understand the nature and depths of TMDs can have severe consequences - more pain and more suffering - for individuals, their families and our society. Temporomandibular Disorders: Priorities for Research and Care calls on a number of stakeholders - across medicine, dentistry, and other fields - to improve the health and well-being of individuals with a TMD. This report addresses the current state of knowledge regarding TMD research, education and training, safety and efficacy of clinical treatments of TMDs, and burden and costs associated with TMDs. Book excerpt: Background: Therapeutic exercises have been used by Physiotherapists to manage musculoskeletal disorders, specifically Temporomandibular Disorders (TMD) for some time but they are less commonly used in dental practice. The current evidence about home exercise programs to treat TMD is weak despite some generally recognised benefits such as low cost, reversibility and reinforcement of the patient's coping skill, essential for management of chronic conditions like TMDs. Objective: To evaluate the effectiveness of home exercise programs used as part of dental management for TMDs. Methods: A search of the Cochrane Central Register of Controlled Trials (Cochrane Library Issue 7, 2012), Medline, Web of Knowledge and Scopus databases (January 1966 to February 2012) and reference lists of articles. Only randomized and quasi-randomised controlled trials published in English that compare exercise therapy to treatments without exercise therapy for TMDs were included. Two authors independently assessed the suitability of trials for final inclusion and also contacted study authors for additional information as required. Results: Eleven trials involving 688 people were included. Two trials compared exercise programs to no-treatment control subjects for Anterior Disc-Displacement with Reduction (ADDwR). A significant risk ratio of 0.44 (0.29-0.59) (44% improvement rate) for improvement in clicking after at least four weeks of exercise therapy was revealed. Three trials compared exercise programs for Anterior Disc-Displacement without Reduction (ADDwoR) with no-treatment control subjects. An overall improvement in the pain-free range of maximal mouth opening with exercise intervention was observed although the differences were not statistically significant in one of three trials. Six trials compared exercise programs for Myofascial Pain patients with no-treatment controls. A standardised mean difference in jaw pain score of 0.73 (95% CI -0.63 - 2.10) indicates no effect from exercise therapy for this TMD subgroup. Conclusion: Exercise therapy appears to be effective for treatment of some TMD subcategories and unlikely to present any adverse outcomes.
