Book excerpt: The Purpose of this study was to determine the most effective method of recovery from an exhausting workout by comparing passive, active, and cold-water immersion (CWI). Exercise induced muscle damage (EIDM) is the micro trauma that occurs during strenuous exercise. Recovery is the physiologic response period following intense exercise where the body repairs EIMD, adapts to exercise, and refuels tissue for performance. There are many physiological and psychological factors involved in recovery. For example, active recovery may be advantageous because it promotes greater blood circulation to the damaged tissue. CWI may be advantageous because it serves to limit inflammation. By manipulating these factors it was hypothesized that CWI would be the most effective method of recovery. The Hanover College Institutional Review Board approved this study.

Book excerpt: The purpose of this study was to determine whether cold-water immersion or foam rolling is more effective treatment of delayed onset muscle soreness. No other study has directly compared these two variables for DOMS. Eight women's soccer players participated as subjects. They each completed a DOMS-inducing protocol which involved eight sets of ten reps of a trap bar deadlift. Each subject performed all the different recovery methods of cold-water immersion, foam rolling, and a control/no recovery. They performed the recovery method immediately after exercise, as well as 24 and 48 hours after. There was a minimum of three days in between each exercise day to allow for appropriate recovery. Delayed onset muscle soreness was measured by a perceived soreness on a visual analog scale, as well as a pressure pain threshold value. It was hypothesized that measurements on the visual analog scale would be lowest for CWI, moderate for foam rolling, and highest for control. It was also hypothesized that pressure pain threshold values would be highest for CWI, moderate for foam rolling, and lowest for the control. It was found that CWI had the lowest visual analog scale means as well as the highest pressure pain threshold means, which indicates that it was the best recovery method for alleviating DOMS. Data concluded that (1) cold-water immersion and foam rolling are both effective forms of recovery. When compared to a control group, they are significantly better at reducing delayed onset muscle soreness. (2) Cold-water immersion is a slightly better form of recovery, in terms of reducing DOMS, than foam rolling. (3) More research needs to be done directly comparing the two, as well as more research in general on their ability to decrease DOMS in general.

The purpose of the present study was to investigate the effects of CWI following an acute bout of intense aerobic exercise on 5 km run performance, perceived muscle soreness (PMS) , range of motion (ROM), thigh circumference, and perceived exertion. Nine endurance trained males (35.9 ÷ 7.8 years old) completed 2 trials consisting of 8 repetitions of 1200 m at a running pace equal to 75% ofVO2peak. Following exercise, subjects underwent CWI (12 mm at 12°C) on the lower body or no treatment. A timed 5 km run was completed 24 hr post exercise. Book excerpt: Physiological responses after maximal and submaximal exercise are routinely monitored in a plethora of diseases (e.g. cardiovascular diseases, cancer, diabetes, asthma, neuromuscular disorders), and normal populations (e.g. athletes, youth, elderly), while slower or irregular post-exercise recovery usually indicates poor health and/or low fitness level. Abnormal post-exercise recovery (as assessed via blunted post-exercise heart rate dynamics) helps to predict the presence and severity of coronary artery disease, while differences in recovery outcomes in athletes might discriminate between fit and unfit individuals. Disturbances in post-exercise recovery might be due to acute or persistent changes in: (1) adaptive responses mediated by the autonomic nervous system and vasodilator substances, (2) cellular bioenergetics, and/or (3) muscular plasticity. Preliminary evidence suggests possible role of time-dependent modulation of nitric oxide synthase and adenosine receptors during post-exercise recovery, yet no molecular attributes of post-exercise recovery are revealed so far. Currently several markers of post-exercise recovery are used (e.g. heart rate measures, hormone profiles, biochemical and hematological indices); however none of them meets all criteria to make its use generally accepted as the gold standard. In addition, recent studies suggest that different pharmacological agents and dietary interventions, or manipulative actions (e.g. massage, cold-water immersion, compression garments, athletic training) administered before, during or immediately after exercise could positively affect post-exercise recovery. There is a growing interest to provide more evidence-based data concerning the effectiveness and safety of traditional and novel interventions to affect post-exercise recovery. The goals of this research topic are to critically evaluate the current advances on mechanisms and clinical implications of post-exercise recovery, and to summarize recent experimental data from interventional studies. This knowledge may help to identify the hierarchy of key mechanisms, and recognize methods to monitor and improve post-exercise recovery in both health and disease.

Book excerpt: Cold-water immersion (CWI) is a popular post-exercise intervention, assumed to be effective in accelerating muscle recovery after strenuous exercise. Previous evidence has proposed that acute increases in oxidative stress induced by SIE cause a long-lasting depression in submaximal force generation called prolonged low-frequency force depression (PLFFD). The focus of this thesis was to test the hypothesis as to whether CWI can enhance recovery of neuromuscular function by decreasing oxidative stress and mitigating the extent of PLFFD. PLFFD was examined by measuring torque from evoked stimulation at low- and high-frequencies before, immediately after and 24h post-exercise. Results showed that PLFFD was observed at 24h post-exercise but no difference was seen in the extent of PLFFD between the cooled and control leg. These results do not support any benefit of acute 30 min post-exercise CWI on the recovery of skeletal muscle function at 24h following SIE.

Book excerpt: Cardiac Dynamics is the name of a relatively young field of study, born from the fruitful interaction between branches of two different disciplines: medicine and physics. "Dynamics" is the branch of physics which deals with the action of forces on bodies or particles in motion or at rest. "Cardiac" relates to the clinical field of cardiology but also to cardiophysiology, both of which are specialized branches of medicine. Narrower than the well established field of Hemodynamics, Cardiac Dynamics is restricted to dynamic phenomena occurring in and around the heart. The mathematical treatment of such phenomena, however, is vastly more complex because of the intricate nature of the mechanisms involved in the cardiac action. Thus, whereas hemodynamics is concerned with predominantly passive (visco-) elastic structures - vessels - containing time-variant flow of viscous flui- blood -, the mechanical study of the heart requires additional con siderations such as: active elastic components representing the contractile mechanism of cardiac muscle, complex geometry and fiber structure in the myocardial wall, autoregulatory mechanisms, and intricate flow patterns associated with valve motion. Viewed in this light it is not surprising that attempts to describe ventricular pump function and to quantify contractile performance have not reached the level of sophistication which is common in e. g. arterial hemodynamics. For the same reason, many of the often simplified approaches to describe ventricular mechanics failed to stand up to more rigorous theoretical, experimental or clinical testing.
