Book excerpt: Individuals with TBI are among the fastest growing population served in the state – federal vocational rehabilitation programs. The increasing rates of individuals with TBI accessing VR programs and the higher rates of unemployment among this population have been reported in the literature. Using the Rehabilitation Services Administration National Case Services Report (RSA-911) fiscal year (FY) 2012, this study examined the predictors of employment outcomes. In order to determine the effective VR services across groups, the moderating effects of race/ethnicity, comorbid disabilities on the effects of VR services on employment outcomes of individuals with Traumatic Brain Injury (TBI) were also examined. A logistic regression was run and results showed a significant disparity in the rate of successful employment outcomes of individuals who are White versus those individuals from racially/ethnically diverse backgrounds, especially Blacks. After controlling for demographic characteristics, on-the-job support, job placement, and on-the-job training emerged as the most important positive set of VR services predictors of successful employment outcome, regardless of race/ethnicity. Maintenance, assistive technology, and job search assistance emerged as the second set of positive predictors. Diagnosis and treatment service was significantly negatively related to employment outcome, regardless of the race/ethnicity of the individual with TBI. College training and assessment services were found to significantly increase the odds of being successfully employed for Blacks compared to the odds of being successfully employed for individuals who are White. Transportation and supported employment services significantly increased the odds of being successfully employed for individuals from ‘Other’ ethnic minority groups (i.e., American Indian or Alaska Native, Asian, Native Hawaiian or Other Pacific Islanders) compared to the odds of being successfully employed for those individuals who are White. VR Service variables were stronger predictors of successful employment than demographic variables and pre-employment status as application. Depression was negatively related to successful employment outcome, but did not moderate the effect of VR services on employment outcome. Limitations, implications, and directions for future studies are also discussed.

Although some people with traumatic brain injury return to work with minor adjustments, high proportions do not return to former employment or find work after their injuries. This study examined the association of types and degree of limitations to functional capacities, and competitive employment outcomes; and selected VR services, and competitive employment, after controlling for demographic variables. The degree of limitation across all areas of functional capacity, used by VR counselors to determine severity of disability and priority for services, were not found to be significantly related to competitive employment among customers with TBI.

Book excerpt: Abstract: The purpose of this study was to evaluate the types of vocational rehabilitation services provided to individuals with psychiatric disabilities through the federal-state vocational rehabilitation program and the relationship of these services to employment outcome. With the cooperation and support of the Rehabilitation Services Administration (RSA), the data for this study consisted of consumers with psychiatric disabilities who received services during federal fiscal year 2002 (n = 79,967). In addition, this study evaluated and described the demographic variables that were involved in successful employment outcome such as gender, race/ethnicity, age, diagnosis, significant disability, years of education, and disability benefits. To ensure that the regression model had both generalizability and transferability to the population and situations for which it was intended, model validation was accomplished by splitting the sample approximately in half, resulting in a top (n = 40,000) and bottom sample (n = 39,967). Correlational analyses and chi-square analyses were used to assess the bivariate relationship between the dependent variables (program closure status) and each of the predictor variables to identify high intercorrelations and assist in determining which variables were entered into the logistic regression model. The best fit model was identified and interpreted based on the top and bottom halves of data. Neither race/ethnicity nor gender were predictors of employment outcome and after being force entered into the initial logistic regression model, race/ethnicity was dropped in the final model. However, job placement services, on-the-job services, job search assistance and other support services were somewhat predictive of a positive employment outcome for individuals with psychiatric disabilities. This may be due in part to the use of these services as a conduit toward achieving competitive employment, thus consumers receiving such services would be closer to achieving employment. In contrast, recipients of disability benefits, most especially individuals receiving SSI and SSDI, were the least likely to have an employment outcome after receiving services. Likewise, significant disability was also associated with fewer closures with employment outcomes. Many of the results of this study mirrored the existing literature on individuals with psychiatric disabilities, suggesting that additional research is needed to clarify the conflicting results of many studies. Model limitations and recommendations for further research are suggested.

Book excerpt: Abstract: The purpose of this study was to explore the relationship between African American consumers with a primary mental health (emotional and behavioral) disability and vocational rehabilitation outcomes through the vocational rehabilitation (VR) system in the United States. The participants in this study were all consumers who sought public VR services nationally for the fiscal year 2002 (October 1, 2001 to September 30, 2002). The sampling frame in the investigation consisted of 1,600 VR consumers. The results from the MANCOVA indicated there was a significant relationship between race (African Americans and Hispanics), age, and total days spent receiving services, as well as age and total case expenditures. Multiple regression analysis was conducted to determine if any differences existed in the type of closure, reasons for closure, and work statuses at closure based on race, age, gender, and educational level at application. The regression models indicated that consumers' race and educational level at application affect type of closure. Consumers' age, sex, and race influence reason for closure. Also, African American consumers were three times more likely to be unemployed or employed without supports at closure. Hispanic Americans were five times more likely to be unemployed at closure, nine times more likely to be employed without supports at closure, and 12 times more likely to be self-employed at closure. Logistic regression was used to ascertain whether race, age, sex, and educational level at application influenced successful closures (Status 26). The results from the regression model revealed race had a significant effect on closure type (successful or unsuccessful). European Americans are more likely to have their cases closed successfully when compared to other racial/ethnic groups. The chi-square test indicated the six most beneficial services African Americans received when their cases were successfully closed (Status 26). These services were college/university training, business/vocational training, substantial counseling, job finding services, transportation and maintenance.

Book excerpt: Numerous studies have indicated racial and ethnic disparities in the vocational rehabilitation (VR) system, including differences in eligibility, services provided, and employment outcomes. Few of these studies, however, have utilized advanced multivariate techniques or latent constructs to measure quality of employment outcomes (QEO) or tested hypothesized models for the relationship between race, personal history characteristics, and VR outcomes. Furthermore, few VR disparities studies have examined southwestern states such as Texas, which has large Hispanic and Black populations. The purpose of this study was to utilize structural equation modeling (SEM) to examine several implied conceptual models for the relationship between race, personal history characteristics, and VR outcomes for White, Black, and Hispanic participants in the Texas VR system. The implied conceptual models were tested for goodness of fit and multiple-group invariance. A measurement model for QEO, a latent construct, was tested and used in the study. QEO was measured by three indicator variables and evaluated using confirmatory factor analysis. A MIMIC model was tested to assess racial/ethnic variation in QEO. The MIMIC results were compared to a multiple regression approach. In addition, a path model and logistic regressions were conducted to assess racial variation in VR closure status among consumers who were unemployed at application to VR. All models were retested with an independent sample to assess predictive validity. The study results indicated good model fit and measurement invariance for the QEO construct. The structural model for race, personal history characteristics, and QEO indicated moderate model fit. It also indicated interaction effects for race by gender and for race by public support. The MIMIC model results suggest that QEO decreased for Blacks and Hispanics compared to Whites. Furthermore, the MIMIC results, which utilized QEO as an endogenous variable, differed from the multiple regression findings, which utilized one criterion. The multiple regression findings indicated no statistically significant difference between Blacks and Whites. The path model for race and VR closure status indicated poor model fit. The logistic regression indicated no racial/ethnic differences in VR closure status. Several model estimates did not cross-validate. Study limitations and suggestions for future research are described.
